    Dear SDS Carmen,

    Greetings!

    I am Daisy Jane E. Martinote, of legal age, single, a resident of Purok 1B, Poblacion,
    Barobo, Surigao del Sur. I am a registered nurse and a graduate of Bachelor of Science
    in Nursing at Davao Doctors College, General Malvar St., Davao City in the year 2010
    and passed the Nurses Licensure Examination on the following year 2011.

    It came to my knowledge through the official website of DepEd Sursur that the position
    for Admiistrative Officer II is open which I am much eager to apply. I am currently
    working as an Administrative Assistant III-Bookkeeper Designate at Gamut National
    High School, Gamut, Barobo, Surigao del Sur.

    With regards to my job experience, I am previously connected at San Francisco Doctors


    Hospital Inc., Barangay Hubang, San Francisco, Agusan del Sur, where I honed my
    leadership and supervisory skills where I function as Nurse Supervisor in the following
    areas: Renal Dialysis Unit (RDU), Operating Room (OR), Delivery Room (DR), Post-
    Anesthesia Care Unit (PACU) and Neonatal Care Unit (NICU).

    I have also worked as a community health nurse under the Department of Health project
    RNHeals and was assigned in the Municipal Health Unit of Barobo, Surigao, del Sur
    and has been a staff nurse at Lianga District Hospital, Lianga, Surigao del Sur. I am
    excited and confident to be given a chance to work and contribute my knowledge and
    skills to this area in the academe.
