FLOW CHART NO.

1—APPLICATION FOR REGISTRATION OF INDIVIDUAL NEW


BUSINESS

Processing of Application for Tax Identification Number (TIN) and Registration of


Individual New Business Registrants (Head Office only), including Facilities
Together with the Application for Authority to Print (ATP)

Secure all necessary


documents for application
for registration as self-

for Registration- Checklist
of Documentary

Submit to the New


Business Registrant
Counter (NBRC) all
the required
documents

Pay RF thru
NBRC, including
other liabilities
and penalties

Wait for the


release

Step 1:
Secure all necessary documents for application for registration as self-employed
individual ([See Annex A1]—Application for Registration- Checklist of Documentary
requirements)
a. Obtain and fill out BIR Form No. 1901 (]See Annex A2]—Application for
Registration (For Self-Employed and Mixed Income Individuals)
b. Obtain any identification duly issued by an authorized government body that
shows the name, address, and birthplace of the applicant ([See Annex A3]—
Birth Certificate, Passport, Driver’s License, Community Tax Certificate)
c. Obtain photocopy of Professional Tax Receipt issued by the Local
Government Unit (LGU) [See Annex A4]
d. No need to secure Mayor’s Permit or DTI Certificate
e. New sets of permanently bound books of account—Notarial Register
f. Proof of payment of Annual Registration Fee (ARF)—applicable after TIN
issuance herein
g. Obtain and fill out BIR Form No. 1906 [See Annex A5]--Application for
Authority to Print Receipts/Invoice
h. Final and clear samples of Principal Receipts [See Annex A6]

Step 2:
Submit to the New Business Registrant Counter (NBRC) the following:
a. All the required complete documentary requirements;
b. Signed Checklist of Documentary Requirements (CDR);
c. Duly accomplished BIR Form No. 1901 in duplicate (2) copies;
d. Duly accomplished BIR Form No. 0605 for payment of Registration Fee (RF),
if not yet paid, in duplicate (2) copies.

Step 3:
Pay RF thru NBRC, including other liabilities and penalties, if applicable:
a. Annual Registration Fee (RF)-- P 500.00
b. Documentary Stamp Tax (DST)-- P 30.00

Step 4:
Wait for the release of the following documents:
a. Certificate of Registration (COR);
b. Ask for Receipt Notice (ARN)
c. eReceipt as proof of payments of the RF and/or other tax liabilities or
penalties.

Step 1:
Secure all necessary documents for Application for Request for Authority to Print
Receipts/Invoices

a. Manual Bound Receipts/Invoices


1. Obtain and fill out BIR Form No. 1906 [See Annex A5];
2. Final and clear sample of Principal and/or Supplementary Receipt/Invoices;
3. Photocopy of last issued Authority to Print (ATP) or PCD; or any booklet from the
last issued ATP for subsequent application.

b. Manual Loose Leaf Receipt/Invoices


1. Obtain and fill out BIR Form No. 1906 [See Annex A5];
2. Permit to Use Loose-Leaf Official Receipts or Sales Invoices;
3. Final and clear sample of Principal and/or Supplementary Receipt/Invoices;
4. Photocopy of last issued Authority to Print (ATP) or PCD; or any booklet from the
last issued ATP for subsequent application.

Step 2:
Submit Checklist of Documentary Requirements (CDR) and duly accomplished BIR
Form No. 1906, together with the required complete documentary requirements

Step 3:
Wait for the release of approved ATP and copy of BIR Form No. 1906

a. Letter Request;
b. Authorization letter (for individuals requesting for the certification)

Step 2:
Submit letter of request together with the required documents
Step 3:
Pay the necessary fees:
a. Certification Fee- ₱100.00
b. DST- ₱30.00 per document

Step 4:
Wait for the release of requested documents

ANNEX A2- Application for Registration (For Self-Employed and Mixed Income Individuals)
ANNEX A3- Requirements for obtaining Birth Certificate, Passport, Driver’s License,
Community Tax Certificate

1. PSA Birth Certificate Requirements:

A. Application Form (provided at PSA)


B. Certificate Fee (P 155.00)
C. Valid ID for both the owner and requester of document
D. Authorization letter and ID of the document owner together with the ID of the
requester are required if the requester is not the following:
a. document owner;
b. parent;
c. spouse;
d. direct descendant;
e. legal guardian/institution in-charge, if minor.

2. Driver’s License Requirements:

A. General Requirements:
a. Duly accomplished original Application Form for Driver’s License
(ADL)
b. Electronically transmitted medical certificate in LTO prescribed form
(present hard copy)

B. Additional Requirements: NON-PROFESSIONAL DRIVER'S LICENSE (NPDL)

a. Original valid Student-Driver's Permit


b. In addition, for foreigner, original and one photocopy of passport with
entry date of at least one (1) month and visa duration of at least six
(6) months from date of application, or if born in the Philippines,
original and one photocopy of birth certificate duly authenticated by
National Statistics Office (NSO) / Philippine Statistics Authority (PSA)

3. Passport Requirements:

A. Confirmed Online Appointment


B. Personal Appearance
C. Accomplished Application Form
D. Original and photocopy of Philippine Statistics Authority (PSA) Authenticated Birth
E. Certificate on Security Paper
a. Married Females (who are using their spouse's last name)
must also present Original and submit photocopy of PSA
Authenticated Marriage Contract on Security Paper or Report of
Marriage
b. Local Civil Registrar Copy is required if PSA Birth Certificate is not
clear or cannot be read
F. Any of the following acceptable IDs with one (1) photocopy
a. List of Acceptable IDs for Passport Processing:
✔ Social Security System (SSS) Card
✔ Government Service Insurance System (GSIS) Card
✔ Unified Multi-Purpose Identification (UMID) Card
✔ Land Transportation Office (LTO) Driver’s License. Student
Permit may be accepted if in card format.
✔ Professional Regulatory Commission (PRC) ID
✔ Overseas Workers Welfare Administration (OWWA) E-Card
✔ Commission on Elections (COMELEC) Voter's ID or Voter's
Certificate issued from COMELEC main office in Intramuros,
Manila (Certificates issued in COMELEC's regional offices may
require further verification. Applicants are requested to present
NBI or police clearance along with the Voter's Certificate).
✔ Philippine National Police (PNP) Permit to Carry Firearms
Outside Residence
✔ Senior Citizen ID
✔ Airman License (issued August 2016 onwards)
✔ Philippine Postal ID (issued November 2016 onwards)
✔ Valid or Latest Passport (For Renewal of Passport)
b. For minor applicants, School ID OR Certificate of Enrolment with
photo of minor and dry seal of school may be presented
c. For applicants based overseas, they may use their host government
issued IDs showing their Philippine citizenship. (Example: Residence
Card)
● All IDs presented should be readable, untampered and contains consistent
information with the documents presented upon application.
● Original and photocopy of IDs and supporting documents are required to be
presented upon application.
● Important Reminder: The Department may require additional supporting documents
as may be necessary.
● NOTE: If woman opts to retain maiden name, a Marriage Contract is not required.

4. Community Tax Certificate Requirements:

A. The community tax is paid in the place of residence of the individual


B. At least 18 years of age, or over
C. Has been regularly employed on a wage or salary basis for at least one (1) month
during any calendar year or who is engaged in any business or occupation, or who
owns real property with total assessed value of at least P1,000.00, or who is
required by law to file an income tax return.
D. The tax is primarily collected by the City or Municipal Treasurer.
