    ACCOMPLISHMENT REPORT OF HOME VISITATION-


    GRADE 5

    I – INTRODUCTION
    During these trying times, home visit is a very
    effective method of bridging the gaps between school
    and home for learners, families and teacher. It helped
    establish a strong positive communication between the
    teachers, and the learners’ parents or guardians. With
    this kind of communication, teachers are able to access
    the individual capabilities of the learners by asking their
    parents the things they learned and their difficulties
    concerning their lessons. Thus, there were few problems
    that needed immediate attention and few changes that
    will ensure the learners development in their academic
    performance.

    II – OBJECTIVE
    - To work with parents to help them provide learning
    opportunities that enhance their child’s growth and
    development.

    - To and out the condition in which the learner’s family lives


    and how to identify how these conditions affect the learner’s
    academic performance
    - to help parents improve their parenting skills and to assist
    them in the use of the home as the child’s primary
    learning environment.

    III – HIGHLIGHTS OF ACCOMPLISHMENT


    Five learners were visited during the First Quarter
    The learners and parents showed positive reaction about
    the home visitation
    The parents fully participated in all aspects of the home visit
    Parents were supportive and gave positive feedback
