    DIALOGUE ON TAKING VITAL SIGNS

    Nurse : “Hello good morning, let me introduce my self, my name is Wilda Arifaturrohmah
    nurse on duty this morning shift from 7 am to 1 pm”

    Patient : “Hai morning”

    Nurse : “For patient safety, may I know your full name and date of birth?”

    Patient : “Yes, my name is Nafilda Shafrina, and I was born on 24th of April, 2003”

    Nurse : “Ok, may I check your wristband for a moment, please?”

    Patient : “Of course”

    Nurse : “Good, it's suitable. So, the reason I came here is to take your vital signs, what I'm
    going to do is about 15 minutes, do you agree?

    Patient : “Sure, no problem”

    Nurse : “So this vital signs check includes measuring blood pressure, pulse, respiration rate
    in 1 minute, body temperature and your weight. This examination is the most common action
    to determine the general condition of the patient”

    Patient : “Alright”

    Nurse : “What I will do is take your blood pressure first, could you straighten your arm
    please?

    Patient : “Of course”

    Nurse : “So, I'm going to wrap the cuff around your arm and pump it up to read your blood
    pressure”

    Patient : “Would it hurt?”

    Nurse : “No, just relax”

    Patient : “Ok”

    Nurse : “Your blood pressure is 100/70, this shows that your blood pressure is normal,
    alright then I will check your pulse and respiration rate for 1 minute”
    Patient : “Ok”

    Nurse : “I will press your pulse using 3 fingers then count your pulse for 1 minute, let me
    start”

    (After checking pulse)

    Nurse : “After this, I will measure your respiration rate for 1 minute, so just relax”

    Patient : “Ok”

    Nurse : “Ok, then I will check your body temperature, so can you help me to take a
    thermometer in your armpit?”

    Patient : “Of course”

    Nurse : “And the last I will check your weight using a weight scale, let me help you to
    stand”

    Patient : “Ok”

    Nurse : “Ok, let's see how much your weight, it shows at 41 kg”

    Patient : “Yes”

    Nurse : “Ok, so your blood pressure is 100/70 it’s normal, and your weight is 41 kg, and
    then your pulse rate is 86x/min while your respiratory rate is 17x/min, this indicates that your
    pulse and respiratory rate are normal and the last is your body temperature that was 38.7, it’s
    indicate that you have a fever”

    Patient : “Oh ok, don’t worry I can do it my self”

    Nurse : “Ok, after this I will bring medicine to relieve your fever, you need to rest and drink
    lots of water and take the medicine that I give”

    Patient : “Yes, I will”

    Nurse : “Ok, see you later”

    Patient : “See you”
