    First HPTA Meeting/Card Giving

    Grade 7 – Matulungin
    SY.2019-2020

    Minutes of the Meeting

    Date: August 23, 2019


    Time: 2:00PM - 3:30PM
    Venue: Building B - Room 202

    AGENDA:

    I. Class Schedule
    Class schedule of Grade7 is exactly 7:00AM until 2:00PM; their recess time is at 8:00AM until 8:20AM
    while their lunch break is at 10:20AM until 11:00AM. Reminders to all parents/guardians that the students
    are not allowed to go outside the school premises during the recess time and lunch break and is advised to
    bring their “binalot”. Flag ceremony is to be conducted every 6:30AM at the school grounds or/at their
    respective classroom.

    II. School Rules and Regulation

    A. Punctuality and Attendance


    For any absence, the learner must present a letter of excuse signed by the parent and/or
    guardian. A student who has been absent for more than three times successively must also be
    accompanied by his/her parents or guardian for the purpose of explaining the reason of the
    absence of the leaner.

    B. Proper Haircut
    Every first MONDAY of the month, male students are expected to have a proper haircut of neat
    barber’s cut with the tips of the hair not touching the ears. The school forbids hair spikes,
    artificially, colored hair, pony tails and other outlandish styles.

    C. Proper Uniform
    The students should wear their complete uniform. In case, that the students are not able to
    wear to wear their proper uniform, parents are advised to make a promissory note stating the
    reasons thereof, affixing their signature. The learners are also advised to wear black shoes and
    no high sock for female students.

    D. Students Requirements
    The learners are required to submit a complete set of requirements: photocopy of PSA,
    Card/Form 138, Form 137, 1x1 pic, 4pcs (white background, formal pose)

    III. HPTA Agenda


    Reminders for the contribution of P100.00 for each student that will be collected by the classroom
    treasurer to be remitted to the HPTA treasurer. Deadline is on September 06, 2019.

    IV. Card Giving


    One by one conversation with the parents/guardians was conducted during the card giving and a
    feedback for individual performance of the learners is observed. The parents is welcomed to share their
    concerns regarding their child’s performance in school. The parents are advised to take pictures of the
    cards of their child after affixing their signature.
    Since all the agenda was discussed and no furthermore questions were raised by the parents
    and/or guardians the meeting is adjourned around 3:30PM. The teacher acknowledge the presence of
    the parents and/or guardians who attended the said meeting.
