    3. BIR Certificate of Registration (COR)
    4. (New Applicant) - SSS Employer Registration Certificate
    (Renewal/Upgrading) – Official Receipts for the last 3 months
    5. (New Applicant) - PHILHEALTH Employer Registration Certificate
    (Renewal/Upgrading) - Official Receipts for the last 3 months
    6. (New Applicant) - PAG-IBIG Employer Registration Certificate
    (Renewal/Upgrading) - Official Receipts for the last 3 months
    7. AMO Seminar Certificate
    8. COSH Seminar Certificate
    9. Updated NBI Clearance – Original Copy
    10. Cedula 2022
    11. (Renewal/Upgrading) – Current Income Tax Return (ITR 2020) together with Financial
    Statements with Notes to Financial Statements duly signed by a Certified Public
    Accountant (CPA) with valid Board Of Accountancy (BOA) Certificate with Compact Disc
    Recordable (CDR) and PCAB Template F/S Imprints
    12. (New Applicant) – Current Bank Certification with Financial Statements with Notes to
    Financial Statements duly signed by a Certified Public Accountant (CPA) with valid
    Board of Accountancy (BOA) Certificate with Compact Disc Recordable (CDR) and
    PCAB Template F/S Imprints
    13. Certificate of Registration (C/R) and Official Receipt (O/R) of Motor Vehicles in the name
    of AMO/CORP.
    14. Deed of Sale of Heavy Equipment, if any, in the name of AMO/CORP.
    15. Photocopy of Land Title/Tax Declaration in the name of AMO/CORP.
    16. Two (2) pcs. 2x2 Size ID picture with nametag
    17. Resume’ or Bio-data: Date of Birth, Name of Complete Wife, SSS #, TIN
