    Recho of the Special Mancom last June 8, 2021 – 9:00 am -3:pm, attended by SDS, ASDS, PSDS,
    Cluster Heads and School Principals.

    Sir Guillena,SDS, reminded all the teachers of the following;


    *Observed and follow health protocols
    * Division Office had given financial assistance to 90 COVID patients by categories
    - 3,000.00- Mild Cases
    -5,000.00- Severe Cases
    -10,000.00 Admitted
    * Continue supporting the “SAMPU MULA SA PUSO”
    * HAZARD pay
    We are encouraged to report once a week, but if your services are required/needed by the school, we
    are encouraged to report three time a week.

    School Head should check the veracity of the documents for those employees classified as
    “vulnerables”

    quarter starting June 30, 2021.

    Division Advisory No 004, s, 2021

    Nurse Brechtnald V. Sison discussed the Division Advisory No 004, s, 2021 Guidelines on COVID
    related Concerns in DepEd Davao City Division

     All personnel should report to their respective Section Chief/Head, School --- as soon as they
    know that they are affected with such. None reporting will be ground for none issuance of Work
    from Home Medical Certificate or Medical Clearance issued by DepEd.

     Failure to report immediately any incident related to COVID19 pandemic, shall be dealt with
    accordingly.
     Penalties- Any person or entity found to have committed any prohibited acts shall be penalized
    with a fine of not less than 20,000.00 but not more than 50,000.00 but not more than 6 months,
    or both such fine and imprisonment at the discretion of the court.
     Health Issues
     Pregnant-WFH - Medical Certificate and latest Ultrasound issued
     Nursing Mothers -They may opt for WFH arrangement, provided that they will coordinate with
    their School Head/School Principal. Intent letter for request for WFH, address and duly approved
    by the School Head. Photocopy of Birth Certificate of their child (until 2 years old)

    NOTE:

     It is now the discretion of the School Head to approve the WFH intent of the teacher.
     School Head shall assign task for the work from home arrangement and ensure that the work
    Week Plan (WWP) and the output of the Teachers in their individual Weekly Accomplishment
    Report (IWAR) is completed.

    Having take all matters for discussion, the Presiding Officer adjourned the meeting at
    4:20 in the afternoon. I hereby certify to the correctness of the above-quoted minutes.
