SUBJECT: Comments on the Alleged Irregularities of Services of BFP

Personnel for the Fire Victims in Purok 3, Brgy. Apo Macote,
Malaybalay City.
1. Reference: Memorandum from BFP Regional Office 10 dated 29

January 2020 Submission of Comment RE: Alleged irregularities of
Services of BFP Personnel for the Fire Victims in Purok 3, Brgy. Apo
Macote, Malaybalay City.

In response to the complaint of the alleged Irregularities of Services of
our investigation unit received by this office OOA 1630H 29 January
2020 from the office of the Provincial Fire Marshal, Bukidnon BFP Unit.
This office sincerely apologize to Mr. Lito Serino (fire Victim) for the
disappointment and inconvenience our office has caused.
3. According to records available and facts presented before me, I

believe our personnel were doing their job as mandated by law and
responded to the fire call from Barangay Captain Julie Olili of Apo
Macote OOA 1444H 19 January 2020 involving the residence of Mr.
Lito Serino. In fact, two (2) of our fire engines were immediately
dispatched, 1. Aglayan fire sub-station which is approximately 21Km
away from the fire scene 2. Rosenbauer Fire truck with complete crew
on-board with 34Km approximate distance, and declared fire out
OOA1524H 19 January 2020.
4. As disclosed by the assigned investigator FO2 Renato A Mahilum Jr.,

on that same date of the fire incident, the initial investigation revealed
suspected tampering of electrical circuit has caused the fire which
damaged the residential structure owned by the complainant/fire
victim. This findings were later on confirmed last 30 January 2020 by
the owner himself during the verbal interview by the undersigned but
refused to admit on his judicial affidavit. While his son Mr. Jonly P.
Serino willingly testified and on his judicial affidavit, which he signed
without constraint.
5. Just like any other fire incident investigated, in accordance to RA9514

the Chief IIS restricted any re-entry of unauthorized personnel to
preserve and protect the evidential value of identified focal point of fire
and avoid evidence spoliation, to conduct thorough analysis of fire
spread pattern.. This order may have caused inconvenience on the
part of fire victim, but is necessary to guarantee verity of the result.
6. That on that mentioned date 21 January 2020, claimed we deliberately

ignore our duties to conduct investigation, the undersigned directed
FO2 Renato A Mahilum, Jr. assigned Fire Arson Investigator of subject
fire incident and the rest of the SRU Team Bravo abided to the memo
order dated 17 January 2020 of SUPT REXCLIBURN L SALVAÑA,
Provincial fire Marshal RE: Schedule of New Year’s Call/ Courtesy Call
CY-2020 to HON. GOV. JOSE MA. R. ZUBIRI JR. Provincial
Governor of Bukidnon scheduled 21 January 2020 0800H.
7. That the follow-up investigation was then conducted on 23 January

2020 three (3) days after the fire incident. The undersigned did not find
any basis of negligence in the part of MCFS IIS in their investigation
procedure as claimed by the complainant.
8. That per findings gathered and thorough investigation, the fire incident
was found to be accidental in nature and therefore considered closed
unless new evidences will arise that will warrant reopening of the
subject fire incident. Hence, this office issued Fire Clearance to the fire
victim last 27 January 2020.
9. Anchored on our organization mission, please know that this office

handle all fire incident consciously without prejudiced to confidently
deliver an accurate findings in any conduct of investigation, and that
there is no intention or motive to neglect and disregard the needs of
our client.

proximate fire sub-station and has effectively respond to all form of emergency calls
in the southern part of the city attaining our organizations mission and vision.
2. In view of, we are pleased to informed you that our personnel and 1 unit Isuzu with
Plate No. SCL153 is already housed at the newly constructed Kalasungay fire sub-
station and has successfully convinced the LGU Malaybalay through Local Executive
Hon. Ignacio W. Zubiri for the immediate installation of the Water and Electrical
connection including monthly bill allocation charge to LGU Malaybalay.
3. To date, water and electrical connection is already effective, the City Engineering
office which is in-charge of the Program of Works is currently at the phase of
concrete walling of the subject fire sub-station.

1. This in reference to our memo dated 25 January 2019 Re: Request for Additional ISO
FSIC Forms.
2. In compliance to the newly enhanced law, the Republic Act No. 11032, known as the
“Ease of Doing Business and Efficient Government Service Delivery Act of 2018” and
to diminish complaints on delayed government transactions, this office opted in
printing the same forms in lieu of the FSIC FORMS until it will be available.
3. To date, to reach our target in MATAM, the FSES conduct thorough Fire Safety
Inspection/verification to all compliant/non-compliant business establishment and
led to increased number of FSIC issuance.
