    AFFIDAVIT OF TWO DISINTERESTED PERSONS


    (ONE AND THE SAME PERSON)

    WE, TERESITA C. MACAINAG and FELICIDAD D. URSUA, both of


    legal age, Filipinos, married and residents of and with postal addresses at
    Ili Norte, San Juan, La Union after having been sworn to in accordance
    with law hereby depose and state that:

    We personally know REGINA N. GARCIA, who is our long time


    barangay neighbor and acquaintance for more than 60 years, and the fact
    of her birth was registered at the Local Civil Registrar’s Office of San Juan,
    La Union having been born on July 6, 1959, but was actually registered as
    one, AQUILINA N. GARCIA, which she uses interchangeably in her
    professional and business transactions and we know this for a fact being a
    close acquaintance in our barangay;

    We also know and attest to the fact that REGINA, A.K.A. AQUILINA is
    married to ________________________ and both are residents of Ili Norte, San
    Juan, La Union since we have known them where they built their family
    home;

    In view of the foregoing, REGINA N. GARCIA pertains to the same


    person registered as one, AQUILINA N. GARCIA with the Local Civil
    Registrar’s Office of San Juan, La Union;

    We are not in any way related to REGINA, A.K.A. AQUILINA within the
    fourth civil degree either by consanguinity or affinity, nor do we have any
    pecuniary interest in the subject matter for which this affidavit may be
    used and/or introduced; and

    We are executing this affidavit to attest to the truthfulness of the


    foregoing statements for all legal intents and purposes it may serve.
