IMPORTANT INSTRUCTIONS FOR ON LINE APPLICATION


Candidates are required to go through the Information Bulletin carefully before
attempting for online application.
Any application not in compliance with the conditions specified in the Information
Bulletin is liable to be rejected.

1 Application for SMFWBEE-2024 has to be done online only. There is no printed application
form.

2 Ensure that you are filling genuine application form available online at www.smfwb.in /
www.smfwb.formflix.org
3 It is essential to have a mobile number and unique email ID.
All future communications will be received from and sent through the registered mobile
number and email ID. Communications from candidates from any other mobile number or
email ID cannot be considered genuine/official and hence are liable to be ignored. Faculty will
not be responsible for non-receipt of any communication by the candidates if the mobile
number and/or the email ID is found wrong/non-existing/changed.

4 Candidates are advised to study the Guidelines of Online Application thoroughly before
attempting online application.

5 Once the registration details i.e. name, father’s name, mother’s name and date of birth are
entered and submitted, they cannot be changed/modified/edited under normal
circumstances.
Also the information must match exactly with the school admit cards, mark sheets,
certificates, photo identity cards, caste/category certificates etc. which you have to produce
to enter the examination hall, during counseling, during admission & during registration with
the Faculty.

6 Do not attempt to make any duplicate application.

7 Do not share your application number; pass word, security question/answer with anyone.

8 Upload scanned copy of coloured photograph and signature as described in the Information
Bulletin.

9 If you receive any SMS/email regarding discrepancy in the photograph/signature uploaded


by you, take corrective action immediately within one day.
10 a) The amount of application fees to be paid for appearing in the SMFWBEE-2024 is Rs.
500 (Rupees five hundred only) The Application Fees can be paid in cashless mode
by ‘Net Banking’/ ‘Debit Card’/ ‘Credit Card’.
b) Rs. 100/- as Registration Fees for participating in Counseling to be paid by Net
Banking’/ ‘Debit Card’/ ‘Credit Card’ only (No Challan Payment shall be accepted at
this stage).
c) Payment status is updated online and hence should show ‘Ok’ after the payment is
confirmed from bank within next 12-24 hours max. But in some rare cases it may be
delayed due to the Bank’s network delay/failure or transaction failure at the
Electronic Payment Gateway (EPG). If the payment status does not show ‘OK’ after 48
hours after making the payment then the candidates are advised to make payment
again. Once payment is confirmed, candidate will then find the option to ”PRINT
APPLICATION FORM” with all details.

d) If the payment was made by net banking/debit card/credit card, and the transaction
failed at EPG, the candidate has to make payment once again.

Do not wait for the last day to make fee payment in order to avoid payment failure
by Bank or EPG (Electronic Payment Gateway).

11 Keep copies of confirmation page, admit card in safe custody. It is not advisable to generate
any duplicate copy.

Introduction
State Medical Faculty of West Bengal (hereinafter referred to as “The Faculty” is an
autonomous body constituted by the Government of West Bengal in terms of the provisions of
the statutes of the Faculty in this respect. The Faculty is empowered with the authority to
conduct Examination of various Para Medical Courses as per course curriculum approved by
the State Government as also to award Diploma / Certificate to the successful candidates of
the approved courses. The Faculty holds the status of an “Examining Authority” in respect of
the approved courses.
The courses being offered by the State Medical Faculty of West Bengal are approved
courses of the State Government and the Diplomas / Certificates being awarded by the Faculty
are recognized by various States as well as Central authorities for the purpose of employment.

For the academic year of 2024, the State Medical Faculty of West Bengal will conduct the
Entrance Examination for admission to Para Medical Courses in different Medical
Colleges/Govt. Institutions / Non-Govt. Affiliated Institutions.

The Courses being offered by the “Faculty” with duration are as follows:
 Diploma in Medical Laboratory Technology [Pathology, Microbiology & Biochemistry] : DMLT [ Tech] : 2 years +
6 months (Six Months) compulsory post examination training.
 Diploma in Radiography [ Diagnostic ] : DRD [ Tech ] : 2 years + 6 months (Six Months) compulsory post
examination training.
 Diploma in Physiotherapy : DPT : 2 years + 6 months (Six Months) compulsory post examination training.

Rules governing the courses

Eligibility for admission :


1. Any person seeking admission to any one of the courses must fulfill the following
conditions:
[a] That he/she has passed the Higher Secondary (10+2) or equivalent Examination
recognized by any Indian University or a duly constituted Board with pass

[b] That he/she will attain the age of 17 years as on 31st December, 2024.
Procedure for submission of Documents
(i) Attested copies of Original Mark Sheet for H.S / equivalent Exam, Admit Card for proof of age,
EWS/SC/ST/OBC-A/OBC-B/PH certificate should be submitted during Online Application .
Mode of selection
(i) Selection of candidates will be made in accordance with marks obtained in Entrance Examination.
The Merit List of candidates will be tentatively available on 01.08.2024 at the above office of the State
Medical Faculty of West Bengal as well as in the website of the Faculty (www.smfwb.in/
www.smfwb.formflix.org )
(ii) After selection or publication of Merit List or during admission if it is found on scrutiny that an applicant
furnished wrong information in the Application Form, his/her application for admission will be summarily
rejected even though his/her name appears in the Merit List and also a seat allotted in the respective
course.
(iii) Counseling will be made for admission in different Medical Colleges / Govt. Institutions
/ other Affiliated Institutions in respect of successful candidates from the Merit List in
order of Merit subject to availability of seats.

(iv) A candidate allotted a seat after particular Counseling will have to deposit
Rs.20,000/- at the time of Admission through Counseling by Demand draft drawn in
favour of State Medical Faculty of West Bengal, payable at Kolkata, failing which
his/her seat will be cancelled. Second installment of course fees, i.e. Rs.20,000/- to be
paid within Nine (9) months from the date of admission.

(v) The candidate will have to appear personally for the selection of a seat during
counseling. If any candidate fails to appear personally his/her authorized representative
will have to produce valid authority letter and undertaking from that candidate. The
authorized representative without valid undertaking and authority letter (as per
Proforma Annexure I) shall not be allowed to participate in the counseling and the
candidate shall forfeit the claim for a seat.

(vi) The candidates or authorized representatives for personal appearance during


counseling should bring along with them all certificates including
EWS/SC/ST/OBC(A)/OBC(B)/ PH [ with locomotor disorder only] and Mark Sheets in
original for verification, failing which they shall not be allowed to participate in the
allotment process in counseling and their claim will be forfeited.

(vii) No separate intimation will be sent to any candidate by the State Medical Faculty for
counseling and / or allotment of seats.

(viii) Allotment of a seat of a particular course and institute for a candidate will be
awarded according to Merit List and as per availability of seats.

(ix) The fact that a candidate has appeared for allotment shall not entitle him / her
for allotment / re-allotment, as the same is subject to availability of seats and
acceptance of the available seats by the candidates based on his / her rank in
the Merit List.

(x) Second Counseling will be held provided there are any vacant seats after first
counseling at a fixed venue, date and time to be notified by the State Medical
Faculty at the appropriate time.

(xi) In the matter of allocation / admission of a candidate to a particular course /


institute, the decision of the President, State Medical Faculty of West Bengal,
shall be final and binding.

(xii) Bank Draft in favour of State Medical Faculty of West Bengal would be released
forthwith at the counseling hall if the candidates decline their admission due to
non-availability of seats according to their choice.

Reservation of seats
Seats for EWS/ SC/ST/OBC-A/OBC-B/PH (with Locomotor disorder only)* will
be kept reserved as per Statutory provisions.
The Disability Certificate should be produced from the duly constituted authorized
Medical Board as and when asked for.
Fees payable by the students
For Govt. Institutions
A candidate allotted a seat after particular Counseling will have to deposit their first
installment of Rs.20,000/- at the time of Admission through Counseling by
Demand draft drawn in favour of State Medical Faculty of West Bengal, payable at
Kolkata. Second installment of course fees, i.e. Rs.20,000/- to be paid within
Nine(9) months from the date of admission.

The above course fees exclude student’s Registration fees (Rs.800/-)(For one time)
and Examination fees (Rs.800/-) each for Regular & Supplementary examinations.
The above Examination Fees are payable by all the students of Para Medical
Courses admitted in Government Medical Colleges/ Govt. Institutions as well as
Affiliated Institutions.

The candidates will have to bear the entire expenses for undergoing the courses
and the Faculty will not award any stipend for this purpose. However, stipend of
Rs.2000/- per month is admissible to all the students undergoing Compulsory
post examination training for 6 months for all the Courses soon after passing the
Final Examination.
For Affiliated Institutions

For Fees payable by the students of Affiliated Institutions, the website


www.smfwb.in of the State Medical Faculty of West Bengal may be visited.
Fees structure of different affiliated institutions may be seen.

However, at the time of counseling, the students are required to pay


Rs.20,000/- for admission even to affiliated institutions, and, all other fees
payable to affiliated institutions will be paid at the time of joining the
respective Affiliated Institution.

MODE OF FEES PAYABLE BY THE STUDENT OF ALL THE INSTITUTES

Registration Fees, Exam Fees & other fees are payable separately by all the students admitted in
Affiliated Institutes (both Govt. & non-Govt.) at their respective Institutions after admission, at the
rate of Rs. 800/- for Registration Fees (For one time) and Examination Fees of Rs. 800/- each for
Regular Preliminary & Regular Final, and, for Supplementary Preliminary &
Supplementary Final examinations as well.

 In case of discontinuity of studies on valid grounds by a student of any of the Medical


Colleges/Govt. Institutions/ Affiliated Institutes, refund of course fee will be made after
deduction of Rs. 3000/- provided an application is made through online mode just within a
month from the date of admission, along with Consent Letter from the guardian of the
concerned student.

 Refund Course Fees after one month is NOT admissible in any way.

Examination of the courses


The Examination of the respective courses will be conducted in terms of the
provisions contained in this respect in the respective course
curriculum/Regulations approved by the State Government.

Internship Training
a) Internship Training (Post Examination Training) for a period of 6 months at a
stretch is compulsory, and, the same should be completed just after passing the Final
Examinations. The said Training should commence uniformly as per the notified date by
the Faculty.
b) 15 days relaxation would be considered from the notified date for starting of
Internship Training on special ground only by the concerned Institute.
c) After 15 days , those Interns, who could not start his/her Internship Training ,would
be allowed to re-start his/her Internship Training within One Year on payment of Fine
amounting to Rs. 1000/- without Stipend, subject to approval of the Faculty.
d) Beyond One year but within 2½ years , after passing month/year of Final
Examinations, re-starting of the Internship Training would be without Stipend and on
payment of a fine amounting to Rs. 2000/-, subject to approval of the Faculty.
e) Finally, a passed-out candidate has to complete Internship Training within a period
of 3 Years from the date of Passing of the Final Examinations.
Eligibility for appearing in any Examination
Internal Assessment :
 A Student has to pass the Internal Assessment from their respective Medical
Colleges / Govt. Institutions / Non-Government Affiliated Institutions in each
subject in each course for Preliminary and Final [Regular & Supplementary]
Examinations.

 The qualifying pass mark in Internal Assessment is 50% each in Theory and
Oral/Practical. However, this mark would not be added to in their respective
examination – that is, Preliminary or Final.

 Status ( Pass/Fail/Absent) in the Internal Assessment Examination will be sent


to Faculty within the specified Time.

Fees payable by a student for appearing at the Examination with shortage of attendance

Any student with shortage of attendance intending to appear at any examination will have
to pay the following Fees:
[i] Minimum 75% (Seventy-five percent) without any fine.
[ii] For percentage between 60 and 74% (Sixty and seventy-four percent), a fine
of Rs. 2000/- (Rupees Two Thousand) per examination per course be paid.

No candidate will be allowed to make good of his/her shortfall in attendance below 60%
without being re-admitted in the course in one of the subsequent sessions. However, the
Faculty reserves the right of admission of any such discontinued students in the
subsequent academic session/s due to non-availability of seat.

Cancellation of admission

The State Medical Faculty shall have the right of cancellation of admission and/or studentship of any
student during the session if he/she is found guilty of any of the following charges:
a] Suppression of information
b] Supply of false information
c] General misconduct unbecoming of a student
d] Adoption of unfair means at the examination and
e] General indiscipline.

General guideline concerning the courses

[i] The State Medical Faculty of West Bengal, with the prior approval of the State Government, shall
reserve the right to alter / modify / amend any or all the rules concerning conduction of the above
courses in any Institution.
[ii] No Hostel facilities will be available
[iii] Annexure I - Performa of authorization letter
[iv] The Faculty does not have any recruitment cell.

Change in Course or in Institution after admission is not allowed :

Once a candidate is admitted through counseling to a course in an Institute,


change either in course, or in Institution is not allowed on any grounds at
any subsequent stage.

Transfer of Training is not allowed :

Each student is required to undergo compulsory Internship Training (Post-


exam practical training) for a period of six months, after passing the final
examination in the very same Institution from where the student appeared at
the final examination.

13 The Mode of Examination
The Common Entrance Examination will be done in Pen and Paper Mode (OMR based
examination) for admission to different Para Medical Courses – 2024 in various Medical
Colleges, Govt. Institutions, and Non Govt. Affiliated Institutions.

14 Subjects of Examination

The “SMFWBEE-2024” will be on Section – A : Physics and Chemistry,


Section – B: Biology

MCQ type questions would be set up in Physics, Chemistry & Biology as taught in H.S.
or equivalent examination Level.

16 Pattern of Question Papers in SMFWBEE-2024


The total marks in Physics, Chemistry and Biology paper would be 25 and 25, and 50,
respectively, and the total full marks would be 100. Each Question would be of 1 mark.
In every subject, all questions will be of Multiple Choice Questions (MCQ) type,
with four options against each of the questions.
Only one option is Correct.

17 The Question for MCQ Exam will be in Bilingual Mode - English / Bengali

18 Mode of answering in the offline examination:


f) Questions are to be answered on specially designed optical
machine readable response (OMR) sheet, which will be evaluated
by the method of Optical Mark Recognition by specially designed
machines only and therefore it is very important to follow the
correct method of marking.

g) There will be four alternative answers for each question.


Candidate will indicate response to the questions by darkening the
appropriate circle completely with blue/black ball point pen.

h) Any other kind of marking e.g. filling the bubble incompletely,


filling with pencil, cross mark, tick mark, dot mark, circular mark,
over writing, scratching, erasing, white ink, marking outside the
bubble etc. may lead to wrong/partial/ambiguous reading of the
response. SMFWBEE-2024 will be, in no way, responsible for
such eventuality.
Response marking cannot be edited/changed/erased/modified.
Hence it is advised that the candidate should be sure about the
answer before marking and should not put any stray mark on
which he/she does not want to attempt.

d) There will be no negative marking.

19 Ranking Methodology and publication of Merit Lists:


Based on the papers (subjects) appeared in the SMFWBEE-2024 followed by marks
scored there-of, Seven separate Merit Rank Lists shall be published as per the following
details:
A. Merit Rank List-I: Combined [General+EWS+ST+SC+OBC(A)+OBC(B)+PH]
B. Merit Rank List-II: Physically Handicapped
C. Merit Rank List-III: EWS
D. Merit Rank List- IV: ST
E. Merit Rank List-V: SC
F. Merit Rank List-VI: OBC(A)
G. Merit Rank List-VII: OBC(B)
a. Ranking of the said Merit List shall be done in the descending order of
total marks scored in all the subjects as stated above. However, there
may be ties and as such tie-breaking rule shall be applicable for them in
determination of the merit rank.
b. Separate reserved category merit lists shall also be published for
respective category of students e.g. EWS Rank, SC Rank, ST Rank, OBC-A
Rank, OBC-B Rank and PH Rank.

c. This said list shall be prepared based on the score of candidates


appeared in the combined paper of Physics and Chemistry and one
paper in Biology.

20 Tie–breaking Methodology in determination of Merit Rank:


Tie-breaking Rule for Merit Rank List- I to VII
a) Higher marks in Biology.
21 Final Tie-breaking for Merit Lists:
After application of the Tie-breaking Rules as applicable if there be still ties, the same
will be broken by the date of birth (DOB) of the candidates, with the older candidate
having preference over the younger one, if there be still ties in date of birth (DOB) then
the candidate who got higher percentage of marks in Physics, Chemistry and Biology in
Higher Secondary Examination or equivalent Examination, would be considered as
higher Rank in the Merit List. This is applicable for both the Combined and individual
Merit list.
22 Rules of the examination (SMFWBEE-2024 ): Rules to be followed during the examination is
given in APPENDIX-I
23 Eligibility and academic qualification criteria for appearing in SMFWBEE-2024
a) Citizenship: Applicant must be a citizen of India.
b) Age Restriction:
(i) The minimum age is 17 (seventeen) years as on 31.12.2024. There is
no upper age limit in appearing in the SMFWBEE-2024.

24 General Academic Qualifications criteria :


For appearing in the SMFWBEE-2024 :
a) Candidate must have passed the Qualifying Examination i.e. the ‘10+2’
examination with Physics, Chemistry & Biology with individual pass marks
(both in theory and practical) in regular class mode.
b) The Board of the said Qualifying Examination must be recognized by the
Central Government or State Government concerned, as per - APPENDIX-II

25 Important information to candidates regarding eligibility:

SMFWBEE-2024 do neither verify the information provided by the candidate during on line
application nor verify any certificate of date of birth, domicile, income, reservation
category, academic qualification etc. for deciding the eligibility of the candidate. Admit
cards issued only on the basis of the information provided by the candidate. Also there is
no scope of changing/correcting any information after the last date of online application.
FOR VERIFICATIONS OF ALL RELEVANT DOCUMENTS :
UPLOAD ONLY SCAN COPIES OF ORIGINAL DOCUMENTS
However, If at any stage after examination it is found on scrutiny that the applicant is
otherwise ineligible, his/her candidature shall be treated as cancelled even if he/she
secured a Merit Rank in the SMFWBEE-2024.Hence, securing a Rank in the SMFWBEE-2024
does not constitute a right/guarantee in favor of a candidate for his/her claim for
admission who ultimately fails to comply with the specified norms and standards.
26 Competent Authorities for the issuance of SC/ST Certificate for candidates claiming under
such reserved category of seats:
SC/ST Certificates are to be issued by any of the following authorities
within the respective local jurisdictions:
A. In case the candidate ordinarily resides within such jurisdictions.

27 Competent Authorities for the issuance of OBC-A / OBC-B Certificate for candidates
claiming under such reserve category of seats:
As per Notification vide No. 374(71)-TW/EC/MR-103/94 dated 27/7/1994, read with
Memorandum No. 1204-SBCW/MR-67/10 dated 27/7/2015 issued by Backward Classes
Welfare Department. Govt. of W.B., the Sub Divisional Officer of a Sub- Division in a District
is t h e certificate issuing authority. In Kolkata; such certificate is issued by such an officer
as the State Government by modification authorizes. Accordingly, the District Welfare
Officer, Kolkata and Ex-officio Joint Director, BCW has been notified to act as the certificate
issuing authority in respect of Kolkata covering the jurisdiction of the Kolkata Municipal
Corporation.
Non-creamy Layer Certificate :
OBC-A and OBC-B reserved seats are to be allotted to OBC-A and OBC-B candidates
provided they belong to Non-creamy layer. Hence, Non-creamy Layer Certificate is required
to be submitted by the OBC-A and OBC-B candidates. The said Non-creamy Layer
Certificate has to be obtained from the same Authority who issued OBC-A and OBC-B
Certificate. This Non-Creamy Layer Certificate shall not be One year old i.e. the same
should be issued after 31.03.2023.

28 Reservation of Seats for Physically-Handicapped (PH) Candidates:


Reservation will be available to candidates with a minimum of 40% disability with respect to Locomotors
Disorder, subject to condition that the candidate is capable of carrying out all activities related to Theory &
Practical work as applicable to Para Medical Courses without any special concession and exemption.
The following rules are also to be maintained in this regard:-
Locomotors disability involving UPPER LIMB should not be considered.
The Locomotors disability involving LOWER LIMB is permissible, but, it should be within the following
guidelines:-
a. If it involves only ONE LOWER LIMB, it should have a minimum
of 40% and should not exceed 60%.
b. If it is involved BOTH the LOWER LIMBS, the total disability
should not exceed 60% with a minimum of 40%.
PH certificates are to be issued by any of the following authorities:-
Officer-in-Charge, or, Medical Head of Primary Health Centers, Block Primary Health Centers, State General
Hospitals, Rural Hospitals, Sub-Divisional Hospitals, or, the District Hospitals, run by the State Government
or, any hospital run by a statutory body or authority.

29 Reservation of Seats for EWS Candidates:


The Candidates belonging to EWS shall get 10% reservation of seats for admission to Para Medical Courses,
subject to production of Valid EWS Certificates issued by the appropriate authorities, along with other relevant
documents required for admission to any Para Medical Courses. EWS certificate is valid for the financial year
of issuance of certificate i.e. such certificate is to be issued after 31.03.2024.
Competent Authorities for the issuance of EWS certificate:

EWS certificates can be issued by any of the following officers of the area where the candidate and /or his
family normally resides:
a) District Magistrate/Additional District Magistrate/ Collector/Deputy Commissioner/Additional

Deputy Commissioner/ 1 Class Stipendiary Magistrate/Sub-Divisional Magistrate/ Taluka Magistrate/
Executive Magistrate / Extra Assistant Commissioner.
b) Chief Presidency Magistrate/ Additional Chief Presidency Magistrate/ Presidency
Magistrate.
c) Revenue Officer not below the rank of Tehsildar.
d) Sub-Divisional Officer.
30 Legal jurisdiction:
a) All matters pertaining to conduction of SMFWBEE–2024 shall fall within the
jurisdiction of Kolkata only.
b) The Faculty will not be a party pertaining to any dispute arising in the process of
admission to any course of study through SMFWBEE– 2024.
31 Procedure for submission of application Form , payment of Examination Fees and downloading/printing
of the Confirmation Page:
32 Application for the SMFWBEE-2024 has to be made ONLINE only through the web- portal
http://www.smfwb.in & http://www.smfwb.formflix.org
For making the application the candidate needs to have a working mobile number and or
an email ID. All future communications will be done using them only.
33 Filling up of Application Form
The candidate will first enter the registration details i.e. Name, Father’s Name, Mother’s name and Date of
birth.
Be careful while entering the registration details. It cannot be changed/edited/modified under normal
circumstances. Also the information must match exactly with the school admit cards, mark sheets,
certificates, photo identity cards, caste/category certificates etc. which you have to produce to enter the
examination hall, during counseling, during admission and during registration with the Faculty.
a) Any attempt by a candidate to make duplicate registration will be stopped by the system.
b) Thereafter, candidates are required to fill up various personal information regarding, Nationality,
Religion, Address, Choice of examination zones, academic detail etc.
c) At the end, the candidate has to choose password and security question/answer.
d) At this stage the candidate can review the entire application and make changes as necessary.
e) The candidate will then be asked to submit the application.
f) Then an application number will be generated and SMS/email will be sent to the candidate.
g) Candidates have to remember password and security question/answer. The system is automatic.
There is no way to recover the password if it is lost.
h) It is not possible for any person/agency to change/edit/input any information without knowing the
password. Hence candidates must not share their password with anybody. The Faculty will not be
responsible for any change resulting from sharing/ divulging the password.
At this stage candidates may logout or continue to next step.

34 Uploading of images :
At this stage the candidate is required to upload original document scanned images of
his/her recent colour photograph, signature and qualification/caste certificate all in one
go.
Essential features of the photograph
a) The Photograph must be recent coloured one.
b) It should be taken in professional studio. Low quality photograph by Mobile Phone
or self composed portraits is not acceptable and may likely to be rejected.
c) Background of the photograph must be of very light colour.
d) The face of the candidate should occupy at least 50% of the entire space for
photograph looking straight to the camera.
e) Spectacles of dark or tinted glasses are not acceptable.
f) The photograph must match with the candidates’ appearance during the
examination.
g) Copy of same photograph need to be produced for entering the examination centre,
during counseling, admission, registration with the Faculty etc.
Format, storage size and physical dimension of the images must be as under:

In case the images do not conform to the above requirement, it will not be possible to
issue the admit card. In which case, only the candidate will remain responsible.
SMFWBEE does not check the images. However, if any discrepancy comes to its notice a
SMS will be sent to the candidate’s registered mobile number. The candidate then has
to log in and upload corrected image.
35 Payment of Examination Fees
After up-loading of images candidates will be directed automatically to the fee-payment
step.
a) The Examination Fees can be paid in cashless mode by ‘Net Banking’/ ‘Debit Card’/
‘Credit Card’
b) There is no other method of paying the application fee besides the above.
c) The amount of Examination Fees
The amount of fees to be paid for appearing in the SMFWBEE-2024 is Rs. 500
(Rupees five hundred only)
d) The fee once paid is not refundable under any circumstances.
36 Confirmation Page On successful completion of all the above steps, candidate shall
automatically be directed to the option for downloading and printing of the
‘Confirmation Page’ which itself signifies the completion of Online-Submission of
Application for SMFWBEE-2024 and accordingly implies that the candidate’s application
is complete and accepted.
37 APPLICATION IS NOT COMPLETE UNTIL THE CONFIRMATION PAGE IS GENERATED.
Candidate should take a Print-out of the Confirmation Page and should preserve the
same carefully till the completion of admission process. The confirmation page cannot
be regenerated at any later stage. Hence it is the responsibility of the candidate to
preserve it safely.

38 Correction of application form


a) It is not possible to correct any registration data i.e. Name, Father’s Name, Mother’s
name and Date of birth.
b) If any candidate intends to correct any other information in his/her application,
he/she can do so during the given correction window period, i.e online application
duration using the helpdesk mode. The Faculty cannot entertain any request for any
correction under any circumstances beyond the correction window period.
c) Confirmation page, admit card, rank card etc. will be issued with the personal detail
as given by the candidate. In case of any problem to the candidate during admission
or thereafter due to any mistake committed by him/her, the Faculty is not in a
position to render any help e.g. issuing any letter of correction. The candidate has
to take corrective actions at his/her end with the institute, where he/she takes
admission.
39 Admit Card
a) Admit-cum-intimation cards will be generated on the notified date for the eligible
students to download from their login and take a print online from website.
Candidate has to read all the instructions as well as Do’s and Don’t mentioned
therein.
Admit Cards will be generated on the notified date for the student to download and
take a print online from website. Candidate, appearing in Offline mode, has to carry
to the examination centre a printed hard copy of the Admit Card and photo ID proof.
b) Candidates must ensure that the admit card is not mutilated/ distorted/ soiled
even by accident. It will be required during DV
c) Duplicate admit cards cannot be generated after the examination. Hence, candidates
are advised to retain their admit cards carefully in secured place in undamaged
condition for all future purpose.
d) SMFWBEE does not verify the information provided by the candidate during on-line
application. Admit cards issued only on the basis of the information provided by the
candidate. All applicants who appear to be prima facie eligible will be issued admit
cards and shall be provisionally permitted to sit for SMFWBEE-2024
All verifications are done after counseling, admission, registration with the Faculty
etc. If at any stage after examination it is found on scrutiny that the applicant is
otherwise ineligible, his/her candidature shall be treated as cancelled even if he/she had
appeared in and secured a Merit Rank in SMFWBEE-2024. Appearing in the examination
and securing a Rank in the SMFWBEE-2024 does not constitute a right/guarantee in
favour of a candidate for his/her claim for admission.
40 Allocation of offline examination center
Allocation of examination centre will be based on the choice preferences given by the
candidate during online form fill-up. However, discretion of the Faculty in allocation of
examination centre shall be final. No request for change of allocated centre will be
entertained under any circumstances for students appearing in Offline mode. Any
examination centre may be dropped if adequate numbers of candidates are not
available.

APPENDIX –II

Rules of the Entrance Examination (Offline Mode)


1. Candidates will be allowed to enter the examination centre 30 minutes before
commencement of the test.
2. Be sure about the exact location of your examination centre and means of commuting in
order to avoid any inconvenience on the day of examination.
3. No candidate will be allowed to sit for the test in any centre other than the one allotted to
him/her & mentioned in the admit card.
4. Carry the following documents in order to enter the examination centre.
a. A hard copy of admit card of SMFWBEE-2024
b. A copy of colored photograph uploaded during online application.

c. Any photo identity card in original such as Aadhar card/ Pan card / Passport/ 10
standard admit card/ School – ID card.

5. Candidates are advised to take their seats at least 15 minutes before the test.
6. No candidate will be allowed to enter the examination hall beyond the scheduled time of
commencement of the test under any circumstances.

Candidates are advised to carry only the following items with them into the examination venue:
a) Admit Card / Call letter (a clear printout on A4 size paper) .
b) A simple transparent Ball Point Pen
c) Personal transparent water bottle
d) Hand gloves are not allowed.
7. Candidates are not allowed to carry any written or printed material, calculator, pen, docu-
pen, log table, wristwatch, any communication device like mobile phones etc. inside the
examination hall. Any candidate found with such items will be reported against & his/her
candidature will be summarily cancelled.
8. Question booklets will be distributed maximum 15 minutes before commencement of the
test. Take out the OMR sheet without breaking seals of the question booklet.

9. Put your signature on the top of question booklet.


10. Read instruction given on OMR & on the cover page of question booklet very carefully.
11. Write question booklet number & roll number at the appropriate places on the OMR. If you
make any mistake while doing so do not over write. Request the invigilator to strike it out &
rewrite the correct no and put his/her signature. Wrong entry of question booklet number &
roll number may lead to rejection of the OMR if not corrected & signed by the invigilator.
12. Darken appropriate bubbles of question booklet number & Roll number.
13. Write your name in BLOCK LETTERS, name of the centre & put your signature in appropriate
places on the OMR. Do not put any stray mark anywhere else; it may lead to rejection of
OMR.
14. Put your signature at appropriate place in the attendance sheet.

16. Check that your Roll number, photograph, spelling of your name in the attendance sheet
matches with those given in your admit card. If any correction is needed, bring it to the notice
of the invigilator.

17. Question booklet seals can be opened only at the time of commencement of test and as
announce by the invigilator. Check all the pages of question booklet. If there is any damage or
missing page or any difficulties to read the question booklet, ask your invigilator to replace
the whole set from the same series.
18. Maintain silence during the test. Any conversation/gesticulation or creation of disturbances
will be deemed as misdemeanor. If any candidate is found adopting any unfair means, his/her
candidature will be cancelled and / or he/she will be debarred either permanently or for a
period for the paper as deemed fit by the Centre- in- Charge.
19. No discussion will be allowed with the invigilator regarding the question.
20. Candidates may do rough work in the space provided in the question booklet.
21. No candidate will leave his/her seat without permission of the invigilator until the test is over.
22. No candidate will leave the hall till the end of the test & all OMRs are collected & tallied by
the invigilator.
23. Candidates are NOT allowed to take his/her question booklet after the test.
24. Any candidate found to occupy a seat other than the one allotted to him/her will be reported
against & his/her paper will be cancelled.
25. If any Examinee is found impersonating he will be handed over to the police & candidature of
the original candidate will be cancelled outright.

IMPORTANT INSTRUCTIONS AT A GLANCE

1. Information Bulletin containing eligibility conditions, list of participating institutions with


no. of sanctioned seats, code no. of Institutes and reservation policy can be
seen/downloaded from website.

2. Before filling the online Application Form the candidates are required to read the
Information Bulletin carefully. For any query, please contact the helpline telephone
numbers/helpdesk email for the applicants.

3. The Admission to Courses for the session 2024-26 will be made on the basis of inter-se
merit of percentage of the Qualifying Examination through counseling by State Medical
Faculty of West Bengal.

4. It will be the responsibility of the candidate to ensure eligibility for admission before
applying for the course. If on verification at a later stage, it is found that a candidate does
not fulfill the eligibility conditions, his/her candidature will be cancelled and the fee
deposited by him/her shall be forfeited.

5. Fees of Rs. 500/- for Online form fill up is to be paid by all candidates for appearing at
Entrance Examination SMFWBEE-2024.
Subsequently , all such candidates who will take part in Counseling will also pay Rs. 100/-
separately on account of Registration fee prior to participation in counseling.
All fees are non-refundable.

6. Fees shall be deposited by the candidate through Payment gateway accessible only through
online application portal www.smfwb.formflix.org
Fee can also be deposited in the Bank through Credit/Debit cards and Net Banking alone with
the charges as applicable by the concerned banks. Bank charges as applicable is, payable by
the candidates.

7. All the candidates must verify their personal information during registration.

8. Candidates must take print-out of their online Application Form.

9. All the candidates who are allotted seats as are required to pay the Course Fees of
Rs.20,000/- by demand Draft during offline counseling.

10. Candidates will produce all the relevant certificates in original including registration fee
receipt for verification along with a signed print out of their online application form.

11. He/She shall then proceed to the Allotted Institute for completing the Admission formalities
along with all the documents. The date shall be informed on the date of counseling.

12. The allotment of seats is liable to be cancelled, if the personal data viz. Category, Sub-category,
percentage of marks in qualifying examination, gender etc. submitted by the candidates is
found to be wrong at the time of document verification of the original certificates.

13. If a candidate is admitted on the basis of the information submitted by him/ her, which is found to be
incorrect or false at any stage then his/ her admission shall be cancelled and all fees and other dues
paid by him/ her shall be forfeited. The Faculty may take further action, as deemed fit, against
the candidate in accordance with law.

14. In case of ambiguity about any rule, the interpretation of the same by the President / Secretary,
State Medical Faculty of West Bengal shall be final.

15. All legal disputes relating to admission to the Course will be subject to Courts having
jurisdiction at Kolkata only.

16. The number of institutes / colleges / sanctioned seats may increase or decrease based upon
the approval and affiliation from the SMFWB. It is reiterated that sanctioned Seats may
increase or decrease on the approval of the competent authorities based on inspection
report.

17. Candidates are advised to consult the details and updated list of institutes along
with their Course/ Fee structures on the website www.smfwb.formflix.org/ .

18. All the updated information/notices/changes/modifications will be displayed on the website.


The candidates are required to visit website regularly to get information.

19. The dates are tentative subject to change for unforeseen circumstances.

List of documents required for Verification :


The candidates, at the time of counseling must keep the following Original Certificates/ Documents/
Testimonials along with two sets of duly attested photocopies of these with them. Any candidate, who
fails to produce any of the required Certificate/Document /Testimonial at the time of counseling, will
forfeit his/her claim to admission.

(i) High School Certificate / Madhyamik Examination Admit Card


(ii) 10+2/Intermediate/Senior Secondary School Examination Certificate & Detailed Marks Card.
(iii) Character Certificate
(iv) Medical Certificate
(v) Caste Certificate
(vi) EWS Certificate
(vii) Authorization Certificate (if the candidates failed to appear in the verification centre)
(viii) Two Recent Passport size Photograph (Color)
(ix) Physically Handicapped Certificates from the recognized Medical Board.
(x) Rank Card.
