Description
ネック部分とは、図18に示すように、動脈瘤を起こしている血管であって、動脈瘤の両側からつながる健常な血管部分をいう。実際には後述するROI内の血管部分であって、動脈瘤領域を除く範囲である。図19に示すように、ステントグラフトはその両端の外周縁が当該ネック部分の内壁に密着することでエンドリークを好適に抑制することができる。この抑制により血管の局部的異常拡張、つまり動脈瘤を抑えることができる。
非推奨領域とは、ネック部分において、ステントグラフトを留置するのに適さない血管部分、つまりエンドリークが起きる可能性が高い血管部分をいう。なお、本実施形態は、ステントグラフト留置に適さないとされる血管部分を技術的に定義するとともに当該定義に従って当該非推奨領域を自動的に同定し、当該非推奨領域を術者(医師)に提示して治療計画立案を支援することを特徴の一つとしている。
推奨領域とは、ステントグラフトを留置するのに適している血管部分、つまりエンドリークが起きる可能性が低い血管部分をいう。実際には、ネック部分から非推奨領域を除外した血管部分をいう。
エンドリークとは、ステンドグラフト留置後に、ステントグラフトの両端の外周縁とネック部分の血管内壁との間に隙間ができることにより、当該隙間から血液が漏洩して、抑制血液が動脈瘤内に流れ込む現象をいう。隙間はステントグラフトの両端の外周縁とネック部分の血管内壁との間の密着不足が主な原因となり生じる。
関心領域(ROI:Region Of Interest )とは、本来的にはユーザーが注目する部位のことをいい、ここでは特に非推奨領域/推奨領域の同定のための処理対象範囲としてユーザーにより設定される画像内の一部分を構成する局所範囲をいう。
Claims (11)
- 被検体に関する動脈瘤を含む画像のデータが記憶される画像記憶部と、
前記画像から血管壁領域を抽出する血管抽出部と、
前記抽出された血管壁領域に基づいて血管径の変化率、血管曲率及び血管扁平率を前記血管壁領域上の複数の離散点について個々に算出する血管径変化率等算出部と、
前記複数の離散点から、前記血管径の変化率、前記血管曲率及び前記血管扁平率の少なくとも一つがそれぞれの閾値を超過する非推奨点を抽出する非推奨点抽出部と、
前記抽出された非推奨点の点列範囲に従って、前記動脈瘤に対するステントグラフトの留置に推奨されない非推奨範囲を決定する非推奨範囲決定部と、
前記記憶された画像に前記非推奨範囲を重ねて表示する表示部とを具備することを特徴とする医用画像処理装置。 - 前記画像内の前記動脈瘤を含む局所範囲を関心領域として操作者指示に従って設定する関心領域設定部をさらに備え、
前記血管抽出部、前記血管径変化率等算出部及び前記非推奨範囲決定部それぞれは処理範囲を前記関心領域に限局することを特徴とする請求項1記載の医用画像処理装置。 - 前記非推奨範囲外であって、前記血管径の変化率と前記血管曲率とがともにそれぞれの前記閾値又は前記閾値より低い他の閾値より低い範囲を、前記動脈瘤に対するステントグラフトの留置に推奨される推奨範囲として決定する推奨範囲決定部をさらに備えることを特徴とする請求項1記載の医用画像処理装置。
- 前記推奨範囲決定部は、前記推奨範囲から前記画像から同定されたプラーク部又は石灰化部を除外することを特徴とする請求項3記載の医用画像処理装置。
- 前記決定された推奨範囲は前記画像に前記非推奨範囲とともに重ねて表示されることを特徴とする請求項3記載の医用画像処理装置。
- 前記表示部は、外部のアンギオX線診断装置により発生されたライブ画像に前記非推奨範囲と前記推奨範囲との少なくとも一方を重ねて表示することを特徴とする請求項3記載の医用画像処理装置。
- 前記非推奨点抽出部は、前記閾値を操作者指示に従って変更することを特徴とする請求項1記載の医用画像処理装置。
- 前記画像上に操作者指示に従って設定される前記ステントグラフトの留置予定位置の両端部分における血管径と前記両端部分の最大曲率と前記両端部分の間の長さとに従って前記ステントグラフトに必要とされる太さ、曲率、長さに関する仕様を決定するステントグラフト仕様決定部をさらに備えることを特徴とする請求項1記載の医用画像処理装置。
- 前記ステントグラフト仕様決定部は、前記仕様に適合するステントグラフトの製品又は種別をステントグラフト製品管理情報から特定することを特徴とする請求項8記載の医用画像処理装置。
- 前記画像上にエンドリーク発生の可能性の高い位置として重ねて表示させるために、前記ステントグラフトの留置予定位置の両端部分における最大の血管扁平率を示す位置を特定するエンドリーク確認支援部をさらに備えることを特徴とする請求項1記載の医用画像処理装置。
- 前記エンドリーク確認支援部は、前記最大の血管扁平率を示す位置における血管長軸と血管中心線とに直交する撮影角度を決定することを特徴とする請求項10記載の医用画像処理装置。
