    JOINT AFFIDAVIT OF
    TWO DISINTERESTED PERSON

    We, HERWELINA RETUBADO-ANCAO, of legal age, Filipino citizen, married


    and resident of Binabag, Bogo City, Cebu and ESTILITA U. TAMPUS, of legal age,
    Filipino citizen, widow and resident of Carbon, Bogo City, Cebu after being duly sworn
    to in accordance with law, hereby depose and state:

    1. That we have personally known the late SERGIO ULTIANO PADUGA, being
    our friend for a long period of time;

    2. That we hereby declare, confirm and affirm that the correct date of marriage of
    Sergio Ultiano Paduga was “May 25, 2002”, however in the Certificate of Live
    Birth of his daughter Meljie Luaton Paduga, was erroneously entered as “May
    25, 2001”;

    3. That the reason of the discrepancy of date of marriage was due to typo graphical
    error;

    4. That the person name Sergio Ultiano Paduga with date of marriage on May 25,
    2002 and Sergio Ultiano Paduga with date of marriage on May 25, 2001, refer to
    one and the same person;

    5. That we are not related to her by blood or by affinity nor we received any
    monetary consideration from her family;

    6. As such, we are executing this Affidavit to attest to the truth of the foregoing.

    IN WITNESS WHEREOF, we have hereunto set my hand this ________ at Bogo City,
    Cebu, Philippines.
