I, JEFF RENZ C. BALBUENA, Filipino, of legal age, married to Lillian F.
Balbuena and a resident of Pawili, Bula, Camarines, after being sworn in accordance with law, hereby depose and say:
1. That I am the holder of a Military Id Card number 877810 issued by
the Armed Forces of the Philippines. 2. That I used to secure and put it into my wallet, among others; 3. That sometime on October 22, 2020, when I arrived home from our duty, I discovered that my Military Id was no longer in my possession. 4. That after diligent effort to locate the same had proved futile; 5. That said Military ID was not confiscated by the authorities of the Armed forces of the Philippines by reason of any violation of their rules and regulations; 6. That I am executing this affidavit to attest the truth of the foregoing facts and to request for a replacement of said Military ID.
