    AFFIDAVIT OF LOSS

    I, FELINA C. SARTE, Filipino, of legal age, married and a resident


    of Vallehermoso, Carmen, Bohol, Philippines, after having been duly sworn
    to in accordance with law, do hereby depose and say:

    1. That I am a bonafide member of the MANILA TEACHERS’


    SAVINGS AND LOAN ASSOCIATION, INC. Tagbilaran City , Bohol,
    Philippines;

    2. That by virtue thereof I was issued a savings and loan PASSBOOK


    by the above mentioned institution;

    3. That sometime on March 2, 2020, our residence located in


    Vallehermoso, Carmen, Bohol was burned and destroyed by fire including
    the said PASSBOOK;

    4. That the above-mentioned passbook is not in the hands of third


    persons for monetary consideration whatsoever but is in fact lost as above-
    stated or said item confiscated for any legal cause;

    5. That I am executing this affidavit to attest to the truth of the


    foregoing statements for the purpose of its re-issuance and for whatever
    other legal purposes this may serve.
