図1,2および4に図示されているように、付属ステント32は、丸みを帯びている一方で比較的鋭く、そのため、血管を突き破る確率を増やしてしまう。
本明細書は、新規性があると考えられる本発明の特徴は特許請求の範囲で定義するが、以下同一部分には同一符号を付して示す図面につき本発明の好適な実施例を説明することによって、本発明をよりよく理解できるであろう。
本発明によるステントグラフトは、これまで当該技術では見られなかった様々な特徴を提供し、このことにより、大動脈の正常な経路または疾患経路内により効率的に留置/適合し、血管を突き破る危険性を減らし、そして血液漏れのない血管結合を増し、そしてグラフトが移動する確率を減らすような血管修復デバイスを提供する。
上述したように、従来技術は、補綴物をルーメン(管腔)内送達する、とくに、ステントグラフトを血管内に送達する多くの異なったシステムがある。送達システムの多くは類似した部分を有し、典型的には、ほとんどは、送達システムを使用する前に患者の鼠径部の近くの大腿動脈に差し込み体を介して挿入したガイドワイヤに沿っ案内する。大動脈および大動脈につながる動脈の突き破りを防ぐために、送達システムはガイドワイヤと同軸状に連結し、大動脈に至るガイドワイヤの経路をたどる。したがって、ワイヤの経路をたどる送達システムの部品は患者の大腿動脈の内径よりも小さな外径を有する寸法にする。ガイドワイヤの経路をたどる送達システムの構成要素にはステントグラフトが含まれ、カテーテルおよびシースと称される、一連の同軸のルーメン(内腔)でできている。典型的には、ステントグラフトは外部カテーテルによって拘束されており、ステントグラフトが圧縮されて外部カテーテル内部に装入することを必要とする。そうすることによって、ステントグラフトを非常に強固に拘束する送達システムの部分ができ、その結果、この部位の柔軟性を減らし、特に大動脈弓のような湾曲したの血管に沿って送達システムがガイドワイヤの後をたどることを困難にする。加えて、ステントグラフトは、カテーテル内部に装入するために圧縮しなければならない量に起因して、拘束しているカテーテルに対して非常に大きな半径方向の力をかけるため、拘束しているカテーテルをステントグラフトからスライドさせて外すことによってステントグラフトを展開させる作業工程は、典型的には展開力と称される、非常に大きな力を必要とする。また、カテーテルはグラフトを拘束するために十分に強くなくてはならず、硬い材料で作られることを必要とする。この硬い材料が、例えば大動脈弓へと進むときに屈曲する場合、この硬い材料はねじれやすくなり、ねじれると、不可能ではないにしても、ステントグラフトを展開するのが難しくなる。
前述したように、露出したステント30は、近位端部12で外側に広がる力を提供する。露出したステント30と近位のステント23は、グラフトスリーブ10に取り付けられたときに圧縮した状態にあり、外側に広がる力をグラフトスリーブ10に提供する。したがって、埋め込まれたときに、これらの力は、ステントグラフト1の近位端部を血管の中心に配置し、グラフトスリーブ10を血管壁に押し付け、グラフトスリーブ10と血管壁との間に生じる漏れを阻止する。このような近位端部12における漏れは、ステントグラフトの埋め込みでは避けられるべきである。

少なくとも2つのステントを有するサポートフレームワークとを含む、血管修復装置であって、
第1の前記ステントが、前記上流端部に隣接して前記グラフト本体の周囲に連結され、
第2の前記ステントが、前記グラフト本体の上流端部で、かつ前記第1のステントの下流で前記グラフト本体に部分的に取り付けられ、前記第2のステントは、前記グラフト本体に完全に覆われており、かつ上流頂部と、下流頂部と、隣接する該上流頂部及び下流頂部それぞれを連結する支柱とを画定する周囲を有し、該支柱のそれぞれが、上流部と、下流部とを有し、前記下流頂部及び前記支柱の前記下流部が、前記グラフト本体の前記内側面に取り付けられており、前記上流部と前記上流頂部のうちのいくつかのみが、前記グラフト本体の前記内側面に取り付けられている、装置。 - 前記第2のステントが、前記第1のステントの下流でありかつこれに隣接する前記グラフト本体の前記内側面に部分的に取り付けられている、請求項34記載の装置。
- 上流端部と、下流端部と、内側面とを有する管状のグラフト本体と、
前記グラフト本体の上流端部にあり、前記グラフト本体に完全に覆われているクラスプステントと、ここで、該クラスプステントは、近位部および遠位部を画定する周期的に変化する形状を有し、前記遠位部でのみ、前記グラフト本体と連結される、
前記上流端部に隣接してかつ前記クラスプステントの上流で、その全体に沿って前記グラフト本体に連結されている第2のステント
とを含む、血管修復装置。 - 上流端部と、下流端部と、内側面とを有する管状のグラフト本体と、
前記グラフト本体の上流端部に少なくとも2つのステントを有するサポートフレームワークとを含む、血管修復装置であって、
第1の前記ステントが、前記上流端部に隣接して前記第1のステントの全体に沿って前記グラフト本体に連結され、
第2の前記ステントは、前記グラフト本体に完全に覆われており、かつ近位部と遠位部を画定する周期的に変化する形状を有し、前記第2のステントが、前記遠位部においてのみ前記第1のステントの下流で前記グラフト本体に連結されている、装置。 - 上流端部と、下流端部と、内側面とを有する管状のグラフト本体と、
サポートフレームワークとを含む、血管修復装置であって、
前記サポートフレームワークは:
前記グラフト本体の前記上流端部にあり、前記上流端部に隣接して前記グラフト本体の周囲に取り付けられるクラウンステントと、
前記グラフト本体の前記上流端部にあり、前記クラウンステントの下流でありかつこれに隣接する前記グラフト本体に部分的に取り付けられているクラスプステントと、ここで、前記クラスプステントが、上流頂部と、下流頂部と、隣接する該上流頂部及び下流頂部それぞれを連結する支柱とを画定する周囲を有し、該支柱のそれぞれが、上流部と、下流部とを有し、前記下流頂部及び前記支柱の前記下流部のみが、前記グラフト本体の前記内側面に取り付けられている、
を有する、装置。 - 前記クラスプステントが上流から下流の範囲を有し、前記グラフト本体が、前記上流から下流の範囲に及んでいる、請求項1記載の装置。
- 上流端部と、下流端部と、内側面とを有する管状のグラフト本体と、
前記グラフト本体の前記上流端部にあり、かつ前記グラフト本体に部分的に取り付けられているクラスプステントと、ここで、前記グラフト本体は、前記クラスプステントを完全に覆い、前記クラスプステントは、上流頂部と、下流頂部と、隣接する該上流頂部及び下流頂部それぞれを連結する支柱とを画定する周囲を有し、該支柱のそれぞれが、上流部と、下流部とを有し、前記下流頂部及び前記支柱の前記下流部のみが、前記グラフト本体の前記内側面に取り付けられており、前記下流部は、前記支柱の長さの約60%〜約70%の長さであり、
前記クラスプステントと前記グラフト本体の上流端部の間にあり、かつ前記クラスプステントに隣接して前記グラフト本体に固定されたクラウンステントと、ここで、前記クラウンステントは、上流頂部と、下流頂部と、前記上流頂部及び下流頂部を連結する支柱を含み、前記クラスプステントの上流頂部が、前記クラウンステントの下流頂部間に収まっている、
を含む、血管修復装置。
- 前記クラスプステントの支柱の取り付けられていない部分の半径方向の崩壊により、前記クラウンステント及び前記グラフト本体の少なくとも部分的な半径方向の崩壊が生じる、請求項1〜26、28〜31及び38〜39いずれか1項記載の血管修復装置。
- 前記クラスプステントの支柱の取り付けられていない部分が、前記クラウンステントの前記下流頂部の近くにある、請求項1〜26、28〜31、38〜39及び41いずれか1項記載の血管修復装置。
