    SUBJECT : LETTER REQUEST FOR CHANGE OF SCHEDULE OF


    MANDATORY LEAVE

    DATE : 10 May 2019

    1. In connection with the fire service, the following personnel attached hereunder
    would like to request changes on their scheduled Mandatory Leave whish was
    submitted at your good office.

    2. The Mandatory Leave Scheduled is sought to be changed for the following


    reasons, to wit;

    A. That, the personnel of this station encountered changes in their duty


    schedule due to the demands of public service, And;

    B. There are some personnel who are about to undergo FBRC and FAIIC
    which then again caused the changes in the anticipated duty schedule

    3. Listed hereunder are the names and schedule of mandatory leave that area sought
    to be changed, to wit;
