    P E T I T I O N

    Comes now, NATHAN P. REYES, of legal age, Filipino, single, and a resident of Purok
    Sagrado, Valencia Site, Polomolok, South Cotabato and unto this Honorable Court most
    respectfully alleges:

    1. That your petitioner is a citizen of the Philippines, 25 years of age and was born on
    January 9, 1984 in Polomolok, South Cotabato and a resident thereof since 1984 with its
    office address at Miranda St., Polomolok, South Cotabato with telephone number (083)
    382-1610;

    2. That petitioner has satisfactorily completed the required course in Law at Cor Jesu
    College, Digos City on October of 2012;

    3. That he has passed the required Bar Examination in the year 2013 with the Roll of
    Attorney No. 14311 and since April 30, 2014 was then submitted as member of the
    Integrated Bar of the Philippines (IBP) in good standing as certified to his chapter, copy
    of Certificate of Good Moral Character is hereto attached as Annex “A” and at present
    practicing law in the Philippines;

    4. That he desires to be commissioned as Notary Public for and in the Municipality of


    Polomolok, Province of South Cotabato and where this Honorable Court exercises its
    jurisdiction and that he possessed all the qualifications and none of the disqualification
    for said office;
    5. That his three (3) specimens of his signature and a true impression of his dry seal
    pursuant to law are hereby as follows:

    6. That three (3) copies of his 2x2 ID pictures are hereto attached pursuant to New Notarial
    Law.
