EMERGENCY MEDICINE

Applications are invited for the post of Specialty Doctor in the Emergency Medicine to
commence as soon as possible for 6 months in the first instance, renewable by mutual
consent and satisfactory appraisal.

We are looking for clinically excellent, motivated and enthusiastic doctors. You will join a rota
of 10 middle grade doctors to provide full shift cover. The contract is for 48 hours a week,
with at least one session for CPD. This post would suit a doctor with 12 months Emergency
Medicine experience who is planning a career in Emergency Medicine or another Critical
Care specialty. The successful applicant will be paid on the Speciality Doctor Scale.

The Trust has all the major specialities and a new Cardiothoracic Tertiary Centre opened in
July 2007 and is progressing to achieve full Teaching Hospital status.

The Accident and Emergency Department sees approximately 95,000 new patients a year.
Medical Staff consists of 5 Consultants, 2 ST3/4 doctors, 10 senior doctors (2 Staff Grades
and 8 Clinical Fellows), 12 Foundation Year 2 trainees, 3 GPVTS trainees and 1 ST1/2 ACCS
trainee. It has excellent case mix including paediatrics, major trauma, and critically ill Medical
and Surgical patients, which presents an excellent educational opportunity. The department
facilities include a 4 bedded Resuscitation Room with overhead X-ray facility and dedicated
Children's waiting and examination areas, in addition to Minor injuries and Majors cubicle
areas. Unique initiatives such as Greet and Treat and RAPT contribute to an excellent
environment for staff to provide quality healthcare.

The Emergency Medicine Consultants are committed to providing experience and training to
their staff in all areas of Emergency Medicine. Formal and informal teaching is provided and
educational advancement and audit will be actively encouraged and supervised in working
hours, within a 48-hour rota. There is an active senior doctor teaching programme both
locally and regionally.

Postgraduate qualifications are not essential and study for the membership examinations
(MRCS, MCEM, MRCP) and the life support courses (ALS, ATLS, APLS, MIMMS) will be
supported. ALS, ATLS and in the near future APLS courses are run locally with the
Emergency Department taking the lead in their programmes.

The post is non-resident but single accommodation may be available. In keeping with the
promotion of smoke free environments and better health.

JOB DESCRIPTION
Specialty Doctor EMERGENCY MEDICINE

THE DEPARTMENT

The Accident and Emergency Department has approximately 95,000 new patients each
year. Approximately 20% of new patients are admitted to the in-patient wards. The
emergency Medicine team deal with resuscitation of all adult patients and are part of the
paediatric resuscitation team. The department runs the cardiac arrest team for ED from
9 a.m. to 5 p.m. weekdays to optimise patient management and provide training
opportunities for junior doctors. The Emergency Medicine Consultants oversee the
trauma team, and the Emergency Medicine middle grades are actively involved in this.
There is a daily review clinical for soft tissue clinics and minor fractures run by the
Consultants. The department also runs thrombolysis for acute myocardial infarction
and steps are being taken to liaise with the Cardiothoracic Centre in the management of
cardiac emergencies and thoracic trauma.

Department Facilities:

• 4 bay resuscitation area, one of them dedicated to neonatal/paediatric


resuscitation
• 12 Majors cubicles
• 1 closed gynaecology/obstetrics assessment room
• 1 isolation assessment room
• 2 closed paediatric rooms
• 1 ENT/eye examination room
• 1 Minor theatre/plastering room
• 2 Nurse triage rooms
• 9 Minors assessment cubicles

• Office accommodation for middle grade doctors with computer facilities
• Staff rest room
• 1 body view room (for the deceased)

DUTIES OF THE POST

The post hold will be expected to attend Emergency Medicine cases when on duty.
Periods of duty will be defined in accordance with the rota prepared by the Medical
Officer in charge. Staff will be contracted to work on an average 48 hours per week,
undertaking day and night duties on a rota basis. Hours of work are subject to the
requirements of the service; alterations may have to be made to the rota, in advance
or at short notice to cover sickness and absence of colleagues.

Duties will involve:

• The assessment, resuscitation, management and safe transfer (where


necessary) of seriously ill and injured patients.
• Care of patients on the observation ward as required.
• Referral of patients to in-patient facilities as appropriate.
• Supervision of junior staff.
• Involvement in Emergency Medicine multi-disciplinary teams and meetings.
• Involvement in proactive patient management to achieve timely and high
quality assessment and management

• Teaching and supervision of junior medical staff and nursing staff and active
participation in teaching.
• Active involvement in audit and the development of departmental policies.
• Play an active role in the day-to-day management and administration of the
Accident and Emergency department.

The post holder will be required to work Bank Holidays, as per the rota but time off in
lieu will be received. Internal cover is built into the middle grade rota for annual leave
and study leave. The post holder undertakes to be available for such irregular
commitments outside normal rostered duties as are essential for the continuity of
patient care.

POSTGRADUATE MEDICAL EDUCATION AND TRAINING

There is an excellent and active Medical Education Centre supported by a


comprehensive library of periodicals and reference material with facilities for on-line
retrieval of research literature and a regional audio-visual library for training
purposes. A wide variety of courses, tutorials and seminars with local and visiting
speakers are held under the auspices of the Postgraduate Clinical Tutor, for clinical
staff of all disciplines, including general practitioners. All categories of medical staff
are able to meet at lunch facilities within the centre.

The successful applicant will be expected to take an active role in the postgraduate
teaching of medical staff including medical students from The Royal Free and
University Colleges, London and will be encouraged to participate in undergraduate
teaching and in the training of nursing and paramedical staff as appropriate.
Teaching sessions in Emergency Medicine are held on a weekly basis.

The successful candidate will be encouraged to utilise study leave to pursue their
Continued Medical Education.

AUDIT AND RESEARCH

The post holder will be expected to carry out at least one Audit project each year.
The department intends to establish a research programme. And the post holder will
be expected to participate in research projects, and take an active part in in-house
teaching to junior medical staff, Medical Students and ENPs.

ADMINISTRATION

No formal administration duties are involved. The post holder may be asked to
review x-rays, Path reports returned to A & E and management of rota schedules.

JOB PLAN

The job plan is based on 48 hours per week, and is based on 24 hour working.

MAIN CONDITIONS OF SERVICE

• In accordance with the Terms and conditions of service for specialty doctors –
England (2008)
• Whole time appointment of 12 programmed activities per week

CONDITIONS OF APPOINTMENT

The doctor accepts that he/she will also perform duties in occasional emergencies
and unforeseen circumstances at the request of the appropriate Consultant, in
consultation (where practicable) with senior colleagues. It has been agreed between
the professions and the Department that while doctors accept that they will perform
such duties, the Secretary of State stresses the additional commitment arising under
this sub-section are exceptional and in particular that doctors should not be required
to undertake work of this kind for prolonged periods or on a regular basis.

The person appointed should expect that duties may be re-allocated from time to
time to fit in with alterations in circumstances, such as changes in staffing,
introduction of new services expansion and reduction of existing services.
