MEMORANDUM

SUBJECT : Investigation Report on Ambush Incident

DATE : December 12, 2019

I- AUTHORITY:

1. This case was investigated by the undersigned Investigator per verbal


instruction by the OIC, Ocampo MPS.

2. PNP inherent duty.

II- MATTERS TO BE INVESTIGATED:

3. To determine the facts and veracity regarding on the ambush incident that led
to the killing of P/CAPT. ARIEL ILAGAN, 36 years old, a resident of Hanawan, Ocampo,
Camarines Sur, transpired last December 12, 2019 at or about 5:29 PM in the aforesaid place.

4. To establish the identity of the perpetrators and be held criminally liable.

III- FACTS OF THE CASE:

5. Last December 12, 2019 at or about 5:50 PM team from Ocampo, MPS, got a
call from a concerned citizen that there was a shooting incident at Hanawan, Ocampo, Camarines
Sur. Immediately, team from this station under PO3 Alexander Rodriguez and PO3 Joshua P.
Colarina, Chief Investigator, together with the aforesaid unit, proceeded to the place of incident
to verify the report and to conduct an investigation.

6. According to investigation, the undersigned was able to talk to Mrs. Delia


Ilagan, who is the wife of the victim P/CAPT. Ariel Ilagan. Mrs. Ilagan said that last December
12, 2019 at about 5:25 o’clock in the afternoon, they already left house in their subdivision
together with their children aboard to a black Fortuner where they should be heading to the
house of their parents when suddenly she saw three men in front of their car, pointing and firing
a gun towards them. According to her, the victim shouted at her to grab their children and leave
the car, that’s when she instantly grab her children and went out of the car.

7. After firing multiple times at the victim’s body, the said unidentified men drove
away with a red color AUV going to unknown direction. By that time, Mrs. Ilagan kept on
worrying since she saw her husband full of blood in the car.

8. The initial assessment on the victim was, he was already dead when the
authorities came considering the gunshot wounds obtained by the victim were all fatal which
were done in a very short range. After lifting some pieces of evidence and gathering important
data, the body of the victim was already loaded to the police mobile patrol going to Ocampo
District Hospital for proper disposition. Thereafter, the attending physician of Ocampo District
Hospital declared the body of the victim dead on arrival. Thereafter, an authopsy or necroscopy
examination were conducted on the victim’s body (please see attached Necroscopy Examination).

9. The undersigned had conversation with Mrs. Ilagan, according to her there
were a lot of death threat that her husband P/CAPT. Ariel Ilagan were receiving since he was a
former Chief of Drug Enforcement Unit.
IV- DISCUSSION:

10. Based from the facts of the foregoing, the perpetrators managed to build-up
their ill-plan against the victim P/CAPT. Ariel Ilagan. Based on the CCTV footage that were
recovered on the area, five minutes before the vehicle of the victim has seen in the incident the
perpetrators riding an red colored AUV with a fake plate number were already on the said area
together with a man on a motorcycle.

V- CONCLUSION:

11. Through careful and deeper investigation, the ambush incident involving the
former Chief of Drug Enforcement Unit, P/CAPT. Ariel Ilagan were related to his former line of
work. Our state, as a democratic country, recognizes the fundamental constitutional mandate and
principle of due process of law, hence, we are obliged to follow that rule as we belong in the
same state.

VI- RECOMMENDATION:

12. The undersigned investigator strongly recommends the filing of appropriate


case against perpetrators if properly identified by the witness/es and if evidence warrants.
