    AFFIDAVIT
    I, YOLANDA B. BORNILLA, of legal age, Filipino and a resident of 175 Brgy. Motag,
    Malay, Aklan, Philippines, after having been duly sworn to in accordance with law, do
    hereby depose and state that:

    1. I am previously a lawful owner of a Glock 9mm Pistol, Model G26 Green, with
    Serial Number LVV756 and was duly issued of a Permit to Carry (PTC) with Control No.
    09113333 and Firearm License No. 92121811705595;

    2. Sometime in 2012, a colleague introduced a certain police officer, which name I


    cannot recall, from Metro Manila, who was interested to acquire the above-described
    firearm, with the undertaking that he will process all documents and all necessary permits
    for its transfer under his name. Immediately thereafter, he presented a Deed of Absolute
    Sale which I signed, and I gave him all original documents and licenses in connection with
    the aforementioned firearm;

    3. However, I was surprised when the Philippine National Police (PNP), Malay,
    Aklan approached me and inquired with regard to the licenses and non-renewal of the
    above-described firearm. I exerted diligent search and effort to locate a copy of the Deed of
    Absolute Sale but the same was nowhere to be found, as it might have been lost together
    with all my personal files and belongings during the devastation of the Typhoon Yolanda,
    hence, is considered LOST for all intent and purposes;

    4. I am executing this Affidavit in order to attest to the truthfulness of the foregoing


    facts.
