Abstract
Description
本明細書で使用される場合、「約」という用語は、「約」という用語に続く値の±10%の範囲内である値を表す。
本発明は、約2,000mg~約4,000mgのβ-GPAを1日2回投与することによって癌を処置する方法を特徴とする。本発明者らは、驚くべきことに、この投与計画が予想されるよりも高いレベルの循環β-GPAをもたらすことを発見した。
β-GPAは、近年、転移の抑制に有効であることが見出された。作用機序は、クレアチン輸送及び/又はクレアチンキナーゼの阻害と仮定された。クレアチンリン酸系は、肝臓の低酸素に耐えるためにATP生成のためのエネルギー貯蔵として作用することにより、肝臓における播種性癌細胞の生存を増強することによって、転移を促進する。癌細胞へのクレアチン輸送の阻害は、ATPの産生における使用に利用可能なクレアチンリン酸の量を制限する。クレアチンキナーゼの阻害は、クレアチンリン酸からクレアチンへの変換を介してATPの産生を阻害する。
適切な賦形剤、及び上記の薬学的に許容される塩の1つ又は複数を含有する組成物は、本発明の範囲内にある。組成物は、薬学的に許容される賦形剤を含有する医薬組成物、食物として許容される適切な賦形剤を含有する食物組成物、又は美容的に許容される賦形剤を含有する化粧品組成物であり得る。
いくつかの実施形態において、医薬組成物はさらに、抗増殖活性を有する付加的な化合物を含み得る。本発明の化合物及び医薬組成物は併用療法で配合及び使用され得る、すなわち、本化合物及び医薬組成物は1つ又は複数の他の所望の治療薬又は医学的手順と共に配合され得るか、或いはそれらと同時に、それらの前に、又はそれらの後に投与され得ることも認識されるであろう。併用レジメンにおいて使用するための治療(治療薬又は手順)の特定の組合せは、所望の治療薬及び/又は手順適合性、並びに達成すべき所望の治療効果を考慮に入れることになる。使用される療法が同じ障害に対して所望の効果を達成し得るか、或いはそれらが異なる効果(例えば、任意の有害作用の制御)を達成し得ることも認識されるであろう。
方法:600mgBID、1,200mgBID、2,400mgBID、又は3,600mgBIDのレジメンにおいて、対象にRGX-202(β-GPAの高圧縮性の塩形態)を投与し、投与後24時間にわたって、対象から血漿サンプルを採取し、以下に記載されるプロトコルを用いてβ-GPAのレベルについて試験した。
方法:進行性胃腸腫瘍(例えば、局所進行性及び切除不能、又は転移性)を有する対象に、単独療法として、或いはイリノテカン、フォリン酸、及びフルオロウラシルと組み合わせて、複数用量の経口投与RGX-202を投与した。研究の単独療法アームでは、各28日サイクルの1~28日目に、RGX-202を1日2回又は3回経口投与した。投与計画は、患者が登録されたコホートに依存した。研究の組合せアームでは、FOLFIRIと組み合わせて、単独療法アームについて記載されたものと同じ方法でRGX-202を投与した。FOLFIRIは、各28日サイクルの1日目及び15日目に、フォリン酸(400mg/m2)を2時間かけて静脈内投与すると同時に、イリノテカン(180mg/m2)を90分かけて静脈内投与し、その後フルオロウラシル(5-FU)(400mg/m2)を静脈内にボーラス投与してから、5-FU(2400mg/m2)を46時間にわたって静脈内注入することによって行った。

Claims (21)
- それを必要としている対象において癌を処置する方法であって、約1,500mg~約4,000mgのβ-グアニジノプロピオン酸(β-GPA)又はその薬学的に許容される塩を前記対象に1日2回投与することを含む方法。
- 前記方法が、約2,400mg~約3,600mgの間のβ-GPA又はその薬学的に許容される塩を前記対象に1日2回投与することを含む、請求項1に記載の方法。
- 前記方法が、約2,400mgのβ-GPA又はその薬学的に許容される塩を前記対象に1日2回投与することを含む、請求項1又は2に記載の方法。
- 前記方法が、約3,600mgのβ-GPA又はその薬学的に許容される塩を前記対象に1日2回投与することを含む、請求項1又は2に記載の方法。
- 前記方法がさらに、1つ又は複数のさらなる抗癌療法を行うことを含む、請求項1~4のいずれか一項に記載の方法。
- 前記1つ又は複数の抗癌療法が、放射線療法、手術、及び/又は1つ若しくは複数の治療薬を含む、請求項5に記載の方法。
- 前記1つ又は複数のさらなる抗癌療法が、フォリン酸、フルオロウラシル、イリノテカン、及び/又はオキサリプラチンを含む、請求項5又は6に記載の方法。
