    AFFIDAVIT OF WAIVER

    I, JAY STEAVEN INOT, of legal age, Filipino, single and presently residing
    at CPG North Avenue, Tagbilaran City, Bohol, after having been duly sworn to
    in accordance with law, hereby depose and state:

    1. That I am currently in the process of enrolling in BIT-International College


    with the course of Bachelor of Science in Maritime Engineering;

    2. That one of the requirements in enrolling in the said course is the Isihara
    Test;

    3. That the result of the Isihara Test indicates that I am suffering from total
    color blindness;

    4. That I hereby request BIT-International College to accept my enrolment,


    despite the result of the Isihara Test;

    5. That I bind myself to save BIT-International College free and


    harmless from any loss or damage that it may incur in connection with
    accepting my enrolment with the said course;

    6. That I am executing this affidavit to attest to the foregoing facts and for
    any legal purpose it may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 8 th day of


    November, 2019, in the City of Tagbilaran, Philippines.
