The scale is a 10-item Likert scale with items answered on a four point scale--from
strongly agree to strongly disagree.
Instructions: Below is a list of statements dealing with your general feelings about
yourself. If you strongly agree, circle SA; if you agree with the statement, circle A; if
you disagree, circle D; and, if you strongly disagree, circle SD.
Scoring:
-For questions 1, 3, 4, 7, and 10 score SA=3, A=2, D=1, and SD=0: Your Total
-For questions 2, 5, 6, 8, and 9 score SA=0, A=1, D=2, and SD=3: Your Total
