MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This agreement was entered into between LADY OF LOURDES HOSPITAL AND
COLLEGES OF CAYBIGA INC., COLLEGE OF BUSINESS ADMINISTRATION, with office
address on # 15 General Luis St. Caybiga Caloocan City Philippines, a duly recognized institution by the
Commission on Higher Education (CHED), represented in this agreement by Mr. Laurence P.
Manaois (Admin Officer / Practicum Coordinator) and hereinafter known as the “SCHOOL”;

and Mary Rose T. Dogaojo a bonafide student of the school as mentioned above with the occurrence of
his/her legal guardian whose name and signature appear on this document and are hereinafter known as
the “PRACTICUMER”;

AND

MC Hotel Fairview, a duly registered Company/Corporation with the office appear address at Blk 144
lot 5 Regalado Avenue Brgy. Greater Lagro, Quezon City. represented in this agreement by (Radel
Matalote)and hereinafter referred to the “COMPANY”.

WITNESSETH:

WHEREAS, SCHOOL is a duly Recognized Higher Education Institution (HEI) that offers
the course Bachelor of Science in Hospitality Management with a curriculum requirement that the
students enrolled in the abovementioned program undergo Experiential Learning (EL) otherwise
known as On Job Training (OJT) or Practicum.

WHEREAS, COMPANY is a duly registered corporation or business entity engaged in

is willing, able, committed, and ready to provide the On Job Training.

NOW, THEREFORE, in connection with the preceding premises, and their mutual covenants
set forth hereafter, the parties have agreed as follows:

1. That the SCHOOL, represented by the Dean/ Department Head/ Program Coordinator/
Practicum Coordinator, about the Experiential Learning Program shall undertake the following
responsibilities:
2.1. Communicate and coordinate with the company representative or immediate superior of the
Practicumer about matters related to the required experiential learning objectives of the latter;
2.2. Provide the Practicumer with program orientations concerning its objectives, to wit; target
learning outcomes, program final output, and evaluation after the program and even behavioral
conduct expectations while undergoing the Experiential Learning Program;
2.3. Make periodic checks of the Practicumer, either through online or telephone calls to monitor
his/her progress;
2.4. Require the students to submit their practicum progress reports during a scheduled class
meeting;
2.5. Provide the company with OJT Evaluation Sheet to be accomplished by the authorized
company representative or the Practicumer’s immediate superior;
2.6. Require and check the Practicumer's final OJT output which is aportfolio or research study on
some of the operational facets of thecompany duty on some of the operational facets of the
company;
2.7. Perform any other related task and activities deemed to be necessary for the successful conduct
of the practicum program.

2. That the COMPANY agrees and promises to provide an Experiential Learning Program and
commits to perform, through the authorized company representatives, the following
responsibilities:

2.1. To provide the Practicumer with a maximum of 600 hours of onsite job experience
and training (knowledge, skills, and right attitudes) that are relevant to their
respective courses and will match the current industry requirements;
2.2. To assign the Practicumer to a Division/ Department/ Section that will match the
work experience required to their respective courses, and under a person that will
serve as the Practicumer’s immediate superior;
2.3. To ascertain the safety and protection of the Practicumer while performing the job
and by refraining from giving them harmful tasks;
2.4. To confirm the Practicumer’s OJT Progress reports by affixing their signature on
the said documents;
2.5. To provide the Practicumer with the essential sets of information that they may
need in the completion of their final OJT output, which is a portfolio or research
study of some operational facets of the company;
2.6. To seek approval in writing, from the school’s representative and/ or the
Practicumer’s parent or guardian in the case that the Practicum will be required to
do overtime work or will be assigned or brought to another place outside of the
original place of work;
2.7. To provide the school’s representative with an honest and objective evaluation of
the Practicumer’s OJT performance before or upon the completion of 600 hours;
2.8. To provide the Practicumer with a certificate of OJT completion. The school’s
representative, however, reserves the right to reduce the 600 OJT hours instead of
some academic or outreach activities that will be required by the college or
department. In such a case, however, the latter will send a formal letter to the
company;
2.9. To accommodate the school’s representative during random checking of the
Practicumer’s progress (either during telephone calls or virtual conversation) and
provide the performer with essential and honest feedback about the Practicum.

3. That the STUDENT/ PRACTICUMER shall undertake the following responsibilities:

3.1. Comply with the requirements and guidelines of the Experiential Learning (EL)
Program;
3.2. Attend the scheduled consultation meetings with the Practicum Coordinators and
submit all the requirements for the Experiential Learning (EL) Program, EL
Progress Reports, and Final Output;
3.3. Observe and follow all the company’s rules, policies, code of conduct, code of
discipline, code of ethics safety procedures, and the like for the effective,
safe, and successful completion of the program;
3.4. Regularly report to the company online for the practicum work and notify the
immediate superior or any key personnel in case of inability to do so for any reason
whatsoever;
3.5. Perform all reasonably OJT/PRACTICUM-related tasks, duties, and
responsibilities assigned by the immediate superior;

4. COMMON PROVISIONS

4.1. That there will be “No Employer-Employee Relationship between the COMPANY
and the PRACTICUMER” and the latter is therefore not entitled to labor standard
benefits within the contemplation of the law;
4.2. Through a written notice, any of the parties have the right to terminate this
Memorandum of Agreement due to the failure of the other party to comply,
observe or follow any of the provisions thereof;
4.3. The SCHOOL and the COMPANY are free from all or any liability, should the
PRACTICUMER suffer illness or sustain an injury while performing the On the
Job Training the if the such injury was sustained due to the failure of the latter to
observe and follow company policies, rules, regulations, safety procedures and the
like;
4.4. The PRACTICUMER and his/her parent or any legal guardian shall accomplish a
waiver for the OJT;
4.5. This agreement shall commence on ______________and shall be deemed
terminated un of the party.
