Unhinged is brutal and truly disturbing, definitely not for the faint of heart. I don't mind disturbing films however, violent brutality is definitely not my genre at all!
I am very fond of Russell Crowe however (an additional disturbing was seeing him physically at this age.) Pretty sure the main reason I watched it at all was for Russell Crowe, and his performance was absolutely outstanding.
The plot is difficult in that it is primarily unfathomable rage - again not a genre I would typically watch. Not sure another actor could pull this off as well as Russell Crowe, keep me watching it and scoring it so high.
Surprised at the low scoring with reviews here, I will assume due to the unfathomable part(s).
As much as I liked it isn't something I would recommend to anyone easily, would require a bit of cautioning it is THAT disturbing.

Ah come on, it's not that bad. Yes it's fluffy. Yes everyone's attractive, yes it's a Costco size cast, yes there's no deep characters and what is so wrong with that?
It is very Garry Marshall, the man knows his stuff and I seriously doubt he set out to remake Doctor Zhivago when making this. I cannot imagine all these stellar actors and actresses signing up for this unless they expected to have some fun doing it.
Unlike some movies where I like actually none of the characters I like all in this one. And that's because the people playing the character are 1 very good at their craft and 2 comedy Ain't Easy. You have to appreciate this many people pulling it off successfully.
I'm not sure exactly who I would steer toward this movie or when. I watched it alone and typically Valentine's Day is simply another day for me. I liked it. It was entertaining. At some point early on I got distracted out of sight of the screen and only hearing the dialogue. The often recognizable voices speaking - the dialogue- brought me back and I Backed up to where I stepped away, sat down, stayed put and enjoyed myself.
I found this movie very disappointing. The most emotion it elicited was an occasional unhappy head shake at (character's) disregard and ignorance. The story based on an actual situation in the 50's having a brother's extraordinary compassion to get his sister out of an insane asylum, the inhumane treatment of mental illness/brain damage and heartbreaking events; this film should have been a real tearjerker. Not saying it was bad just remarkably ineffectual. I should have had tears streaming, sobbed even at one tragedy after another and instead my throat never closed or had even one slightly moistened eye moment. Nada (and I've choked up with Reach Out and Touch commercials.)
