Abstract
The following is a de-identified case study that presents the assessment, diagnosis, and
(CBT) and Exposure and Response Prevention (ERP). To protect the client’s anonymity, names
and other identifying details have been changed. When Simon presented to treatment, this 23-
year-old White male was struggling to manage OCD checking and rearranging behaviors related
to maintaining a “just right” sensation. He was assessed with a general history interview, the
Compulsive Scale (Y-BOCS), and a battery of self-report assessments. Simon solely met criteria
for OCD and over the course of five months, attended 15 ERP treatment sessions.

concluded at the submission of this report, this client demonstrated significant improvement in
the management of his symptoms and his Y-BOCS scores decreased substantially.

Treatment of OCD:
experience of obsessions and/or compulsions that are clinically significant. Obsessions are
disturbing, recurrent, intrusive thoughts, urges, or images the individual attempts to suppress
or ignore. Compulsions are repetitive behaviors, or mental acts, a person feels driven to perform
to reduce anxiety or prevent a feared outcome. Compulsions are often performed to decrease the
distress prompted by the obsessive thought. Individuals with OCD spend significant time
engaging in obsessions and/or compulsions and must experience distress and/or related
The DSM-V does not require an individual to have both obsessions and compulsions to
meet criteria for diagnosis; however, the incidence of cases without both is very low. In a study
where the Yale-Brown Obsessive Compulsive Scale checklist (Y-BOCS; Goodman et al., 1989)
was used to diagnose individuals, only 4% of the 431 participants did not have both obsessions
and compulsions, and just 2% were identified as primarily having obsessions (Foa et al., 1995).
In fact, current research suggests “pure obsessional types” do not exist and previous research was
incorrectly omitting mental rituals and reassurance seeking as compulsive behaviors, likely due
to the fact that mental rituals (e.g., counting) are not as obvious to researchers as behavioral
rituals (e.g., handwashing; Williams et al., 2011). That is, when mental rituals and reassurance
seeking were included as compulsions, all individuals with Y-BOCS scores that met the cut-off
In 2017, the World Health Organization (WHO), published Depression and other
common mental disorders wherein they listed anxiety disorders, including OCD, as the sixth
largest contributor to non-fatal health loss globally, with OCD also appearing in the top 10
causes of years lost to disability in all WHO regions. OCD is a severe and chronic disorder
executive functioning such as encoding information, switching from task to task, and impulse
control (Jacoby et al., 2014; Snyder et al., 2015). OCD also causes interference in daily
functioning. The National Comorbidity Survey found that in a sample of 2,013 individuals who
reported lifetime OCD, people engaged in and were occupied by their obsessions for 5.9 hours
per day and their compulsions for 4.6 hours per day, on average (Ruscio et al., 2010).
Additionally, 90% of individuals with OCD have a comorbid diagnosis, most commonly anxiety
disorders (e.g., generalized anxiety disorder) followed by mood disorders (e.g., major depressive
disorder), impulse control disorders (e.g. trichotillomania), and substance use disorders (Ruscio
et al., 2010).
The estimated 12-month prevalence of OCD in the United States is approximately 1.2%
with estimates of lifetime prevalence varying between 1.9 to 3.3% (Coles et al., 2018; Ruscio et
al., 2010). While research suggests age at onset (AAO) is important for predicting the clinical
course of OCD, there is a lack of agreement on a clear definition for AAO as there is much
variability in what constitutes the beginning of OCD symptoms in addition to what should be
considered a threshold for ‘adult-onset’ phenotypes (Dell’Osso et al., 2016; Sharma et al., 2015;
Taylor, 2011). AAO for OCD has been shown to be bimodal, with peaks around the age of 14
and then again in early adulthood. In a recent epidemiological study that included 431
individuals with OCD, 57% reported onset prior to 18 years of age (Dell’Osso et al., 2016).

While the incidence of OCD is equal in males and females, the literature indicates gender
differences related to age at onset. Females typically experience a later onset and are affected at a
higher rate during adulthood. Males appear to be more frequently affected in childhood, with
25% of cases appearing prior to age 10 (APA, 2013; Ruscio et al., 2010). An international,
epidemiological study found significant gender differences between pre-adult (before age 18)
and adult-onset groups which resulted in an adulthood ratio of 33.2% males and 66.8% females
(χ2 = 10.9, p < .001). Further analyses uncovered additional gender incongruence when age at
onset was greater than or equal to 40, with 15 of the 18 patients who experienced delayed onset
being female (Dell’Osso et al., 2016). Simon’s presentation aligns with these findings, as he

misinterpretations are not weakened or eliminated, the obsessions are maintained. Several studies

dysfunctional belief systems. First, “inflated responsibility and overestimation of threat” (p.
1528) is described as the individual believing they are solely responsible for preventing their
feared outcome(s). Second, “perfectionism and need for certainty” (p. 1528) involves individuals

who fear small errors and believe they cause catastrophic outcomes leading to a low tolerance for
uncertainty which, like intrusive thoughts, is a very normal part of life. Lastly, “importance and
associated with thought action fusion. Thought action fusion occurs when individuals believe
simply having a thought is the same as carrying out the thought or makes the action they obsess
about more likely to occur. Ultimately, these developments in the conceptualization of OCD
serve as the foundation of treatments which are consistent with the cognitive model, attempting
suppression, the normalcy of intrusive thoughts, and cognitive restructuring (Rachman, 1997).
Prior to 1966, obsessive compulsive disorder was seen as mostly behavioral, and
conceptualized as such in initial models. For example, Mower’s (1960) Two Factor Theory
suggested OCD is developed through classical conditioning, and the principal mechanism
classical conditioning model of OCD suggests a stimulus that naturally causes anxiety—labeled
otherwise inherently harmless—labeled the Neutral Stimulus (NS). Once the association between
the UCS and NS has been established through singular or repeated pairing, the previously neutral
stimulus will continue to provoke anxiety for the individual whenever they interact with it and
becomes the Conditioned Stimulus (CS). In hopes of avoiding the feelings of anxiety and
discomfort, the individual then engages in a compulsive behavior or ritual that only leads to
short-term relief.

Operant Conditioning has been used to explain how relationships between obsessions and
compulsions are maintained. Through a process of negative reinforcement, the removal of the
anxiety acts as the reinforcing agent that makes the person more likely to continue engaging in
this ritual in the future in order to feel the same anxiety reduction. Because the compulsions
become habitual in nature, extinction of the irrational fear is not able to occur as the individual
cannot learn that the experience or thought (obsession) is not dangerous. Figure 1 below
demonstrates Mower’s (1960) proposed model demonstrating the relationship between classical
and operant conditioning in the initiation and maintenance of OCD symptoms, using Simon’s
presentation as an example.

Foa and Kozak (1986) also proposed a learning model within emotional processing
theory. This theory proposed two types of fear structures in memory: a normal/adaptive structure
associations that are based in reality and fear of stimuli which are innately dangerous. For
example, an individual knows bears are dangerous to humans, so seeing one in the woods
activates the fear structure which, in turn, leads to the adaptive response of slowly backing away
from the bear. On the other hand, maladaptive structures are not based in reality and include
associations that cause avoidance of safe stimuli. These structures are the underlying mechanism
of not only OCD, but all anxiety disorders (Foa & Kozak, 1986; Foa et al., 2006). For example,
an individual with OCD might believe that stepping on a crack in the sidewalk will cause a loved
one to die, which could result in the maladaptive response of avoiding sidewalks altogether.
Because this person always avoids sidewalk cracks, and no loved ones ever die, this mere

individual must activate the maladaptive fear structures through exposure. During exposure, the
theory stresses the importance of within and between session fear reduction (i.e. habituation)
(Arch & Abramowitz, 2015; Foa et al., 2006). Foa and colleagues (1986) suggest habituation
allows memory structures to be modified, resulting in the extinction of the conditioned, irrational
obsessional stimuli. In essence, the anxiety no longer occurs or becomes more manageable, and,
therefore, the individual is able to resist engaging in the avoidance/compulsion that previously
However, more recent research has shown that some of the basic assumptions of
emotional processing theory are not well supported (Craske et al., 2008, 2012). Researchers have
habituation in explaining fear extinction; however, these principles do not explain spontaneous
recovery or reinstatement of fear which can often occur within treatment. They also do not
account for the context dependency of extinction or rapid reacquisition of the fear. Recent
research is demonstrating that an inhibitory model of fear reduction shows more promise in
treatment protocols for OCD, such as exposure and response prevention (ERP). However,
current research suggests that behavioral theories do not fully explain the etiology and
maintenance of OCD. Many individuals suffering with this disorder do not appear to have a
history of conditioning experiences that would have caused their obsessional fears (Taylor et al.,
2007). In addition, classic behaviorism suggests that creating new associations through exposure
and the resulting habituation should consistently lead to a reduction in symptomology, but this is
not supported by the research. Several studies have demonstrated successful habituation does not

and response prevention (ERP), developed based on the behavioral understanding of OCD
(Berman et al., 2017; Rector et al., 2019). ERP was originally developed from learning theory
with the efficacy of the treatment being understood through an emotional processing theory lens

(Foa & Kozak, 1986; Foa et al., 2006). More recent research, however, suggests the mechanism
of this treatment is best conceptualized within the inhibitory learning model (Craske et al., 2012).
The inhibitory learning model has furthered our understanding about the establishment
and extinction of learned associations. Individuals with OCD and other anxiety disorders are
considered to have deficits in inhibitory learning; therefore, these advances are relevant for
enhancing the efficacy of exposure therapy (Craske et al., 2012). Furthermore, the inhibitory
learning model postulates the association between the Conditioned Stimulus (CS) and the
Unconditioned Stimulus (US) that leads to a fear response is not eliminated during extinction as
previously posited (Bouton, 1993). Alternatively, secondary inhibitory learning about the CS-US
relationship develops. Specifically, the CS now contains two meanings: the new-found inhibitory

The inhibitory learning model informs our understanding of how new learning occurs
during exposures and has strong practical implications for clinicians when designing exposures
that will have maximum effectiveness and generalizability for individuals with OCD. Craske and
colleagues (2012) outlined several methods for maximizing exposures such as: 1) designing
exposure trials so they dispute existing beliefs; 2) varying stimuli, duration, and intensity of the
exposure; and 3) maintaining high levels of anxiety during exposures by removing safety
signals/behaviors. However, within this model, habituation is not emphasized during in-session
exposures. Continuing with the stepping on cracks example, ways in which an in vivo exposure
for this fear could be maximized include directly stepping on cracks and evaluating what
happens as a result to help dispute their belief that something bad will happen; gradually
increasing how many cracks are stepped on or stepping on cracks in different locations, and
eliminating safety behaviors such as closing their eyes or engaging in breathing exercises which
would reduce anxiety level. Within each exposure, response prevention is also important in
ensuring that learning is maximized and that fear/anxiety reduction cannot be attributed to a
compulsion or ritual.
The response prevention (RP) component of CBT + ERP, involves instructing the client
to not engage in compulsions or rituals of any sort. This is important because clients perceive the
rituals as preventing the feared outcome from occurring (Williams et al, 2013). Only by stopping
the rituals can clients learn that rituals do not protect them from their obsessional concerns.

Treatment for OCD includes implementing response prevention both during exposures as well as
individual RP goals outside of exposure. At the beginning of treatment, it can be helpful to begin
implementing response prevention goals to target disproving their necessity, even before
implementing exposures. For example, for someone whose OCD involves contamination
concerns, an early response prevention goal could include resisting handwashing outside of times
it would be deemed appropriate (e.g. after using the restroom, before preparing food). In this
example, the clint is not engaging in exposure as they are not purposefully bringing on anxiety
by touching something they view as contaminated, but are simply asked to resist an OCD urge
that commonly occurs. Once exposure work begins, specific RP goals are set for each exposure
are performed to alleviate an obsession, or thought, that prompts anxiety. However, some
patients with OCD, such as Simon, also report compulsions preceded not by obsessions but by an
uncomfortable, distressing subjective experience (Ferrão et al., 2012). These sensory phenomena
have previously been referred to as “premonitory urges,” “sensory tics,” “just right perceptions,”
Tourette’s syndrome (Ferrão et al., 2012; Prado et al., 2014). More recently they have been tied
to OCD and termed “not just right experiences” or NJREs (Coles et al., 2003; 2005). The term
“incompleteness” is included within NJREs and is described more specifically as the sense that a
behavior has not been fully executed. Research has shown that NJREs most commonly occur in
association with symptoms related to ordering, arranging, checking, and symmetry and can be
less responsive to CBT/ERP than more traditional presentations of OCD (Coles et al., 2003; Foa,

et al., 1999). Simon fits with this description of NJREs as he could often not articulate a negative
outcome that is prevented by performing rituals, rather the performance of the ritual feels “just
right.” In this case, the function of the ritual is to reduce anxiety or discomfort, and the disastrous
consequence is psychological, such as not being able to handle the discomfort or rather being
Since 1966, Cognitive Behavioral Therapy (CBT) has become the most commonly used
treatment for OCD due to the large amount of empirical support for its efficacy (Arch &
Abramowitz, 2015; Law & Boisseau, 2019; McKay et al., 2015; Rector et al., 2019). CBT for
OCD targets behavior through Exposure and Response Prevention (ERP) combined with
elements of Cognitive Therapy (CT) for a well-rounded approach. Multiple meta-analyses have
found both ERP and CT to be equally effective on their own (Eddy et al., 2004; Rosa-Alcázar, et
al., 2008; Ost et al., 2015). Controlled clinical trials, however, have repeatedly shown CBT can
significantly reduce OCD symptoms beyond what has been found when applying CT or ERP by
themselves (Anderson & Rees, 2007; Bolton & Perrin, 2008; Franklin & Foa, 2002; Rufer et al.,
2006). Consequently, the combination of CT and ERP (i.e. CBT) has become the treatment of
choice for OCD to ensure a cohesive and comprehensive approach that increases the likelihood
cognitive-behavioral treatment for individuals with OCD when comparing ERP and cognitive
restructuring (CR), both alone and combined. They evaluated 19 studies published between 1980
and 2006 and found large effect sizes with ERP alone (d = 1.127), CR alone (d = 1.090), and

ERP combined with CR (d = 0.998). Based on these results, patients showed improvement
within each treatment modality and would likely benefit equally from ERP/CR alone or
combined. The study also showed that therapist-guided exposure was superior when compared to
therapist-assisted self-exposure and a combination of in-vivo and imaginal exposure was better
trials (RCTs) that provided CBT to individuals confirmed to have OCD and compared treatment
outcomes to a control group (e.g., psychological placebo, waitlist, pill placebo). The 16 studies
included in the meta-analysis had a total sample size of 756 participants. The use of CBT
produced a large effect size (g = 1.39), and out-performed control conditions on primary OCD
participants’ comorbidities, such as depression, also demonstrated improvement for the CBT
condition (g = 0.5).
An even more extensive meta-analysis was conducted by Ost et al. (2015) comparing
several treatments and their efficacy for OCD. They evaluated CBT/ERP, CT alone,
interventions were employed simultaneously. The analysis examined 37 studies totaling 2,041
participants. They found every treatment produced statistically significant improvement in OCD
symptoms, but CBT/ERP produced the largest effect sizes when compared to both waitlist (g =
1.31) and placebo conditions (g = 1.33). Finally, CBT/ERP was found to be significantly more

efficacious treatments with the reduction of logistical barriers and stigma (Brand & McKay,
2012).
Research supports the use of telehealth for delivery of CBT and has been shown to be an
effective substitute to face-to-face treatment. Brand and McKay (2012) reported encouraging
findings when they compared the efficacy of CBT/ERP for obsessive-compulsive disorder when
delivered in person compared to via telehealth. Based on their findings, telehealth treatment for
OCD appears to be an efficacious alternative to face-to-face treatment (effect sizes of 1.41 and
0.91-2.32, respectively). In wake of the Covid-19 pandemic, wherein treatment delivery relied
colleagues (2021) demonstrated similar findings in comparing telehealth to face to face treatment
for anxiety disorders. They included 5 RCTs, with three utilizing video and two utilizing
telephone for telehealth services. Overall, they found comparable treatment outcomes including
Research has not merely shown telehealth to be comparable to face-to-face services in the
treatment of OCD but suggests telehealth may be the preferred format for some client
presentations. Specifically, utilizing telehealth to treat OCD allows access to treatment within the
client’s environment where symptoms are often the most disruptive (e.g., at home). That is, when
treatment is administered in the specific setting where symptoms occur, its efficacy is
significantly enhanced as exposures are more salient (McKay et al., 2010). Lovell et al (2006)
compared ERP conducted over the telephone to face-to-face ERP for 72 patients with OCD and
found both treatments to be similarly effective for the treatment of OCD. Four follow-ups were
conducted by researchers (immediately after treatment, and at one, three, and six months) which
included the administration of the Y-BOCS Severity Rating to measure clinical outcomes. Scores

were equivalent for both groups at each follow-up. Similarly, several RCT’s for prolonged
exposure delivered via telehealth (synchronous videoconferencing) showed telehealth did not
reduce efficacy of treatment when compared to in-person services (Acierno, Gros, Ruggiero et
al., 2016; Acierno, Knapp, Tuerk, et al., 2017). Overall, current available literature points to the
efficacy of exposure treatment via telehealth and demonstrates equal treatment outcomes when

presentations of OCD, which include specific obsessions and feared outcomes, versus
individuals with OCD who could not articulate a specific feared outcome, such as those with
NJREs. They found that the patients who could articulate their feared outcomes had lower Y-
BOCS scores after treatment when compared to individuals who could not articulate a feared
disconfirmation of the expected harm during exposures. While CBT has shown lower efficacy in
treating NJREs when compared to other individuals with OCD, it has still demonstrated overall
Coles and Ravid (2016) examined the clinical presentation of NJREs in patients with and
without OCD diagnoses and evaluated the efficacy of CBT/ERP treatment. They found
individuals with OCD reported both significantly more, and being more distressed by, NJREs
compared to anxious and unselected controls, confirming the association between feelings of
incompleteness/NJREs and OCD. They also found that, after completing treatment, patients with
OCD reported significantly fewer NJREs and experienced less distress associated with them

when compared to before treatment. Therefore, while exposure-based therapy is often used to
demonstrate the low likelihood of a feared outcome occurring and typically demonstrates better
treatment outcomes in these cases, it can also be effective in treating OCD that is maintained by
Identifying Information
For the purpose of protecting the client’s anonymity, the name “Simon” is used in this
document rather than the client’s real name. Simon is a White male in his early 20s who lives
with his parents in a midsize city in the Midwestern United States. He works as a software
engineer at a large corporation, and his income places him in the SES category of middle class.
Presenting Problem
Simon presented for treatment during the fall of 2021 with chief complaints related to
repeated checking and “just right” behaviors present since middle school but had worsened in the
few months leading up to his initial appointment. He also reported spending long periods of time
adjusting items in his environment until they are in the “just right” place/positions, excessively
making to-do lists, and repeating things in his head or out loud until they feel just right. He cited
stress of graduating college and beginning full-time work as a possible stressor leading to
worsening symptoms. Simon noted telling a friend on New Year’s Eve one year during early
adolescence that he was going to be “perfect.” While Simon could not recall exactly what
prompted this need to be perfect, he reported symptoms have been present since. While Simon
stated these symptoms do not cause much interference with work or relationships, he reported
the symptoms cause significant amounts of emotional distress and take up most of his time
outside of work, thus not allowing for him to relax or engage in any hobbies.

Psychiatric/Medical History
Simon presented to a group practice setting that specializes in the treatment of anxiety-
related disorders. He had previously received treatment for symptoms related to OCD from a
psychologist within his pediatrician’s office in middle school for about a year. The only specific
therapy technique Simon remembered from this experience was a behavioral plan instituted by
his parents to help incentivize resisting compulsions. His recollection was that this intervention
did not help reduce his OCD symptoms. Simon denied a family history of mental illness. He also
Simon recently graduated with a bachelor’s degree from a large public university. He
began working as a software engineer after graduating, which is the first job he has ever had.
Prior to college, Simon reported a strong academic history having maintained a high GPA
throughout all of high school. He also reported being highly involved with sports in high school.
He enjoys spending time with family and friends, watching television, and playing video games.
Assessment
Diagnostic Interviews
In addition to a general clinical interview, Simon was administered the Mini International
Neuropsychiatric Interview version 7.0.2 (MINI-5; Sheehan, 2015) and the Yale Brown
Obsessive Compulsive Scale (Y-BOCS; Goodman et al., 1989). Following the initial diagnostic
interview and assessment, Simon received the clinic’s standard set of self-report measures via
based diagnostic interview containing questions designed to identify the presence of most of the
major psychiatric disorders. The most recent version, 7.0.2 (MINI-5; Sheehan, 2015), was
utilized in this case study as a diagnostic tool since it has been recently revised to reflect the
diagnostic criteria in the DSM-5. The MINI-5 consists of 17 modules to assess for disorders such
questions administered by a clinician, with the client’s Yes/No responses indicating the presence
or absence of symptoms. Each module opens with a screener question. If the client’s response
indicates symptoms are present in that disorder-specific section, the clinician continues
administration of the module until it is completed or until the client responds with a “no” that
would indicate they do not meet criteria for the disorder for which the module is assessing. Once
the modules have been administered in their entirety, the clinician designates whether the client
is expected to meet criteria for each DSM-5 disorder assessed in the interview. While this
measure is self-report in that the questions are simple yes/no answers, it is administered in an
interview format and is used for the purpose of diagnosing individuals in an efficient and precise
manner. Clinicians are responsible for asking follow up questions to provide clarifications or
assessing for OCD. It has been shown to have excellent inter-rater reliability of kappa = 1.00 and
test/retest reliability of kappa = 0.85 when administered after two days (Sheehan et al., 1998).
Strong convergent validity for the MINI-5 has also been reported with kappa = 0.63 when

compared to the Structured Clinical Interview for DSM-III-R Patients (SCID-P) for diagnoses of
OCD (Sheehan et al., 1998). Additionally, sensitivity and specificity for the MINI-5 when
diagnosing and differentiating those with and without OCD was good to excellent. For
specificity, 98% of the 349 individuals with non-OCD diagnoses were correctly identified as not
having OCD (Sheehan et al., 1998). Sensitivity was lower, with 62% of the 21 individuals in this
sample known to meet criteria for OCD correctly identified (Sheehan et al., 1998).
repeatedly in response to an obsession, and utilizing excessive rituals to reduce distress. His
symptoms caused significant distress and consumed more than one hour per day. He also
demonstrated good insight into his symptoms regarding how they are maintained as well as how
they are interfering with his life. Simon did not endorse any other symptoms assessed by the
MINI-5 as he did not make it past for the first question for any other modules, and thus did not
meet criteria for any other clinical diagnoses. See Appendix A for guidance on how to access the
MINI-5.
clinician-administered, semi-structured interview considered the gold standard for assessing the
presence and severity of clients’ obsessive-compulsive symptoms. Rather than being a diagnostic
tool, it is primarily used to establish baselines pre-treatment and then as a measure of progress in
treatment. The first part of the Y-BOCS is the symptom checklist, which includes 67 items
pertaining to obsessions, compulsions, and avoidance. The symptom checklist is not scored but is

used by the clinician to determine the client’s primary obsessions and compulsions. The second
part is the severity scale, which includes 10 items (e.g., “How hard do you try to stop the
thoughts or ignore them?”) which are rated by the clinician on a 5-point scale based on the
client’s response. The response options vary to best fit how each question is worded; however,
the most common response option is on a 5-point Likert scale format where 0 = none and 4 =
extreme. Half of the severity questions address obsessions, and the remaining five address
compulsions. Both types of questions identify time spent, interference, resistance, degree of
control, and distress. The Y-BOCS is scored by summing the 10 items from the severity scale
and producing a score that ranges from 0 to 40, with higher scores indicating more severe
symptomology.
While the Y-BOCS is not a diagnostic tool, there are clinical cutoffs that are well-
established from the research (Goodman et al., 1989) and can be used to assign a severity level,
as well as to help track treatment progress. These cutoffs are 0-7 (Subclinical), 8-15 (Mild), 16-
score of ≤ 14 has been shown to be the best predictor of symptom remission, and a score of ≤ 12
the best predictor of wellness when compared to clients’ scores on measures of symptom
The Y-BOCS’ psychometric properties are strong across the literature. Several studies
have shown good internal consistency (α = .87), good test-retest reliability after 48.5 days (rs=
0.81-0.97), and excellent interrater reliability (rs = .93 to .99). (Frost et al., 1995; Goodman et
al., 1989; Kim et al., 1990; Woody et al., 1995). Refer to Appendix B to view a copy of the Y-
compulsive behaviors from several content areas. His primary obsessions were the need to
remember and the fear of losing things. Simon’s primary compulsions involved excessive
checking of lists and ordering/arranging items until they feel “just right.” For the Y-BOCS
severity rating, Simon’s pre-treatment score of 26 placed him in the severe range. The Y-BOCS
severity scale was administered monthly to track treatment progress. Table 1 below illustrates the
substantial progress Simon experienced in treatment, as his Y-BOCS score was reduced to a

Following the diagnostic interview, Simon filled out the clinic’s standard battery of self-
report measures to aid in confirmation of diagnosis and treatment planning. He completed these
measures again at the mid-point. Scores on all self-report measures can be found in Table 2.

The short form version of the Depression, Anxiety, and Stress Scale (DASS-21;
Lovibond & Lovibond, 1995) is a self-report measure that assesses the occurrence and severity
of depression, anxiety, and stress within the past week. This measure consists of three subscales:
depression (e.g., “I was unable to be enthusiastic about anything”), anxiety (e.g., “I felt I was
close to panic”), and stress (e.g., “I found it hard to wind down”). The items are evenly
distributed into these subscales and rated on a 4-point Likert scale ranging from 0 = did not apply
to me at all to 3 = applied to me very much, or most of the time. The score for the DASS-21 is
calculated by summing items within each subscale and multiplying by two, thus producing a
range of 0-42, with higher scores indicating more severe distress surrounding the symptoms
Different cutoffs have been established for each subscale within the DASS-21. Lovibond
and Lovibond (1995) identified the following cutoffs for the depression subscale: 0-9 (normal),
10-13 (mild), 14-20 (moderate), 21-27 (severe), 28+ (extremely severe). The anxiety subscale
has cutoffs of: 0-7 (normal), 8-9 (mild), 10-14 (moderate), 15-19 (severe), 20+ (extremely
severe). Finally, cutoffs for the stress subscale are: 0-14 (normal), 15-18 (mild), 19-25
All three subscales on the DASS-21 have demonstrated good internal consistency with
coefficient alphas = 0.97, 0.92, and 0.95 for depression, anxiety, and stress, respectively (Antony
et al., 1998). The DASS has also displayed good convergent validity, with the anxiety scale
being strongly correlated with the Beck Anxiety Inventory (r = .85) and depression scale being
strongly correlated with the Beck Depression Inventory (r = .79; Antony et al., 1998). Refer to

The Sheehan Disability Scale (SDS; Sheehan, 1983) was developed to measure how
work, social life, and family. Respondents are required to designate to what degree symptoms
have disrupted their activities in the previous week. The measure consists of three items, one per
domain, which are rated from 0 = no disruption in activities to 10 = extremely disrupted. The
three items are then summed to produce an overall score from 0 (unimpaired) to 30 (highly
impaired) which indicates functional impairment. In addition, another two items, which also
refer to the last week and are not included in the final score, inquire about the number of days of
work and/or school missed due to symptoms and how many symptoms led to them feeling
unproductive.
There are no formal norms or cutoff scores for the SDS; however, Sheehan (1983)
recommended that scores of 5 or greater in any area should receive special attention as they
indicate significant functional impairment in that area. The SDS has demonstrated good internal
consistency reliability (α = .89) in a sample of primary care outpatients who were assessed for
psychological disorders (Leon et al., 1997). Convergent validity has been demonstrated by strong
relationships with other measures of functional impairment, while divergent validity has been
Chinese individuals seeking psychotherapy (Zeljic et al., 2020). Refer to Appendix D to view the

in the following dimensions: washing (e.g. “I wash my hands more often and for longer than
necessary”), checking (e.g. “I repeatedly check gas and water taps and light switches after
turning them off”), ordering (e.g. “I get upset if others change the way I have arranged things”),
obsessing (e.g., “I am upset by unpleasant thoughts that come into my mind against my will”),
hoarding (e.g., “I have saved up so many things that they get in the way”) and mental
neutralizing (e.g., “I feel I have to repeat certain numbers”). The OCI-R contains 18 self-report
items rated on a 5-point Likert-type scale where 0 = not at all distressed and 4 = extremely
distressed. Total scores computed by summing rating across all items produces a score between 0
Foa and colleagues (2002) found the OCI-R to be useful as a diagnostic screening tool
due to its efficacy in discriminating between OCD and other anxiety disorders. Sensitivity (65.6)
and specificity (63.4) of the OCI-R were used to determine clinical cutoffs. The mean score for
persons with OCD has been established at 28.0 (SD = 13.53), and the suggested clinical cutoff
score is 21. Therefore, scores above 21 are suggestive of the presence of OCD. Current scientific
literature has established good psychometric properties for the OCI-R. Foa and colleagues (2002)
reported a high two-week test-retest reliability (rs = .74 to .91). Abramowitz and Deacon (2006)
reported high internal consistency for persons with OCD (α = .83) and moderate convergent
validity when comparing OCI-R total scores to the Y-BOCS and the Beck Depression Inventory

Within treatment for OCD and other anxiety related disorders, clinicians often use an
exposure hierarchy (Gillihan et al., 2012). Creating an exposure hierarchy involves collaboration
with the client to create a list of feared situations pertaining to obsessions and/or compulsions.
Hierarchy creation occurs early in treatment in preparation for completing exposures to the
situations listed. When creating the hierarchy, the client and clinician start by discussing what
important for hierarchy items to be specific (e.g., touching doorknob in the hall bathroom). Items
are then rated and ranked, based on the level of anxiety associated with each item.
When assigning anxiety ratings, the client and clinician used a tool known as the
Subjective Units of Distress Scale (SUDS; Wolpe & Lazarus, 1967). This scale is 0-10 and
includes the following anchor points: 0 = no distress, 5 = anxiety is definitely bothersome, but
individual is able to cope, and 10 = worst anxiety the individual has experienced or can imagine
experiencing. SUDS ratings are used to order the anxiety provoking situations to create the
hierarchy. Once exposures begin, SUDS rating are also used to gauge how anxious the client is
before, during, and after the exposure. SUDS ratings and the exposure hierarchy are helpful tools
for treatment planning, monitoring progress throughout treatment, and to measure individual
treatment outcomes.
In developing an exposure hierarchy for Simon, a list of anxiety provoking situations and
respective SUDS was generated during the session following the initial assessment. This list was
used to direct initial in-session exposures and as treatment progress, additional exposure targets

disorders are quite different (APA, 2015; Coles, et al., 2008). During the clinical interview,
Simon endorsed a pattern of preoccupation with orderliness and perfectionism at the expense of
flexibility as indicated by his worries and behaviors surrounding rules, lists, order, and
organization that could potentially indicate an OCPD diagnosis. However, OCPD is not
performed in response to these intrusions. Rather, OCPD involves a persistent and pervasive
maladaptive pattern of extreme perfectionism and rigid control that creates dysfunction within
the context of interpersonal relationships (APA, 2015). Simon’s difficulties with inflexibility
were clearly driven by intrusive thoughts leading to compulsive behavior that resulted in internal
psychological distress. He was able to derive satisfaction from interpersonal relationships, stating
he had several close friends and family and therefore, a diagnosis of OCD was the best fit for his
presentation.
Case Conceptualization

disorder (Barlow, et al., 1996; Adams et al., 2018; Bey et al., 2017). Research has indicated that
for OCD, periods of psychological distress affect symptomology in three ways where: stress can

When Simon presented for treatment, there were several factors maintaining his
symptoms. When feeling anxious or uncomfortable, it is a natural response to attempt to get rid
of that feeling as quickly as possible. However, for individuals with OCD, attempts to get rid of
seeking, and compulsive behaviors work to reduce anxiety, but only short-term; these behaviors
prevent the individuals from learning anxiety will eventually pass, will not be as bad as expected,
they can continue to function despite anxiety, and/or feared outcomes are likely worse than the
distress and discomfort associated with not just right experiences, but these behaviors served to

maintain his symptoms. Simon frequently checked his many lists, repeated conversations in his
head, checked and scanned rooms, arranged items, and gave himself reassurance. His
compulsions served to reduce anxiety that he would forget something as well as get things back
to a “just right” state and reduce the discomfort associated with items or lists being unorganized
or incomplete. He would also often suppress his thoughts for several hours during the day, which
causing an eventual uptick in obsessions and compensatory compulsive behaviors in the evening.

intrusive thought occurs for a person with OCD, he or she tends to pay significant attention to the
thought and may erroneously interpret the thought as especially important or even dangerous.
Simon had strong insight into his symptoms and was able to recognize his intrusive thoughts
were not dangerous; however, he still paid significant attention and labeled his intrusive thoughts
as important. Simon’s intrusive thoughts were focused on fears of possibly forgetting important
individuals with OCD, Simon displayed thinking errors within his intrusive thoughts. For
example, underestimating his ability to cope with uncertainty (e.g. “I won’t be able to handle
feeling like my surroundings are disorganized) and overestimating the likelihood of feared
outcomes (e.g. “If I don’t check where items are located, I will lose them and have to spend

Simon was seen over the course of 5 months and attended a total of 15 CBT/ERP
sessions. Most of these sessions were conducted via televideo due to clinical utility of
completing exposures in his home environment. After several months of treatment, Simon found
it helpful to have more practice with exposure homework between sessions; thus, session

frequency was spaced out to every other week. Treatment was initiated with a diagnostic
interview and subjective assessments, followed by treatment interventions which included self-
monitoring, psychoeducation about OCD and ERP, identifying and creating goals for response
prevention, in-vivo and imaginal exposures, and establishing opportunities for self-directed
exposures.
The first two sessions were centered on assessment, psychoeducation, and introducing
self-monitoring forms. Following the assessment, Simon received psychoeducation regarding the
basics of CBT/ERP and how this treatment could be applied to his presentation. When he
initially presented for treatment, Simon was familiar with OCD, having received a diagnosis in
his teens. However, he was largely unfamiliar with contemporary theoretical conceptualizations
of OCD, or the CBT and ERP-based treatment approaches. Additionally, psychoeducation was
given throughout treatment when needed. During the second session, Simon was introduced to
thoughts, and the compulsion/behavior he engaged in. Reviewing this self-monitoring aided in
the establishment of initial response prevention targets to help reduce compulsions. Self-
monitoring was also used throughout treatment to record adherence to response prevention goals
Simon and clinician collaborated to design exposures that would systematically and
gradually challenge OCD fears and discomfort as well as to create response prevention goals to
increase frequency of resisting compulsions. Consistent with the inhibitory learning model, in-
vivo and imaginal exposures were implemented and designed to vary in content, duration, and

intensity and included the gradual removal of safety behaviors and compulsions. Following
assessment and psychoeducation, weekly sessions were focused on designing and completing in-
well as creating and modifying response prevention goals. Most of Simon’s independent
exposures occurred at home, as this is the area where most compulsions were occurring.
Throughout treatment, Simon displayed excellent adherence and consistently completed nearly
In-Vivo Exposures
In-vivo exposures were frequently used throughout treatment to allow Simon to directly
confront discomfort associated with not just right experiences and uncertainty and eliminate
home and work environments as a part of his treatment homework. In-session exposures focused
on increasing tolerance of not just right experiences and uncertainty while reducing, delaying, or
eliminating compulsions. For example, many exposures focused on disorganizing items in his
bedroom by putting them into places that felt “not just right” and delaying the compulsion of
putting them back in the “correct” location. These exposures first consisted of delaying the
compulsions by several hours with the ultimate goal of Simon being able to leave items in his
bedroom disorganized until the end of treatment. Within these exposures he was also instructed
to resist engaging in any checking behaviors such as scanning his room or engaging in mental
checking to remember where items were placed. Other exposures focused on disrupting Simon’s
several lists while reducing or eliminating the associated checking behaviors or redoing of lists to
make them feel “just right.” For example, several in-session exposures consisted of Simon
deleting “less important” lists, such as books to read or activities to do with friends. He focused

on the fear that he might forget what was on these lists, while resisting remaking the lists or
engaging in mental checking to ensure he would not forget the items that were deleted. During
in-session exposures, Simon presented as engaged and willing to take on difficult exposures and

Imaginal scripts were utilized throughout treatment to allow Simon to confront fears of
forgetting or losing items/thoughts. A primary imaginal exposure script was developed during
the sixth session and repeated for homework for several weeks following. This script depicted
what a day might look like if Simon was not able to engage in any checking or rearranging
compulsions, causing him to lose objects, be distracted at work, forget several important tasks,
and fall behind with work. The script also included the general discomfort Simon feels when
compulsions are not completed to increase the salience of this feeling and ultimately increase his
tolerance for the discomfort associated with NJREs. Simon reported difficulty engaging with the
primary imaginal exposure script as he felt it was not realistic. He reported that he mostly
engages in compulsions to reduce discomfort and NJREs, rather than to avoid a catastrophic
Secondary imaginal scripts were frequently used in tandem with in-vivo exposures to
promote engagement with thoughts and face uncertainty associated with resisting a ritual. The
session ten transcript (See Appendix F) depicts how a secondary exposure script was developed
and incorporated as a part of treatment. Simon was instructed to engage in secondary exposure
scripts when completing exposures for homework as well. Secondary exposures scripts were
especially helpful, as they ensured Simon could not engage in mental compulsions, distraction,

goals were identified for individual exposures such as resisting putting items in their “just right
location” or mentally checking and reviewing lists. In addition to identifying response prevention
goals prior to individual exposures, daily ways to challenge anxiety were also identified.
Examples of some of Simon’s daily response prevention goals included limiting the number of
times a to do list is checked in a day, eliminating checking and scanning of areas of his house
and office, and limiting the number of items added to his daily to do list. Each week, session
consisted of reviewing a resist/submit log within which Simon would log how often he was able
to engage in the established goals. After demonstrating several weeks of consistency with a goal,
that response prevention target was set as an expectation for his behavior moving forward and no
longer needed to be tracked. New goals were then established. At any given time, Simon was
responsible for engaging in and tracking 2-4 daily response prevention goals.
While treatment was not concluded at the time of submission for this report, Simon
demonstrated consistent progress and the ability to manage his symptoms. His progress is
especially notable given his rather unique presentation of symptoms. Typical OCD presentations
that include a concrete feared outcome that is easily disproven with exposures often respond
more quickly to ERP. However, given Simon’s feared outcomes were more ambiguous and
likely to occur, his treatment moved more slowly, focusing on increasing his tolerance for
discomfort and NJREs, as well as increasing his flexibility and allowance for uncertainty.
Through the use of exposure and response prevention, Simon was able to reduce frequency of
compulsions, especially regarding checking and rearranging. The remainder of treatment will

continue to focus on reducing compulsions to normative levels in the hopes that, as compulsions
The Y-BOCS is the gold standard for assessing severity, progress, and success in treating
OCD. Farris and colleagues (2013) collected data from four treatment outcome trials (N=288) to
identify criteria for determining response, remission, and wellness in individuals with OCD using
Y-BOCS scores. They determined a reduction from pre to post treatment Y-BOCS scores of 35%
was indicative of treatment response. A score of 14 or less at post-treatment was the best
predictor of symptom remission. A score of less than or equal to 12 was the best predictor of
26, moderate; post-treatment = 11, mild). This 57% decrease and score of 11 is indicative of a
positive treatment response, high level of wellness, and substantial symptom remission. Despite
this low score and his report that compulsions cause zero interference, at the 5-month mark,
Simon still felt there were several areas he wanted to work on before terminating treatment, such
as continuing to reduce reliance on lists and hoped to see a greater reduction in obsessive
In addition to completing the 5-month YBOCS in session, Simon also completed the
remaining self-report measures at home. Simon’s self-report OCI-R score (21, moderate) went
down one point from pre-treatment which is inconsistent with the much larger reduction in his
Y-BOCS scores. However, within the OCI-R, his scores for checking and ordering both
decreased, with checking moving into the normative category. The reduction in these scores is
indicative of treatment progress as they are the two primary areas of OCD that had been focused
on thus far in treatment. Slight increases in his scores within the obsessions, mental neutralizing,

and hoarding scales lead to a nearly equivalent pre to post OCI-R, however, these scales

This result is inconsistent with treatment progress and Simon’s verbal report of symptoms
causing little interference in daily life. The subscale related to symptom interference at work
decreased into the normative range which is notable as the beginning of treatment was focused
on reducing compulsions at the work place; this decrease denotes that success. Regarding the
home subscale increasing, it would appear this result may be due to his difficulties experiencing
more obsessive thoughts in the evenings now that he is able to delay compulsions during the day
while at work.
His 5-month DASS scores appear to reflect continued distress. Treatment thus far has
focused on OCD symptoms, which the DASS does not directly measure; thus, this score would
not necessarily be expected to decrease significantly. The subscale for anxiety, which would be
most related to OCD symptoms, decreased by two points and was in the normal range at 5-month
check-in.

to get through his day without giving into many compulsions. He continued to struggle, however,
with resisting compulsions in the evening, as anxiety builds from resisting compulsions

transcript includes additional comments highlighting areas of clinical strength, relating to aspects
of the session to the theory, and reflecting on areas of improvement. This session was conducted
via televideo and involved reviewing homework assigned in the previous session and developing
The treatment outcomes in this case and the procedures implemented during the sample
session indicate my strong grasp of the theorical models which underlie ERP. When reviewing
the transcribed session, I was able to reflect on specific areas where I exhibited effective
therapeutic skills. For example, when developing response prevention goals, I was integrating
knowledge from previous sessions and Simon’s success with goals, his presentation of
symptoms, and the theory underlying how to implement response prevention goals to create
objectives that would reduce compulsions and ultimately lead to symptom reduction.
Additionally, when creating these goals, I allowed for Simon’s input not only to increase rapport,
but also to continue helping him build his skill and knowledge of ERP so that once treatment is
complete, he feels more able to apply these techniques independently and effectively.
Furthermore, designing exposures is an integral part of an ERP protocol. Creating multiple in-
session exposure targets allowed for us to address multiple difficulties in one session and helped
progress treatment. When creating these exposures, I was able to clarify what fears or

discomforts arise and how compulsions are implemented to decrease those sensations in order to
design an exposure that directly triggers those fear structures. I was also able to provide feedback
to Simon when going through the secondary exposure script to increase the salience of fears.
Addressing core fears is a vital part of an ERP protocol, and completing these in-session
allowed for Simon’s core fears to be continuously and systematically addressed in a way that

Reviewing the transcribed session also allowed me the opportunity to reflect on areas
where I can continue to improve as a clinician. Throughout the session, I could have increased
collaboration with Simon by having him more directly lead and design exposure homework, thus
further increasing his independence and knowledge of ERP. I also noticed that I could have let
him sit with each exposure longer to allow for more reduction in anxiety and discomfort as well
as to do a more extensive cognitive debrief after each exposure. It is helpful to discuss what is
learned after completing an exposure to allow clients to reflect on the experience and compare it
to their expectations from before completing the exposure, thereby concretizing the adaptive
changes. Often times we find that if clients are surprised by the outcome of an exposure (e.g. it
was easier than expected) they benefit more. Absent active and purposeful reflection on the
outcome of the exposure they might not realize an expectancy violation has occurred. Reflecting
upon this case in its entirety, I also could have used more salient measures for tracking progress,
such as the Not Just Right Experiences Questionnaire, to help better evaluate the progress made

Session 10 Transcript
T: Hey, how are you?
C: Good, how are you doing?
T: Good, thanks. Alright so I’m thinking pretty typical session for today, although it might be
possible we fit in two exposures just because we typically have a little extra time at the end and
obviously will review homework here at the beginning. Anything you wanted to add to our
agenda for today?
Per standard CBT session structure, I set an agenda for session and solicit client input.
Attending to homework each session also positively reinforces homework compliance, making
his compliance more likely in the future and helps to emphasize the importance of work outside
of session time.
C: Nope, that sounds good.
T: Awesome, alright so talk to me about how homework went. I was just going over your
tracking so let's talk about response prevention first. How did it go with the To Do List? I know
that's one of our newer goals.
For this session, Simon had the following response prevention goals that he had been tracking
daily over the past week via a shared Google Sheets document: 1) Resist checking daily to do list
more than 10 times, use maybe, maybe not when noticing urge to check to do list 2) Recognize
urge to “organize my thoughts”, use “maybe, maybe not”/script and mental flash cards to resist
checking. 3) Use mental undoing when giving into compulsions e.g. “Even though I checked I
probably forgot something, my memory isn’t reliable.” Goal 1 was set to reduce the amount of
checking of his to do list, as before treatment he was checking his to do list upwards of 50 times
each day. Goal 2 refers to a mental compulsion Simon often engaged in that he called
“organizing his thoughts.” This was a checking compulsion that he used to make sure he had not
forgotten anything important such as conversations or tasks. Given it is hard to control thoughts,
he implemented a cognitive task (mental flash cards) to make it more difficult to engage in the
mental compulsion and increasing the uncertainty that he would forget. Goal 3 is a tactic that he
would implement when a compulsion has already happened to help continue fighting OCD by
using a short uncertainty script to help “undo” the compulsion, lean into uncertainty, and
minimize relief from completing the compulsion.
C: Yeah, so this was a tough week definitely kinda have a lot more checks in terms of
submitting. Yeah, I just had a lot of anxiety this week I don’t know specifically why but yeah it
was just harder to fight don’t really know what was going on but yeah definitely was not as
successful.
T: Yeah, I think it's fair if we notice our anxiety is higher from the week it's gonna be harder to
do things that are gonna bring on anxiety so that makes total sense. I noticed that we had a
couple days where we were resisting. On the 1st and the 30th you marked those as resist so I'm
wondering what felt different about those days or anything you noticed?

C: Hold on let me pull that stuff up. You said on what days?
T: So it looks like on the first and the thirtieth we had some success with resisting the checking

C: Yeah, before then I was doing pretty good. I felt good about everything I think yeah there
wasn't any real specific reason on that day on those days why I did any better. I think I was just
feeling better at that point about everything.
T: Yeah, gotcha. Well and I think what I was also thinking about is like decision fatigue. So, I
imagine that's kind of happening with this where it's like if we give in or we have so long of a
day and so later on in the day do we notice it's harder to resist that as well just because like we've
had to resist all day?
C: Mhmm, definitely.
T: Yeah, so I think with that we kind of want to think about like trying to have some early wins
earlier in the day, like almost like we’re playing savies with like when we can check just to help
make those decisions later in the day easier. So, if in the morning we're having like more wins
with resisting checking I think that'll make it easier for later in the day because I imagine we're
seeing more checking at that time.
C: Could you say that one more time?
T: Yeah so I think I'm just saying if we are trying our best in the mornings to try to keep that as
like the lowest time of day where we're gonna be successful and resist I think it'll help with later
on in the day because we'll have like right now we'll probably keep our goal to 10 so if we're able
to resist and only check like two times in the day that might make it easier for in the evening
'cause we know we have more ability to check later in the day and there's also like gotten some
wins in so that we know we can do it later on in the day. Does that make sense?
C: Yeah, I see what you’re saying.
T: So I think just like mornings are important for this goal and it's like if we're trying really hard
in the morning, not that you're not always trying really hard, but I think that'll actually help it and
make it easier for later in the day.
C: Yeah, giving me a little more leeway.
T: Yeah exactly. So that kind of might be our focus for this week because I really want to stick
with this goal. We're also just going to start trying to do exposures with the lists and that also
might increase anxiety but help with kind of the checking stuff. Sound good?
C: Yeah.
T: OK and then we had our other goal. Looks like that one kind of was harder this week too
probably due to stress?
This is referring to the second goal of response prevention of resisting “organizing his thoughts”

have any particular reason I just had a lot of anxiety this week.
T: Well it might have been because our exposures too. That's my best guess since we really
upped the ante with that this week with including that during the work day. So that being said tell
me how exposures went this week.
His exposure homework that was reviewed in the session was to disorganize his room each
morning and make it so that items aren’t in their “just right” location and delaying the
compulsion of putting them back until the end of the work day. Delaying compulsions is helpful
when completely eliminating them feels too difficult for clients to help them build confidence that
they can resist and still allowing time for habituation.
C: Yeah so the exposures too they were tough. I got them all done but especially I think it was
last like Thursday or Friday was very anxiety provoking, I think I put a 9 or something and I was

yeah I had to lay down to just kind of breathe through some things I was feeling very stressed but
yeah I think it was just all a higher level of anxiety. I got through work but it definitely did its job
it caused a lot of anxiety.
T: So that was definitely to be expected and why we really tried to build up to the work day
because we knew it would just increase our overall anxiety for sure but I'm glad to hear that we
got through it and we got to a nine, which is probably the highest that we've ever gotten to. So,
what did you notice when you were at a nine?
C: I mean I was just couldn’t think about anything. My mind was very focused on trying to put
them back where they were supposed to be and yeah I just kind of had to lay down couldn't
really think about anything else and was just trying to get through it.
T: Yeah well really good job getting through it and we're still resisting that urge to put things
back where they go. I think that's awesome that we upped the ante and you were still able to get
through it and tolerate that anxiety so I think those are big things to keep in mind this week. OK
well great job this week and I know it's very stressful so we'll talk about homework at the end
and how we can keep things the same or modify them where we feel necessary. So, I know I
mentioned the possibility of two exposures today so we can kind of talk about what we think is
doable. So given that we had high levels of anxiety with exposures this week we have a couple
options. What I was thinking is the next step with the exposures that we have been doing is really
rearranging stuff in our room with the goal of we're not going to put them back to like the place
that they go until like the end of treatment really so that would be kind of the next step with the
exposures that we have been doing. I think ultimately that like just extended period of time
would kind of eliminate or help to eliminate the compulsions associated with checking and
rearranging but I know we had harder exposures for this week with doing it during the work day
so I'm wondering if we feel ready for that or if want to keep with trying the same exposure we
did last week for this week. What are your thoughts?
Again adding in validation and complementing the client on being able to complete the
exposures despite high anxiety. Here I should have engaged in more cognitive work to allow for
consolidation of learning by having him reflect on the exposure more with his ability to resist
despite anxiety and what happened as a result of delaying the compulsion.
C: Yeah, I think that’s definitely too big of a jump. That feels like a lot especially after how
anxiety fueled this past week was I think I would definitely feel more comfortable just kind of
sticking with the same and trying to trying to do the same exposures and yeah not freaking
myself out too much after already having a tough week.
It is important to get the client’s input and collaboration on exposures given they know what they
can and cannot handle. Simon often demonstrated a strong ability to challenge himself with
exposures up to this point and had a good understanding of how to develop exposures. Here, we
begin to calibrate two exposures. The first we are discussing here is regarding the exposure he
had been doing for homework over the past week and how to increase the difficulty in a way that
feels doable. Our goal was to continue increasing the difficulty of the exposure by delaying
compulsions for longer amounts of time each week, to ultimately get to a point where he would
leave items in the “just wrong” location in perpetuity, to increase his flexibility with where items
can be placed.
T: OK, fair. So yeah so, I think since we kind of know how that exposure works we’ll probably
save that for the end. I'll have you rearrange stuff for the next work day and go through the script
for a little bit but I'll let you kind of finish that off after our session if that sounds good just since
you're kind of a pro with that you've been doing it all week you know what the deal is. But then I

kind of wanted to start in on To Do List exposures so maybe doing one of those together today to
kind of dip our toes in that pool. So what I was thinking is, definitely multiple things that we
could, my thinking is just kind of going through some of our less challenging to do lists and like
deleting some items from those as an exposure for today.
C: Yeah, I think maybe not, like deleting would be fine, but also like I told you last week like a
big problem with that is kind of going through and like adding things to the weekly list and
adding things to the daily list. So I think a big a big part of that would also just not only deleting
things but kind of like adding something to the weekly list that doesn't need to be on the weekly
or having something that should be on my daily list that I know I need to do today but just not
including it on that list. I think those since my biggest struggle with the list definitely seems to be
the fact that I have to go back and check to rearrange what time I'm supposed to do items I think
that might be very helpful for kind of getting a real feeling of a true exposure in there.
T: Okay, yeah, I love that idea so kind of what you're thinking for today is maybe going on to
our daily To Do List and deleting an item that we know that we need to do and sitting with that
discomfort and uncertainty.
Here we develop the second in session exposure and the first exposure in treatment to directly
target his to do lists. His to do lists caused a significant amount of interference with constant
checking and rearranging compulsions. This first exposure attempts to increase the uncertainty
that he might forget the item that is deleted off the list as well as reduce the “just right”
sensation that he is tries to maintain with his to do list. I am also allowing for collaboration
within the session so that the client feels like he has independence and to foster skills for
developing exposures in the future independently.
C: Yeah and I mean I guess I should also clarify like with my To Do List like not everything gets
deleted at the end of the day so I mean some things are like…they hang on there for a while so
like I'll need to do a task in them with that day but I might not be completely done with it so I'll
just take it off of that list but not delete the item fully.
T: Gotcha yeah and will you actually just tell me what the app is that you use so I can look into it
and be a little bit more?
C: Microsoft to do.
T: Microsoft to do, alright.
C: Yeah and when I say weekly list you'll see there's a starred list in a my day list so the weekly
list I star things that should be on my week and then my day is everything for the day.
T: Gotcha that makes sense. And, when we're thinking about like when we're checking our list
what is usually the thought that typically prompts that check to happen?
C: Well the most common and what's actually done is like when like I'm done with something
that's on my list and I'm just going there to update its status, delete it or take it off my day, take it
off my week or whatever if I've completed a task. Or if I'm not doing anything I'm sitting and I'm
not sure what to do next then checking it to see what to do next so those are like the real kind of
uses for the To Do List. The reasons that I check them. And then yeah the other one is kinda that
whenever I get in that kind of reorganizing, scanning kind of mode that I do. Again the scanning
has been a lot better recently because of what we've worked on. It used to be kind of that same
thing whenever I got that feeling that things aren't just right then I would go through kind of
rescan the rooms and then recheck the list. Especially in the morning I'll do my To Do List and
then it's completed and then I feel like well that doesn't look that long maybe I should be doing
something else today or that looks crazy long maybe I don't need to do this many things today
then I'll go back through the list and try to remove items and add items again and oftentimes

when I redo it, it ends up being the same except maybe I might have changed like one little tiny
thing. I spent 30 minutes redoing my lists but I only changed one tiny thing that didn't really
matter.
T: Yeah so it's kind of like the practical ones are like OK yeah I've completed a task I'm gonna
go check it off or I just finished something and I don't know what I need to do next so I gotta
check it for that but then it's also like oh I feel like unorganized or things aren't just right so we
check it or feeling like I should be doing something but I don't know what. Is that kind of a good
synopsis of all of that?
Here I am clarifying what compulsions look like and the reason for doing them to make sure that
exposures are targeting the fear.
C: Yeah those are those are good descriptions of those.
T: OK yeah so think that's helpful for also like when we're trying to engage in that response
prevention is like noticing when we start to feel that way and either like doing some maybe
maybe nots, diverting our attention to something else to just help with resisting the checking can
all be kind of helpful things like when we're noticing oh I'm probably it's gonna get to that point
trying to like do something different.
The phrase “maybe, maybe not” refers to using that when noticing an obsessive thought such as
“I forgot something important.” Responding it with “maybe, maybe not” can help clients lean
into uncertainty while resisting compulsions.
C: Mhmm.
T: Cool. Yeah so, I think today would be good to go through and delete something off of our
daily To Do List. I think since we're gonna be continuing with the exposures I think one thing
that might be helpful that we could add in as a response prevention then is like the same thing
but like response prevention would be to resist adding one thing that we need to do to our To Do
List for the week since we're gonna delete it for our exposure. Does that seem like a good way to
follow up with this exposure from session?
C: So like something comes up that I need to do and then just don't add it to the list, is that what
you're saying?
T: Yup!
C: Yeah that's fine.
T: OK sounds good. Alright so let's get out our To Do List. What worries are coming up when
thinking about completing this exposure?
C: That I will forget to do something.
T: Yeah, anything else?
C: Um… yeah that I just kind of get out of that… it's just more of a worry that like I'll be out of
that kind of just right state.
T: So like it won't feel right.
C: Yeah, I mean I've got the way…the way everything with my list is like everything that I need
to do that day is on the list. I really use the list to like help me keep track of all the things that I'm
gonna do. Like especially what it's really useful for…what I use it for is like when I can't… I’m
trying to think how to put it. A lot of times if I add something that day and I need to do it that
day it's in a situation where like it's not something that I can do until that evening like if I'm at
work I need to do something at home then it's great to put it down so I don't have to keep track of
that thought in my mind I can just rely on my list to have it. So it’s almost like a trust has kind of
been broken by deleting things. If I get my notes in like a bad state then there's almost a little bit
of like I feel like it's an extension of my memory of things that I need to do so there's a little bit

of like trust broken in there and I feel very disorganized because I feel like my memory is off and
then that kind of causes me to go back and try to think through things again do the other thing
that we've been doing for the response prevention, stuff like that.
T: Right yeah so we're juggling a lot of different worries, a lot of different propulsions that are
associated with checking our lists or adding things to it.
C: Mhmm.
T: And it sounds like some of it is just that like I can't trust myself to remember to do that thing.
Here I am summarizing what the client is saying into a short statement to let him know that I
understand what he is saying.
C: Yes, exactly that's a big part of it is yeah I don't trust myself to remember things.
T: Yeah so I think that's all stuff to keep in mind for once we delete it we’ll obviously kinda do
our normal script. So what's our fear that if we forget to do something, what would happen?
C: Um…yeah I don't really know. Again yeah it's just more of a worry that I'll forget to do
something. I don't worry that something bad will happen that because I didn't do. I’m just a
completionist and just want wanna do everything I'm supposed to do.
T: So, it’ll be like we haven’t gotten it done that will be just really anxiety provoking and we
may or may not be able to handle it may or may not be able to resist compulsions. Is that maybe
part of it too, is that it increases other compulsions?
C: Yeah, I mean a lot of times yeah my that's the part that causes my whole redo mode, I think a
lot of times there'll be situations, things that are anxiety provoking, something that gets
misplaced or whatever that'll cause me a lot of stress and I almost hit like a breaking point when
things hit a certain point then I just wanna start from scratch and do everything over again. So I'll
scan all my rooms. I’ll do all that. I'll think of things to add to the list. As I'm scanning the room
I'll add things to my list. I'll keep going through I'll keep thinking of things. I'll probably sit there
for 15 minutes and continue to think after I've looked through everything and then try to
reorganize those lists completely. So it's a whole kinda reset pretend nothing is organized reset
organize everything and so, sorry I'm kind of rambling.
T: No I know what you mean. Yeah it just shows that it's kind of like a tidal wave like it's just
like kind of all or nothing it feels like and so that's what we're gonna be trying to like dive into is
challenging whether it has to be all or nothing.

C: Yeah exactly I feel like when especially yeah the list getting out of a good state out of that just
right state is definitely the times that it causes the most anxiety and I have to feel like I've gotta
redo everything. It doesn't happen so much with like physical objects it's just because the To-Do
list is how I keep track of things so if one thing gets messed up on there then I feel like
everything is messed up and I gotta redo it all.
T: Totally, that makes that makes sense. OK so what's our anxiety level thinking about deleting
an item off of our daily to do list.
Getting SUDS ratings is an important part of exposures to ascertain difficulty level and when
habituation has begun to occur.
C: Umm…5.
T: Okay. So let's uh…do we have it open?
C: Yeah.
T: Alright so I want us to pick an item that feels risky but I know it's our first To Do List
exposure so it doesn't have to be the hardest item on the list. So, what do you think that might be
when you're looking at it?

Just like with the rearranging items in his room, we are using a graduated approach with this
exposure, with the intention to increase difficulty in the future by deleting more important items
or more total items. At the end of treatment, the hope would be that Simon is able to use his lists
in a more normative way which would likely look like not checking excessively, not remaking the
list multiple times a day, and only using the list when he needs to check what to do next or mark
an item off of the list. The hope for this exposure is to make the list feel not just right and lean
into that discomfort. Previous to this exposure, we had already limited the behavior of adding
items to the list that he likely wouldn’t forget (e.g. “brush teeth” or “eat dinner.”
C: I think a good thing might be I have a couple to-do items to help me plan my ski trip that's
coming up here in January. That might be a good thing because yeah it's more important than
just some of these things that like I knock out today but it's not like of the utmost importance. I
wouldn’t forget about that kind of stuff.
T: OK well we don't know, we might forget about it right?
Here I help Simon lean back into the uncertainty that he could forget as telling himself he
wouldn’t forget is a form of self-reassurance that we are trying to eliminate.
C: That's true.
T: OK so yeah let's go ahead and delete that item off the list.
C: OK it is deleted.
T: Alright so sorry you said it was like a notification to prompt you to work on planning your ski
trip.
C: Yeah. Yeah which also my To Do List you said notification so just to clarify with that I know
a lot of people do when they have like to do items and stuff they have reminders to set them to
remind them to do things. I don't use my To Do List like that. My calendar has things that remind
me of places to be and things like that but my To Do List is not reminders it's just all I always
like manually and check it. I mean my To Do List is my most used app on my phone. It's always
open, I'm always checking it so I don't really need notifications to remind me about the things.
T: Right gotcha. OK so we deleted that item, what's your anxiety level?
C: 7.
T: OK so go through a script with me. I know we talked about feeling like you'll forget
something, that's totally possible, or I feel like your memory can't be trusted by deleting things
and it also might cause you to have to redo everything. So yeah walk me…walk me through that
script.
C: Yes, so I just deleted an item off of my To Do List. I'm worried that I will forget to do the
thing.
T: So let’s word that a little bit differently. So I might forget it.
Here I am providing gentle feedback on his script. It is helpful to eliminate the phrase “I’m
worried that”and more directly lean into the certainty by stating that the feared outcome might
happen.
C: Yeah, I might forget to do the task and I might have to…because I forgot the task I might
spend an hour trying to redo the lists and think back through things that I need to do and redo
everything. All that is kind of causing me anxiety but I'm just gonna sit with it.
T: What's out anxiety level now?
C: Probably an 8. I think it's moved up a little bit.
T: OK, keep going.

productive and all that is giving me anxiety I'm just gonna sit here.

C: Um maybe a 7. It’s gone down just a tad.

forget to do the other things that are on my list and that it's disorganized so I need to go back
through and think through the things that I need to add the list. I don't trust my memory to
remember all of these tasks that I have to do. So all this is giving me anxiety but I'm just going to
sit.

C: I think it's kind of gone back up to an 8.
T: Okay, what do you think made it go up?
C: I don't know when I just..the last time I said it when I started saying it I just started feeling a
knot in my stomach.
T: Mhmm the physical feelings of anxiety.
C: Yeah it just kind of goes up and down throughout the process.

on my list is disorganized and because of that I might spend an hour trying to reorganize all my
entire list and everything on it so that I can remember what I need to do. But all of this is giving
me anxiety and I'm just going to sit with it.
T: OK. Are still at an 8?
C: I think it's dropped down to a five.
T: OK so uhm awesome. I think yeah I think it's hard with…so a lot of times with exposures we
want…you know our anxiety to go way down during the exposure. I think with a lot of the
exposures that we design for your it's just like it's just a longer process because we're not gonna
put the item back on and we're probably gonna have to keep resisting our compulsions
throughout the day so it's kind of…don't always expect it to go down a bunch like while we're in
session together kind of like how you probably noticed with your at home exposures it takes
longer than just like 15 minutes to let it go down. But notably it does always go down which is
really nice.
Here I am demonstrating my general knowledge of habituation as well as what this looks like for
his presentation. Given that Not Just Right experiences take longer to habituate, most exposures
in session did not see a large decrease in SUDS and the most important part was continuing to
resist compulsions outside of session. In spite of that, a drop in SUDS from an 8 to a 5 is a nice
decrease.
T: So yeah awesome job with that. So what do you think this might look like for response
prevention this week? I know we talked about resisting adding one item so what do you think
that'll look like?
Again, allowing for the client to apply the skills he has learned to formulate his treatment
homework.
C: I would say in the morning when um I'm making my list something that I know that I need to
do that day just not adding it to the daily list.
T: Yeah and I think it'd be good to kind of switch it up each day with what type of item needs to
or to keep off the list. So things that are riskier, things that we have to do at work, or things that
we have to do at home we can kind of just change it up each day since we are hoping to
generalize that over time. Does that sound good?
Continuing to allow for a graduated approach within this response prevention goal and setting
more clear expectations for what that should look like during the week.
C: Yeah.
T: OK so and then like we said let's kind of just set ourselves up for the week of repeating
exposures from last week. So what did that look throughout the week with what items we
arranged?

High use items tended to be more difficult for Simon to put out of place as this increased the fear
that he wouldn’t be able to find them when he needed to use them, so it was important to move to
those more difficult items and continue leaning into that uncertainty.
C: Yeah, I my goal was to get to my night stand and to get things moved out of my night stand
but again because of the high anxiety levels I did not end up getting there. But everything else
like the second toughest place in my closet I did all that. In my desk drawer that's also tough did
that. So there are some of the more tough things that I did. The toughest being my nightstand did
not get to go through but I got through everything that I did do.
T: Yeah awesome so I think that just sets us up perfectly for this week since we are repeating it
we have the time to get to those more difficult areas. So yeah I think let's just go ahead and
rearrange some stuff, specifically items in your night stand, and sit with our script like we
normally do and then we'll finish up and talk about homework for the week like we normally do.
C: Alright.
T: So what did we rearrange?
C: Yeah so I moved some clothes around in my closet to where they're not supposed to go and
took items out of my nightstand. I took a few things out of my dresser and then I moved things
around in my desk drawers.
T: Alright so walk me through our exposure script for a few minutes.
C: I just disorganized my room. Things are not where they're supposed to be and I'm worried that
I will forget where they are or what to do because the items are misplaced. That feeling of
forgetfulness is causing me anxiety and I feel like I want to I go through my room, rescan, figure
out where everything goes. I might spend an hour trying to remember where things are or what to
do and that's time that could be better spent doing something else and so all that is giving me
anxiety but I'm just going to sit with it.
T: What’s our anxiety level?
C: Probably a 6.
T: Okay.
C: So I just disorganized my room. Moved things in my dresser, my closet, in my desk drawer,
and nightstand. Disorganized them and I might forget those things and I might forget to do
something because those items are misplaced. I'm worried that I'll spend a bunch of time trying
to remember where to put those things or trying to remember something to do because those
items are misplaced and that's time that can be better spent doing something else and so all this is
giving me anxiety but I'm just going to sit with it. and
T: Let's remember what we talked about last time. We talked about the kind of the worries too
with like work the next day how about how it might be distracting at work or anything that's
related with that kind of how we're extending it to the end of the work day.

could be better spent doing something else. Especially at work when I have no control over the
organization at home and it's time that I need to be very productive. So all of this is giving me
anxiety but I'm just going to sit with it. So I just disorganized my room. I moved things out of
my dresser, in my closet, and in my desk drawers. I may have forgotten an item and I might have
forgotten something to do because those items are misplaced I might spend an hour or longer
trying to remember where things um where things are or remember something to do because
those items are misplaced. Due to all this I think…sorry I got confused. I might spend an hour
trying to remember where things are or what to do because the items are misplaced this is time
that could be better doing something else. Especially when I'm at work because I have no control
over the organization at home and that's time that I need to be very productive so all of this is
giving me anxiety but I'm just going to sit with it.
T: Nice. What’s our anxiety level?
C: Uhh 6 maybe.
T: Alright so um let's talk about homework. So, I think it's pretty much the same as last week so
we have really trying to resist our checking our daily To Do List more than 10 times. I think
again kind of keeping in mind like in the morning maybe trying to get our early wins in there to

make the whole day easier in general. And then I think just due to difficulties with our goal of
resisting organizing our thoughts from this past week we will stick with that goal. Has it still felt
like mental flash cards are helpful with resisting checking or do we wanna like switch up what
we do?
C: No I think that's very helpful. It's done a pretty good job of just keeping my mind occupied.
T: Yeah alright and then I think with that too we just want to keep in mind that like it's to keep
our mind occupied but it also it can help us lean into the fear of like I'm doing something else so
I might forget what needs to be done. So we really wanna make sure we're like leaning into those
worries like anytime that we are able to.
Providing additional rationale for why this particular goal is important. Throughout treatment,
we were trying to increase his overall engagement with uncertainty to help increase
generalization.
C: Mhmm
T: And then the thing I added was our To Do List so resisting adding one item that needs to get
done in our daily To Do List when we like going through it in the morning? Cool, and I already
added that to your tracking form so that's just like our third goal for the week. And then our
exposure. So same as last week with daily exposure of disorganizing items in your room and
then resisting putting them back until after work the following day and then just making sure
we're sitting with our scripts for 15 minutes after we disorganize everything.
C: Yeah sounds good.
T: Alright yeah that is all I have for you. We got through a lot of good stuff today and I think it'll
be helpful that we're starting in on the to-do list since it seems like that's like something that
takes up a lot of our time with OCD.
C: Yeah I mean when I say like when I give you those values for the YBOCS or whatever where
like my time spent the majority of that time is spent going through my to-do list. That's what
sucks up most of my time for sure.
T: Yeah so I think plan for next week is to if we I think even if it's still pretty hard with the
exposure that we're doing this week that we did last week I think it would be great to try to move
to this organizing things and then leaving them be. It'll probably be hard at first and then we'll get
to the place where that feels pretty comfortable I think and then then when we get to the end of
treatment we can kind of decide how we can put those things back in a non OCD way and we
can kind of discuss what that might look like more next week as well.
C: That sounds good.
T: So, to just mentally prepare you, I think I wanna give us a good challenge next week not that
you haven't been doing really hard stuff already because you're doing so awesome but I think
that'll just kind of put the nail in the coffin for like that portion of our treatment.
C: OK cool sounds good.
T: Alright and I assume same time next week works for ya?
C: Awesome and we can talk about next week too like holiday scheduling and I know you have
your trip to so we can talk about what future sessions’ appointments scheduling and stuff like
that.
C: Yeah sounds good.
T: Alright well then I will see you next week and good luck with your homework.
