Subcontractor HSE Mobilization Checklist

1. Subcontractor HSE Plan with appropriate signatures


2. Letter of acceptance to utilize CB & I HSE Project Procedures and Forms as the working
document
3. Designated Competent Persons Form completed with appropriate support documentation for
mobilization activities scope
4. Certification and inspection forms for all tools and equipment to be mobilized during the
mobilization period. This is to include assured grounding color coding and inspection; rigging
tackle color coding and inspection, heavy equipment, cranes, boom trucks, ladders, scaffolding,
fire extinguishers, light vehicles, transportation buses, etc.
5. Operator Certification for all heavy equipment including dump trucks, forklifts, cranes, etc.
6. List of Spill Kit components for all heavy equipment. Subcontractor HSE Training documentation
relative to HSE Orientation, Fall Protection and other HSE courses
7. Certification of the DCP (Designated Competent Person) for Fall Protection if working at heights
is an activity during mobilization and copy of their SENA Certification.
8. Certification of the DCP (Designated Competent Person) for Confined Space, Lockout/Tagout,
Excavations, etc if these activities are a part of their work. Fitness for duty medical
examinations for all personnel designated for mobilization
9. Letter of certification confirming compliance with the Project Substance Abuse Program
10. CV for all HSE staff for formal approval
11. Organization chart to reflect appropriate chain of command for proper application of the
disciplinary action program, Area Hazard Assessment Program and overall management
12. Copy of the Health and Safety Act
13. Copy of Companys Occupational Health Risk Analysis Matrix
14. Overall Project Hazard Assessment Form and specific JSA for applicable mobilization activities.
These must be approved and signed off by CB & I. These must match the detailed Method
Statement and be consistent with the mobilization activities schedule.
15. COPASO documentation
16. ARP Certification Automatic Monthly Notification of Payment or Cessation of Services.

17. Candidates submitted for Permit To Work Training training must be completed prior to any the
initiation of field activities
18. Certifications of approved drinking water and ice supply
19. Submission of Plan for site office, lunch room, meal provision. Including provisions for
bathrooms, hand washing & shower facilities etc (if applicable) in compliance with Law 9 of 1979
for the Preservation and Improvement of Individual Health in the Workplace.
20. Submission of MSDSs for approval prior to purchase and/or arrival to site
21. Submission and approval of PPE.
22. Submission of First Aid Kit components and backboard complying with Colombian Legislation
and Project Requirements.
23. Submission of Multi-Gas Detector Equipment and Operator Certifications
24. Avantel Radios and Cell Phones (adequate for intrinsically safe environment if applicable) for
Emergency Communications.
