Abstract

본 발명은 정형외과용 세멘트 비드조성물 및 그의 제조방법에 관한 것으로, 만성골수염환자를 1차소파술을 시술한후 생성되는 사공(死空, dead space)에 채워넣은, 항생제와 세멘트를 혼합하여 제조한 정형외과용 세멘트비드이다.
만성골수염은 재발의 빈도가 높고 각종 항생제에 내성이 강한 원인미생물의 많은 것이 특징인바, 원인미생물의 내성(저상성) 역시 완치를 위해서는 집중적인 관리가 요구된다.
따라서, 본 발명은 1차소파술후 생긴 사공(死空, dead space)에 채워 넣어 직접적인 살균효과를 얻고자 고안된 것으로, 많은 미생물에 유효하고 열안정을 갖춘 이상적인 항생제들을 혼합하여 조성시킴으로서 재발의 위험성은 물론 전신적 부작용을 줄이고자 안출된 것으로, 항생제 혼합비율이 페니실린계 티카실린 5-10g, 세파로스포린계 세파졸린 5-10g, 아미노글리코사이드계 토브라마이신 5-10g 및 폴립펩타이드계광범위 항생제 반코마이신 5g의 항생제를 혼합하여 파우더세멘트와 균질화시킨후, 이를 액체세멘트에 혼합한 다음, 경화되는 도중에 비드형태로 잘라서 실에 연결하여 사용하는 것이다.

Description

정형외과용 세멘트 비드조성물 및 그의 제조방법
본 발명은 정형외과용 세멘트비드 조성물 및 그의 제조방법에 관한 것으로, 보다 상세히는 급만성골수염(acute and chronic osteomyelitis) 및 근골격계 제반감염증의 치료시 유효한 항생제가 혼합된 세멘트비드 조성물 및 이를 제조하는 방법에 관한 것이다.
만성골수염(chronic osteomyelitis)은, 재발의 빈도가 높고 항생제에 저항성이 있는 세균을 산생시키므로 치료시 저항성을 나타내기가 쉽고, 항생제의 상습 및 장기적인 투여로 인한 부작용을 보이므로, 정형외과분야에서는 치료하기 어려운 질병중에 하나로 알려져 있다.
그럼에도 불구하고, 만성골수염의 원인으로 과거에는 급성골수염이 주요요인으로 되어 있으나, 현재에는 교통사고와 산업재해의 증가, 또는 이에 수반되는 개방성골절의 후유증으로 초래된 만성골수염의 환자가 증가일로에 있는 추세이다(대한정형외과학회지, 24 : 549∼556, 1989).
즉, 만성골수염은 항상 치료시 내성(resistance)과 잦은 재발(recurrence) 위험성의 치료상 딜레마에 봉착하게 되며, 그밖에 골막비대(periosteal hypertropy), 경화된 피질(sclerotic cortex), 부골(腐骨) 및 반혼조직(scartissue)과 함께 골을 둘러싸고 있는 열악한 혈액공급등, 많은 요인이 만성골수염의 치료를 곤란하게 하는 역할을 한다. 이와 같은 문제점을 극복하기 위하여 많은 치료방법이 제안되었다. 그중, 골소파술(saucerization)은 감염으로 괴사된 골을 제거하기 위한 필수적인 수단이며, 만성골수염의 외과적방법으로 오래전부터 널리 사용되어온 방법으로서 여기에는 이견이 있을수 없다. 그러나 골소파술 치료후 필수적으로 초래되는 폐공간[이하 사공(死空) 이라 한다; dead space]을 처지하는 방법으로서, 근육경조직 이식(muscle pedicle graft), 지속적인 관주(continuous irrigation) 및 와세린꺼즈 삽입 방법등 여러가지가 있으나, 이들 모두가 각각의 많은 문제점을 안고 있는 실정이다.
골수염의 철저한 치료목적으로 광범위하게 시행하는 소파술(curettage)은 언제나 사공(死空, dead space)를 크게 생기게 하며, 이 사공의 처리는 치료의 성패를 결정하는데 중요한 척도가 되는 것이다.
1970년경 최초 부크홀즈와 엥겔브렉트는 세멘트정에서 일정기간동안 항생제가 방출될 수 있다는 이론에 기초를 두고, 수술후 감염을 방지하기 위하여 인공고관절치환 수술시 겐타마이신(Gentamicin) 항생제를 함유한 골세멘트를 사용한바 있고(Buchholz.H.W., and Engelbrecht,H : Uber die Depotwirkung einiger Antibiotica bei Vermischung mit dem Kunstharz Palacos, Chirurg, 41 : 511∼515, 1970), 1976년 클렘과 1978년 월리히는 골소파술후 사공(dead space)내에 겐타마이신을 함유한 세멘트정을 비드를 이식하여 만성골수염을 치료한 결과 만족할만한 결과를 얻었다고 보고하고 있다(Klemm, K : Treatment of chronic bone infection with gentamicin-PMMA chains and beads. 그러나, 세멘트정으로부터 항생제가 유출되는 기전에 대하여는 많은 이론이 있다. 베이스톤, 클렘 및 부크홀즈에 의하면 항생제의 농도와 비례하여 일어나는 확산(diffusion)에 의하여 비드주변에 유출되는 것으로 여겨지는 반면(Bayston, R. and Milner, R.D.G : The sustained release of antimicrobial drugs from bone cement. J.Bone and Joint Surg., 64-B : 460-464, 1982), 베이커등은 확산을 통하는 것보다는 오히려 세멘트정의 공간과 틈새를 통하여 직접 방출되는 것이라고 제시한바도 있다(Baker, A.S. and Greenham. L.W. : Release of gentamicin from acrylic bone cement. J.Bone and Joint Surg., 70-A : 1551-1557, 1988)
항생제의 유출기간에 대해서도 역시 상이한 견해가 있는바, 조직내로 방출된 항생제의 최대농도가 수술후 1-3시간이라는 견해가 있는가 하면, 항균효과가 1주 내지 3주일간 지속되었다는 보고도 있다(Pickmell, B., Mizen L. and Sutherland, R. : Antibacterial activity of antibiotics in acrylic bone cement. J.Bone and Joint Surg., 59-B : 302-307, 1977). 또한 호프등은 7개월동안 효과가 지속되었다고 보고한바도 있다.(Hoff, S.F., Fitzgerald, R.H. and Kelly, P.J. : The depot administration of Penicillin G and gentamicin in acrylic bone cement. J.Bone and Joint Surg. 63-A : 798-804, 1981)
즉, 지금까지 밝혀진바 의하면 겐타마이신 단독 또는 기타 항생제 2-3종이 혼합된 세멘트정이 사용되고 있으나, 이들은 수술실에서 의사에 의해 직접 제조된 것들이며, 따라서 세균에 대한 감수성이 제한적이며, 각 세멘트정마다 농도도 일정하지 못하고 실험적인 제조일 뿐이었고, 상업적 규모의 공급은 불가능하였다.
따라서, 본 발명의 목적은 가능한 폭넓게 각종 원인미생물에 대하여 유효한 감수성을 보이는 항생제와 세멘트를 혼합하여 비드를 만들어 상업적 규모로 제공하고자 하는 것이다.
이하, 본 발명을 상세히 설명하고자 한다.
먼저, 만성골수염 환자 31명에 대하여 균을 배양하여 확인한 결과 하기 표 1과 같은 원인미생물의 존재를 확인하였다.

따라서, 상기와 같은 원인미생물 분포를 확인하고 이들균 대부분에게 감수성을 보일 수 있고, 비드제조시 발생되는 열에 대한 안정성을 고려하여 항생제를 선택한 결과, 바람직한 항생제의 종류 및 혼합비율은 다음 표 2와 같았다.

상기 항생제를 감수성에 따라 선택한 4종의 항생제 분말을 무균하에서 잘 혼합한후, 이 항생제 분말을 파우다세멘트(폴리메칠메타크릴에이트, polymethylmethacrylate)와 균질하게 혼합한다. 이때 사용되는 파우다세멘트는 상업적으로 공급받을 수 있는 제품으로 포장규격은 40g(120cc)인데, 이에 섞는 항생제의 전체 부피는 20 내지 25cc이며, 25cc를 초과하면 세멘트경화에 장시간을 요하게 되므로, 항생제와 파우다세멘트의 부피의 비가 20-25 : 120이 되도록 항생제 20-25cc와 파우다세멘트 120cc를 섞는다.
이렇게 섞은 항생제와 세멘트의 분말혼합물에 액체세멘트(메칠메타크릴에이트 모노머, Methylmethacrylate monomer) 20-30cc를 혼합한다.
이렇게 하여 굳어져가는 상태에서 직경 약 5mm의 일정크기로 절단하여 염주알 형태의 세멘트비드를 실에 꿰어 제조하는 것이다.
비드의 사용량은 수술시 발생되는 사공(死空, dead space)의 크기에 따라 다르며, 대략 약 50-200개 정도의 비드를 사용하게 된다. 본 발명의 세멘트비드를 사공속에 삽입하여 30일후 측정한 비드주변의 항생제 농도는 MBC(Minimum Bactericidal Conecntration)의 60배에 이르러 충분한 항균효과가 있었으며, 반면에 혈증으로 흡수된 항생제의 농도는 거의 무시될 정도로 적어서, 항생제의 전신적 부작용은 전혀 없었다.
이하, 본 발명에 따른 세멘트비드 제조예와 세멘트비드를 이용하여 치료한 실험예를 하기에 기재한다.
[제조예]
티카실린 5g, 세파졸린 5g, 토브라마이신 5g 및 반코마이신 5g을 무균하게 잘 혼합한 다음, 이 혼합된 항생제분말 25cc에 파우다 세멘트(Polymethylmethacrylate수지, PMMA) 40g(120cc)를 가하여 잘 섞어 균질화시킨다.
상기 균질화된 분말을 액체세멘트(Methylmethacrylate monomer) 30ml와 혼합하여 잘 저어주면 시간의 경과에 따라 천천히 경화가 이루어지며, 굳어가는 도중에 직경 약 5mm의 크기로 잘라서 염주알 형태(약 100개)로 만들고 실바늘을 통과시켜 실에 연결된 세멘트비드를 얻는다
[치험예]
만성골수염의 환자 31명을 대상으로 치료하였다.
이들의 년령과 성별분포도는 하기 표 3과 같다.

[수술방법]
만성골수염 환자 31명 대하여 소파술(saucerization)을 시행하였는바, 연부조직 절개시 가능하다면 근피부군(Myocutaneous territories) 사이로 절개하여, 골조직에 접근함으로써 가능한 혈관손상을 최소화하려고 노력하였다.
소파술후 본 발명에 따라 제조된 항생제가 혼합된 세멘트비드를 사공(死空, dead space) 속에 삽입하고 창상을 봉합하였다. 수술후 약 4주에 세멘트비드를 제거하고 골이식술(bone graft)로 치료하였다.
[치료결과]
치료결과는 아주 만족스러웠으며, 17명의 예는 만성골수염으로부터 완전히 회복되었고, 나머지 10명에 대해서도 사공(dead space)은 아주 깨끗하여 확실히 세멘트비드의 효과는 있었으나, 그후 환자의 전진상태 불량, 또는 재외상등으로 인하여 재발하여, 그중 7명은 소파술만으로, 3명은 소파술과 골이식술로 치료하여 회복되었다. 나머지 4명은 병적골절, 또는 편평상피세포암의 발명으로 인하여 절단술을 하지 않으면 안되었는바 이는 매우 애석한 일이었다.
이상과 같은 치료효과를 표 7에 기재하였다

이상과 같이, 본 발명에 따른 세멘트비드를 만성골수염 소파술후 사용할 경우, 사공(死空, dead space) 주변부의 살균효과를 장기간 유지함으로써, 골수염의 치료효과가 탁월함을 알 수 있었다.
종래와 같이 시공을 방치한 채, 전신적인 항생제만을 투여할 경우는 항생제의 장기간 투여로 인한 경제적손실 및 전신적 부작용이 높았고, 재발도 많았다.
본 발명에 따른 항생제 혼합 세멘트비드 조성물을 사용할 경우 장점을 요약하면 다음과 같다.
① 전신적 항생제와 투여가 필요없거나, 적은 양만의 투여만 필요하므로 부작용이 거의 없다.
② 고가항생제의 장기간 투여 필요성이 적어져 경제적이다. (즉, 치료비 절약효과가 기대된다)
③ 세멘트비드를 삽입하고 환부를 봉합하므로 창상치료(dressing)가 간단하고 입원기간이 짧아진다. (즉, 입원비의 절감효과가 기대된다)
④ 만성골수염 주변은 혈액순환이 나빠 어짜피 전신적으로 고단위 항생제를 투여한다하여도, 골수염 부위에는 항생제가 침투되기 어려운 반면, 본 발명품은 사공 그 자체에 삽입하므로 직접적인 살균효과가 있었다.
⑤ 수술후 4주간 살균에 필요한 충분한 양의 항생제가 세멘트에서 유출됨을 알 수 있었다.
따라서, 본 발명은 산업적으로 매우 우수한 발명임이 틀림없다.

Claims (2)

  1. 정형외과용 세멘트비드 조성물에 있어서, 티카실린 5-10g, 세파졸린 5-10g, 토브라마이신 5-10g 및 반코마이신 5g으로 조성된 항생제 혼합분말 20-25cc와 파우다 세멘트인 폴리메칠메타크릴에이트 120cc 및 액체 세멘트인 메칠메타크릴에이트 모노머 20-30cc로 구성된 정형외과용 세멘트비드 조성물.
  2. 정형외과용 항생제 혼합 세멘트비드를 제조함에 있어서, 티카실린 5-10g, 세파졸린 5-10g, 토브라마이신 5-10g 및 반코마이신 5g의 항생제를 무균하에서 혼합한 후, 이 항생제분말 20-25cc에 파우다세멘트인(폴리메칠메타크릴에이트)120cc를 가하여 혼합, 균질화시킨후, 균질화된 분말을 액체 세멘트인 메칠메타크릴에이트 모노머 20-30cc에 혼합하여, 굳어져가는 도중에 직경 약 5mm의 크기로 절단한 후 실에 꿰어 연결시킴을 특징으로 하는 정형외과용 세멘트비드의 제조방법.
