Abstract
【解決手段】流体送出エレメントを含むデバイスを前立腺内の尿道の内腔内に位置決めする。流体流を、流体送出エレメントから外方に尿道内腔の壁に向かって差し向ける。流体送出エレメントを移動して流体流で壁に亘ってスキャンし、内腔を取り囲む所定容積の組織を除去する。流体を治療物質又は切除効率を高める物質と組み合わせてもよい。比較的固い組織に損傷を加えずに軟質組織が除去されるように組織を選択的に切除するため、流体力を調節してもよい。尿道内に作業空間を形成するため、尿道の治療領域にガスを注入するため、別の流体を導入してもよい。

Description
本発明には、添付図面を参照して読むことにより、本発明の以下の詳細な説明及び添付の特許請求の範囲から明らかになるこの他の利点及び特徴がある。
確かに、流体送出エレメントの特定の大きさ(ノズル直径等)が与えられると、各組織の種類は、代表的には、流体流源の臨界圧力(以下、Pcritとも呼ぶ)を有する。臨界圧力以下では切除速度がゼロに近付き、それ以上では切除速度が全体に単調に、恐らくは指数関数的に増大する。詳細には、組織組成の相違により、流体流源の圧力は、特定の種類の組織を選択的に切除し、臨界圧力が比較的高い他の種類の組織をほぼ無傷で残す。
例1:様々な腎臓組織組成の例示の臨界圧力
組織の臨界圧力を豚の腎臓で計測した。腎臓組織は、その組成が前立腺組織と似ているために選択された。組織の切除に直径が約200μmの柱状流体流を使用した。腺組織(腎臓のピンク色の外部分)は非常に柔らかく、指の圧力で容易に切れてしまうが、腎臓の内部は比較的丈夫な血管組織を含む。この流体流に関する腺組織の臨界圧力は、約5.624kg/cm2(約80psi)であり、血管組織の臨界圧力は、約35.15kg/cm2(約500psi)であることがわかった。以下の表1を参照されたい。
例2:末広がり流体流を使用する組織貫通。イヌの前立腺嚢の組織の切除で計測されたデータを図11に示す。嚢の貫通時間を組織から流体送出エレメントまでの距離の関数として計測した。流体流の開き角は約7°であった。貫通時間を、嚢の貫通に要した時間としてプロットした。嚢の厚さは1mm未満である。
作業空間WSの形成後、キャリヤチューブ380を細長いエレメント310に対して軸線方向並進、回転、揺動、又は回転揺動でき、作業空間WS内で露になり、もはや流体送出エレメント320は周囲組織Tによって塞がれていない。この時点で、デバイスは、第2流体331を流体流として送出し、周囲組織Tを切除することによって治療を有効に開始できる。

- 前立腺の経尿道的治療を行うための組織修正デバイスにおいて、
近位端及び遠位端を有し、尿道に挿入されるように形成された細長いエレメントと、
第1流体を尿道内に送出し、作業空間を形成するように形成された、前記細長いエレメント内の第1内腔と、
前記細長いエレメント内に配置された、尿道を取り囲む前立腺組織の容積を切除する流体流として前記第1流体と異なる第2流体を放出するように形成された、流体送出エレメントと、
前記シャフトが尿道内にあるとき、陰茎の基部と係合するようになった外部アンカーフレームとを含む、デバイス。 - 請求項1に記載のデバイスにおいて、

前記流体流の内部を通してエネルギを伝達し、前記組織を焼灼するように形成されたエネルギ導管を含み、前記デバイスは、前記流体流の力を、前記エネルギを伝達するには十分であるが前記組織を切除するには不十分なレベルに低下するように形成されている、デバイス。 - 請求項8に記載のデバイスにおいて、更に、
前記流体流の前記内部を通して伝達された前記エネルギを使用して前記組織を切除するように形成されている、デバイス。 - 請求項8に記載のデバイスにおいて、
前記エネルギは、光エネルギ又は高周波エネルギ等の電磁エネルギである、デバイス。 - 請求項1乃至10のうちのいずれか一項に記載のデバイスにおいて、更に、
前記細長いエレメントの前記遠位端の近くに膨張可能なアンカーを含み、前記膨張可能なアンカーは、膀胱内で膨張し、切除中に前記細長いエレメントを安定するように形成されている、デバイス。 - 請求項1乃至11のうちのいずれか一項に記載のデバイスにおいて、更に、
前記流体送出エレメントの周囲の前記組織に吸引力を加え、切除発生物を除去するため、前記細長いエレメント内に第2内腔を有する、デバイス。 - 請求項12に記載のデバイスにおいて、
前記第2内腔を通して切除発生物を吸引するため、前記第1内腔と前記第2内腔との間に所定の圧力差を維持するように形成されている、デバイス。 - 請求項12に記載のデバイスにおいて、
前記第1内腔は、前記第1流体を前記流体送出エレメントに向かって差し向けるように形成されており、これによって前記流体流の一体性を維持する、デバイス。
