Congratulations and thank you for considering UnitedHealth Group as your future employer! I am pleased to confirm United
HealthCare Services, Inc.'s or its affiliate's (UnitedHealth Group's) offer of employment to you for the position of Center Manager in
the MedExpress division of Optum Care Solutions.
Included in this letter are all aspects of our employment offer to you. We look forward to you starting on September 7, 2021 to help
us deliver on our mission. This is a target and will be confirmed upon successful completion of your pre−employment screening.
Your Recruiter and/or Service Advisor will be in close contact with you regarding your start date as it approaches.
You are eligible to participate in the OTH Incentive Plan. Details about the plan will be available when you begin employment.

• You will be automatically enrolled at a pre−tax contribution rate of 3% of eligible pay as of your second pay date following
your hire date unless you elect a different contribution level or opt out of the plan (by selecting a 0% rate) by 3 p.m. CT on
the Wednesday after your first pay date following your hire date under the plan. You will have access to your account at
Fidelity, the plan's record keeper, as soon as administratively possible following your date of hire. You may make a
change to your contribution rate or investment election in the plan, or opt out by logging in to www.NetBenefits.com or
calling 800−624−4015.
• If you do not make an investment election, your contributions will be invested in a Target Date Fund, based on your date
of birth and the assumption that you will retire at the age of 65. You can review or change your investment elections at
any time.
• Your contribution rate will automatically increase annually on Feb. 1 by 1%. This will happen unless you opt out of
automatic increases or until you reach the plan designated automatic increase limit, though you may manually elect to
increase your contribution rate at any time.
• Fidelity will send a welcome kit within the first week of employment with additional details.
*If there are any differences between the official plan document for any benefit plan and this summary, the official plan document
will govern.
We are making this offer of employment to you due to your talents and skills, not to gain access to confidential information or other
property of your former employer. You may not use or disclose any confidential information of your former employer in connection
with your work for UnitedHealth Group. You may not bring any confidential documents or proprietary materials with you to
UnitedHealth Group and you may not share any such documents or materials with anyone at UnitedHealth Group.
Upon acceptance of the offer, you will receive a request to provide additional information needed to conduct your pre−employment
screening. You will then receive an email from our screening vendor, HireRight with information regarding your drug test. Your drug
test must be completed within two business days from the delivery of the drug testing email. Due to COVID 19, some collection
sites may require masks or face−coverings, which may or may not be provided by the site. As a result, it is strongly recommended
that you bring appropriate face covering to avoid the potential of being turned away, which could impact your start date. Per the
instructions you will receive from HireRight, please call the collection site prior to arrival to confirm site hours of operation, service

You are being offered a position in which you will have direct client or patient contact; therefore, your offer is also contingent upon
satisfactory completion of Occupational Health Screenings and requirements, as applicable. This may include: screening for
tuberculosis (TB); immunity to measles, mumps, rubella, varicella and Hepatitis B; a nonDOT physical; vision examination; and/or
proof of vaccination(s).
Your TB screening will be an IGRA test, which is a blood draw. If a blood draw is not available in your area, then you will be
scheduled for two TB skin tests. HireRight will send you an email with directions to schedule your blood draw to screen for TB and
immunity to measles, mumps, rubella, varicella and Hepatitis B. This screening and any other Occupational Health screenings (if
applicable) must be completed prior to beginning your employment with UnitedHealth Group.
If you are in the process of receiving the COVID−19 vaccination, the CDC advises TB testing to be conducted 4 weeks after
completion of the 2nd COVID−19 vaccination (Pfizer or Moderna) or only dose of Johnson & Johnson COVID−19 vaccination.
Delays in testing will not impact your target start date. After the 4 week waiting period, you will be contacted with instructions to
complete your TB testing.
As a condition of your employment, you must agree to be bound by the terms of UnitedHealth Group's Employment Arbitration
Policy, accessible here. The Arbitration Policy is a binding contract between you and UnitedHealth Group to resolve through
arbitration all covered employment−related disputes that are based on a legal claim, and mutually waive the right to a trial before a
judge or jury in court in favor of final and binding arbitration. By accepting employment with UnitedHealth Group, you agree to be
bound by the terms of the Arbitration Policy.
Consistent with UnitedHealth Group's emphasis on ethical business practices and value of integrity, this offer of employment is
contingent on satisfactory completion of our pre−employment background investigation and drug testing prior to your start date.
This offer is contingent on you having, or obtaining within the required timeframe, any state and/or national certification that
UnitedHealth Group requires for your position.
This offer is also contingent upon your ability to undertake and perform the responsibilities and requirements of the offered position
without violating the terms of any restrictive covenant or confidentiality agreement to which you may be a party.
By accepting employment, you will have agreed to all terms of this offer letter and its attachments.
