The map "Monolith" was actually designed primarily by BlazeDillon. This was the first of several maps of his published officially with the launch of the game. Others include maps in "Shooty Squad" and Maps in Halo's matchmaking.
The Reflex game in the movie is actually a real game, released in 2014
