    Tooling was made incorrectly. The spool from, mold ABCTO100, Cav.1 and 1 were found to be out of
    specification. The Quality Department checked drawing for specifications and verified dimensions were
    correct. The Quality Department verified that material was in specification and not causing / contributing
    to this problem.

    The Quality Department first asked tool and die fabricator to rework the die to bring the cavity into
    specification. Fabricator said that the die had been surface treated and welded. Reworking the cavity
    would greatly reduce the life/overall quality of the die. Only corrective action we can take is to use a
    better fabricator for future dies.
