    2. Wash through needs dewatering after rain. (unsafe condition)
    3. Rigger found using cotton gloves. (unsafe act)
    4. PTW open and signed but pink copy not placed at CCB. (unsafe act)
    5. MLT concrete blocks lifting works without signboards (unsafe condition)
    6. TA gate access, workers logging the log book, full ppe & using pedestrian walkway (good observation)
