Our plan for the Better Care Fund

This easy read booklet tells you about


East Riding of Yorkshire Health and
Wellbeing Boards plan for the Better
Care Fund. March 2015

Better Care Fund


This easy read booklet tells you about
our plan for the Better Care Fund.

The Better Care Fund is a pot of money


that is used to make sure health and
social care services work together.

It does this by helping NHS and councils


work closely together and put the
people who use services at the centre
of their work.

Our vision for health and


social care services

We want to make sure that health


and social care services work closely
together to help people in East Riding
get Better Care.

This means that everyone needs to work


differently to make sure this happens.
This includes patients, carers, members
of the public and health and social
care professionals.

Our plan for better health and social


care services has come from our Joint
Strategic Needs Assessment (JSNA).

JSNA

A Joint Strategic Needs Assessment


(JSNA) looks at the health needs of
people in East Riding of Yorkshire. It looks
at what is happening now and helps us
to plan health services for the future.

East Ridings Joint Strategic Needs


Assessment (JSNA) tells us that there are
three areas of health and social care
services we need to look at.

These are:
1. Health Services for older people.

2. Health Inequalities this is when some


people do not get as good
healthcare as other people.

3. Mental and physical health for


children and young people.

East Riding of Yorkshire Council,


East Riding of Yorkshire Clinical
Commissioning Group (CCG) and the
Vale of York Clinical Commissioning
Group (CCG) have said this plan is OK.

Over the past year we have worked


closely with patients, carers, members of
the public and health and social care
professionals to look at our plan (vision)
to get the best health and social care
services for people in East Riding.

Our plan looks at 4 main things. We


want:

1. People to have more power and


control over their own health and
care - Health and social care services

will support this work. They will help


people to think about their health and
care needs and offer support and
services to help people know how to
stay well.

2. Services to be run in the local


community More health and social

care services will be run in the local


community. We will set up Hubs to help
this happen. A Hub might be a building
where health and social care services
are based or lots of buildings where
health and social care services work
closely together.

3. Health and social care services to


work together locally through a Hub
so they know what people need

Local hubs will break down barriers so


that people get the service they need
at the time they need it, not when they
are in crisis.

4. There to be clear hospital services


that support these ideas with expert
help Only people who have complex

health and care needs or ill patients


who need expert help will use hospital
services. All other health and social care
services will be run in the community.

We think that these ideas will make a


big difference to the lives of people in
East Riding of Yorkshire and give people
the best health and social care services
locally.

But to be able to do this we need to


make sure that health and social care
services work closely together.

The case for change Why


we need to do this and what
we need to do
There are lots of health and social care
services in East Riding of Yorkshire. Some
of these services are spread across the
county.

In the past health and social care teams


have not worked closely together. This
has meant that lots of people only get
a service when they are in a crisis. This
needs to change.

We have looked at lots of information


about health and social care services to
see what needs to change in the future
to make these services work better.

We want to make sure health and social


care services move away from hospitals
and nearer to where people live.

To do this we will need to:


1. Make services easier to access and
use.

2. Make sure health and social care


services work closely with primary
care services. This means GPs and
other community services.

3. Make sure services for people with


complex needs, social care and
mental health work as a big team.

4. Make sure these big teams have


support from medical services.

5. Make sure there are other services


for people so they do not have to go
to hospital.

6. Look at different types of support and


different ways we can pay for health
and social care services.

7. Make sure the local community


have the power to help make
services better.

To be able to do these things we need


to make sure that there are local teams
spread across East Riding of Yorkshire
that can offer health and social care
services 24/7.

Plan of Action
We have looked at what needs to
happen over the next year and have
written a plan to say how we will do this.

The plan looks at what we will do, who


will do it and when it should be done
by. This will help us meet our targets and
deliver good services to the people of
the East Riding of Yorkshire.

Our main target is to make sure people


are not admitted to hospital if they
dont need to be and that more people
get the care and treatment they need
closer to home.

We have set up a Better Care Fund


Board to make sure the plan will
happen. The Health and Wellbeing
Board will check that the Better Care
Fund Board are doing their job and that
the plans are working well.

Risks
East Riding of Yorkshire CCG, Vale of
York CCG and the Local Council work
closely together to make sure health
and social care organisations are
included in decisions and plans.

Together we have looked at our plans


for the Better Care Fund and the things
that could go wrong (these are called
risks).

We have thought about what we can


do to make these risks less likely to
happen and what we can do if things
go wrong.

How Better Care fits with


other plans.
Better Care works well with all of our
future plans.

Better Care

The Better Care Fund Plan supports the


work of the Joint Health and Wellbeing
Board Strategic Plan and is also
included in all of our joint plans with the
local council for health and social care.

East Riding Clinical Commissioning


Group (CCG) think that this will help to
make sure services are more accessible
and person centred.

How we make sure our


services work to what the
Government says.

We need to make sure that adult social


care services in East Riding of Yorkshire
still get the funding they need to support
the local community.

We need to make sure our services


do what the law says. The Care Act is
a new law for everyone. This law may
mean that more people get health and
social care support.

We have set up a board called the


Care Act Executive Board to look at this
new law and to make sure our services
are doing what they should.

What people have told us


about the Better Care Fund
We want to make sure that people who
use services and their carers are at the
centre of our services. People have told
us that this is very important.

We spoke to lots of people about our


Better Care Fund Plan. We ran lots of
workshops, presentations, spoke to
people on the telephone and put a
form on our website so that people
across East Riding of Yorkshire could tell
us what they thought of our plans.

People told us that they want their care


to be built around what they need,
and that this should be at home or at a
place near to their home.

Better Care

We ran workshops with local groups


and organisations to find out what they
thought of our plans. We have used lots
of the things people and organisations
have told us to write our Better Care
Fund Plan.
