    JOINT AFFIDAVIT

    We, BARBARA P. LEQUIGAN ,of legal age, Filipino citizen, widow and resident of
    Polambato, Bogo City, Cebu, and LUZ R. NEIS, of legal age, Filipino citizen, widow and
    resident of Cogon, Bogo City, Cebu after being duly sworn to in accordance with law, hereby
    depose and state:

    1. That we have personally known the late AVELINO YLANAN VERALLO, being our
    family friend and neighbour for a long period of time;

    2. That we know for the fact that the name “AVELINO YLANAN VERALLO” was
    correctly entered in the Certificate of Marriage, however in the Certificate of Death was
    entered as “AVELINO Y. VERALLO”, furthermore the name AVELINO VERALLO,
    was erroneously entered in the Social Security System (SSS) Member Data Record
    without the middle name “YLANAN” ;

    3. That the reason of the discrepancy of names was due to the negligence of the person
    who filled up the SSS E1 form and the person who filled up the certificate of death
    shorten the middle name “YLANAN” to “Y.”;

    4. That the name AVELINO YLANAN VERALLO, AVELINO Y. VERALLO and the
    name AVELINO VERALLO, refer to one and the same person;

    5. That we likewise know that the name “MA. BELINDA M. VERALLO”, as appearing in
    the Certificate of Marriage and the name “BELINDA M. VERALLO” appearing in her
    Senior Citizen’s Identification Card and other public documents; refer to one and the
    same person;

    6. That the reason of the discrepancy of names was due to typo graphical error;

    7. That the name MA. BELINDA M. VERALLO and BELINDA M. VERALLO, refer to
    one and the same person

    8. That we are not related to her by blood or by affinity nor we received any monetary
    consideration from her family;

    9. As such, we are executing this Affidavit to attest to the truth of the foregoing.

    IN WITNESS WHEREOF, we have hereunto set our hands this ________ at Bogo City, Cebu,
    Philippines.
