    I, MELODY A. MENDOZA, Filipino, of legal age, and a resident

    having been sworn in accordance with law, hereby depose and state
    thus:

    1. I am respectfully informing and requesting the Bureau of


    Customs, Account Management Office for the amendment and
    change of our principal address from:

    Certificate of Registration (AMO), with CCN: IM0004531078, valid until
    August 16, 2020.

    4. I am executing this Affidavit of Change of Address in order to


    amend and change our business address indicated in our Certificate of
    Registration in Bureau of Customs.
