Task 14,17,20,23

Patient : “Assalamualaikum”

Nurse : “Waalaikumsalam, please sit down ma'am”

Patient : “Okay, thank you ners”

Nurse : “Oh, hi miss wulan how are you?”

Patient : “Yes ners laily, i'm not to bad”

Nurse : “Are there any other complaints? Why did you come here again?”

Patient : “Yes nurses, after yesterday's complaint was healed, i had a cough, shortness of
breath and chest pain”.

Nurse : “Okay fine, before I check you I will reconfirm your demographic data, then I will
review patient data starting from clinical history, and also study the gordon pattern.
Are you ready?”

Patient : “Yes I do”

Nurse : “Is it true with miss wulan?”

Patient : “Yes nurse”

Nurse : “How old are you?”

Patient : “22 years”

Nurse : “Where is your address?”

Patient : “Pekalongan, Central Java”

Nurse : “Okay, did you have any previous medical history?”

Patient : “Yes nurse, yesterday I had a fever”

Nurse : “Is there a history of other diseases?”

Patient : “Yes nurse, I have a history of Pneumonia”


Nurse : “Oh yes, when was you with Pneumonia?”

Patient : “I was diagnosed with Pneumonia about 6 months ago”

Nurse : “At the beginning, how was you diagnosed with Pneumonie?”

Patient : “At first I was often coughed, shortness of breath, accompanied by pain in the
chest and also fever”

Nurse : “Okay, when was the last time you had a checkup?”

Patient : “The last time I had a checkup was one week ago, because I had a fever.”

Nurse : “Does your family have a history of Pneumonia?”

Patient : “No ners”

Nurse : “What do you feel currently?”

Patient : “I have chest pain and shortness of breath”

Nurse : “When did you feel pain?”

Patient : “Uncertain”

Nurse : “Was the pain like being prickling or burning ?”

Patient : “Like being pricked”

Nurse : “Does the pain interfere with your activities?”

Patient : “Yes, annoying”

Nurse : “What activities do you do?”

Patient : “I'm just at home and studying online”

Nurse : “Okay, continue. How many times do you usually eat a day?”

Patient : “3 times a day”

Nurse : “Does the food always run out?”

Patient : “Yes”
Nurse : “Okay, are your bowel movements and bowels smooth? How many times a day?”

Patient : “Yes, ners, I defecated twice a day and bak 4 times a day”

Nurse : “How long did you sleep?”

Patient : “I sleep around 8 hours at night and take 3 hours of naps”

Nurse : “Okay, then I will do a physical examination (Vital sign, and IPPA)

Patient : “Yes”

Nurse : “I’m going to check your vital sign”

Patient : “ Yes ners”

Nurse : “ okay, next I will start with inspeksi by looking at the movemens of the stomach
while breath and exhale then I will continue with palpation, percussion and finally
auscultation”

Pateint : “How was the result sus? Am I bad?”

Nurse : “For vital signs everything is normal, except for your breathing 24 x minutes. So for
Pneumonia you have to live a healthy lifestyle such as getting enough rest, eating
nutritious foods, and exercising regularly”

Patient : “Yes ners, thank you”

Nurse : “Yes miss wulan, your welcome get well soon and stay healthy”
