Standards
Reporting Standard

Statement
Accurate reporting throughout the Slickline operation provides the means to monitor and evaluate the
effectiveness of the Slickline service delivery.

Field reports must be completed for all Slickline field operations and the reporting shall meet the company
and Client requirements. All reports must be legible, clear and accurate, while the reporting forms and format
may vary from location to location the reporting requirements for every operation must be discussed during
the pre-job briefing.

Reports provide inputs to future operations planning, equipment utilisation and status, equipment
effectiveness, client feedback and satisfaction and crew performance.

All Slickline operations

Responsibilities
Management
Management are responsible for the implementation of the Slickline Reporting Standard, ensuring all
personnel comply with its provisions whilst ensuring the Slickline Reporting Standard remains effective and
current.
Management shall: -
Provide the correct training and instruction to the relevant personnel, update and maintain
training material and ensure any new reporting requirements are properly communicated
Review the reporting requirements of each new job during the pre-job briefing and ensure the
requirements are clearly understood by the Field Supervisor
Review the completed field report(s) during the post job debrief and evaluate the quality of the
reporting and provide feedback

Field & Base Personnel


All Slickline personnel participating in wellsite operations will implement the requirements of the
Slickline Reporting Standard
Slickline Supervisors will be responsible for the accuracy of the wellsite reports
Slickline Supervisors will be responsible for completing the required reporting on time, ensuring
that any Client signatures are obtained on critical reports and copies of the same reports are
made available for the client at the well site

Reporting Standard
Pre-Job Briefing
The pre-job briefing is a formal documented process in which the Base Supervisor reviews and discusses all
aspects of the upcoming operation with the field crew. Reporting requirements shall be reviewed and the

field supervisor shall confirm his understanding of the mandatory reporting requirements by signing the pre-
job briefing meeting notes.

The reporting requirements for every new job must be reviewed during the pre-job briefing

Standard Wellsite Reporting


The Field Supervisor shall be responsible for having all of the required forms and documentation at the well
site, these will be available in a Wellsite Documents Pack. The contents of the Documents Pack may change
depending on the Client and Company requirements.

The following are examples of types of reports, while not an exhaustive list it shall be regarded as the
minimum to meet the Slickline Reporting Standard requirements: -

Pre-Job Checklist - The Checklist covers all pre-job activity, from equipment preparation to
rigging up and pressure testing and is used to ensure that all aspects of the operations are
correctly covered.
Toolbox Meeting Form - At the start of every shift a Toolbox Talk will be held at the worksite.
Additionally, a tool box talk can be carried out at any moment during an operation, for instance
when a new program phase starts, a stop the job situation arises, a change in the program or an
unexpected outcome of an operational step
Shift Handover Form - The Shift Handover Form must be completed, printed off and signed by
the oncoming shift to confirm their understanding of the current job status and upcoming
activity. Signed copies must be handed in to the operations supervisor at the end of the job
Sequence of Events - The Sequence of Events is used to record all the information about each run
in the well and the outcome of the run, as well as all the key stages of the job and the time they
occurred
Wire History Card - The Wire History Card shall be completed to ensure the maintenance of an
accurate record of the wire history. All relevant run history events, results of wire testing and
wire cut off shall be reported
Communication Summary Sheet - The Communication Summary Sheet is used to record all
critical communications with the client and the Base Supervisor. The Slickline Supervisor will
make a daily report to his Base Supervisor to give an update on the progress of the operation. In
the event an incident occurs at the well site the Slickline Supervisor must communicate
immediately with the Base Supervisor to inform him of the change in the situation at the well site
Client Satisfaction Report - The Client shall be asked to sign the Client Satisfaction Report after
every well site job, the CSR provides Management with direct feedback on the crew performance
and a measure of the Client’s satisfaction with the outcome of the job
Equipment & Personnel Record Sheet - A record of the personnel hours and the primary
equipment hours at the wellsite, divided into standby and operating hours, critical for invoicing,
equipment utilisation and maintenance tracking
Equipment Maintenance & Failure Report - All equipment related issues such as equipment
failures, damage or performance Improvements shall be reported so that repairs or maintenance
can be scheduled or equipment replaced

Personnel Appraisal Form – Every employee working shall receive at least one personal appraisal
per job from a senior person on the crew. The completed appraisal shall be presented to the Base
Supervisor
Service Ticket – Before leaving location the Service Ticket shall be completed and signed by the
client, a copy should be retained by the Client and a Copy given to the Base Supervisor during the
job Debrief

Reports Submitted on Time


To meet the requirements of the Slickline Reporting Standard all field reports must be prepared and
submitted on time.
Daily reports, such as the daily activity summary, must be completed at the end of the working
day or shift, with copies sent to the base and submitted to the Client’s Representative
End of Job Reports requiring the Client Representative’s signature or input shall be completed
before leaving the location

All other reports shall be completed and available for review with the Base Supervisor for the Post Job Debrief
meeting

Post-job Debrief
After every field operation a Post Job Debrief meeting shall be held with the Base Supervisor and the Field
Supervisor and the crew, the primary objective of the meeting is to review and assess the effectiveness of the
Slickline Service Delivery and Client satisfaction as well as to review crew performance and the reports and
documentation generated in the course of the operation. The meeting shall be documented and signed off by
all participants.
