AFFIDAVIT FOR CANCELLATION OF ENTRIES

IN THE TRANSFER CERTIFICATE OF TITLE
I, JANET C. ALLADO, of legal age, Filipino, single, and a

resident of and with postal address at Sto. Rosario, San Juan, La
Union, Philippines and presently residing at No. 18 Atis Street,
Admiral Village, Talon 3, La Piňas City, HEREIN represented by her
Attorney-In-Fact, ELMA BACANI CABADING, pursuant to a Special
Power of Attorney (attached herewith as ANNEX “A” and made
integral part hereof) after having been duly sworn to in accordance
with law, hereby depose and state, that:
1). That I am the true and registered owner of a certain parcel of

land which is covered by Transfer Certificate of Title No. T-6364
(attached herewith as ANNEX “B” & “B-1” and made integral parts
hereof), more particularly described as follows;
“A parcel of land (Lot 16 Block 14 of the cons. subd. Pcs-01-003674, being a
portion of Lots 50149, 10869, 12710, & 11608, Cad. 539-D, San Fernando Cadastre,
LRC Record No. N-64450), situated in Brgy. Biday Rurban Code No. 013314, City of
San Fernando, Island of Luzon. Bounded on the SW., along line 1-2 by Lot 17 BLKCK
14; on the NW., along line 2-3 by Road 10; on the NE., along lines 3-5 by Road 13; and
on the SE., along line 5-1 by Lot 15 Block 14, all of the cons.-subd. plan. Beginning at a
point marked “1” on plan, being N.50-04 E., 2333.89 m. from BLLM No. I, Cad. 539-D,
San Fernando Cadastre, thence N.02-01 W., 10.29 m. to point 2; thence N. 87-59 E.,
13.00 to point 3; thence S. 47-01 E., 2.83 m. to point 4; thence S.02-01 E., 8.29 m. to
point 5; thence s.87-59 W., 15.00 m. to point of beginning, containing an area of ONE
HUNDRED FIFTY (152) SQUARE METERS.
All points referred to are indicated on the plan and are marked on the ground
by PS cyl. conc. mons. 15x50 cms., bearings Grid; date of original survey, June 11-
September 21, 1977 and that of the cons.-subd. plan executed by December 5, 2004”.
2).That annotated on said Transfer Certificate of Title No. T-

6364 are the following entries, to wit:
“Pursuant to Sec. 7 of Republic Act NO. 26, this certificates of titles are without
prejudice to any party whose right or interest in the property were duly noted on the
original reconstituted certificates of titles RT-3143 (48535) RT-3144 (48536) ro-240
(2761) at the time they were lost or destroyed, Order reg. under Entry NO. 27572. Date
of Inscription , November 23, 2004 at 2:00 P.M. (SGD) PABLITO M. VALDEZ, Register of
Deeds.
Subject to Deed of Restriction under Entry No. 38691 executed by Eso-Nice

Realty Development Inc., represented by its duly authorized representative Reynaldo
Q. Nisce, General Manager and Janet C. Allado, Buyer, duly notarized by Ronella
Balbin Concepcion of San Fernando City, La Union under Doc. No. 431; Page No. 87;
Affidavit of Cancellation of Entries in the Transfer Certificate of Title Page 1 of 2

Book No. 19; Series of 2006. Date of Instrument-January 11, 2006. Date of inscription-
January 27, 2006 at 10:40 A.M.”
3.That the effectivity and efficacy of said entries have since

expired as shown by the dates thereon anent the Deed of Restriction
as described and are being caused to be canceled in view of the fact
that the party to said annotation has registered no opposition to its
cancellation and that affiant has already complied with all the
requirements appurtenant thereto;
4.That I am executing this affidavit purposely requesting the

Register of Deeds to cause the cancellation of the above-mentioned
entries in the aforementioned Transfer Certificate of Title;
