Good day!

We, the undersigned 3rd year BS Nursing students, are currently conducting a research study
“Caregiving to the Elderly in Batangas City Before and During the COVID-19 Pandemic”.

You will be asked to answer and complete the two sets of questionnaires that we provided. In
total, the two questionnaires comprise 40 items and will take approximately 10-15 minutes of
your time. Your participation is completely voluntary and you can withdraw anytime you please.
Rest assured that the information that you’ll provide will be treated with the utmost and sincere
confidentiality and will be used for academic purposes only.

For any concerns, questions, and clarifications, please feel free to contact Mark Jerwin G. Abella
