4. Approve revision to Board Policy 5101. The district will pay 85% of the cost of the single plan,
employee plus one dependent, or family plan: Administrators will be responsible for 15%.

5. Approve two new high school courses of study for the 2009-10 school year: Calculus III and Latin IV.
6. Approve the service agreement with Atrium Medical Center for athletic training services. Athletic
training services will be provided at all high school and junior school campuses. This agreement
coincides with the approved Atrium marketing agreement. The cost to the district is a $1.00 per year.

7. Approve a trip for Lakota East High School Junior Statesmen of America to Washington DC from February
12th – 15th, 2009 to attend Winter Congress. The students will not miss any school days. There is no cost
to the district.

8. Approve the acceptance of a $300.00 Miami University grant awarded to Cherokee Elementary School

9. Approve the acceptance of a $500.00 Community Foundation of West Chester – Liberty grant awarded
to Lakota East High School and authorize the treasurer to establish the appropriate fund. Funds from
this grant will be used to purchase materials and supplies for the Grief Support group at Lakota East
High School.

10. Approve the acceptance of a $14,914.11 Butler Health Plan Wellness grant awarded to the Lakota Local
School District and authorize the treasurer to establish the appropriate fund. Funds from this grant will
be used to purchase various health related supplies and materials.

11. Accept donations as presented in Appendix A.
