In the beginning of the movie when Nick and his wife witness the traffic collision, both cars are seen skidding to a stop on their wheels. In the next scene after Nick has his wife get his medical bag, both cars are upside down and on fire.
(at around 22 mins) The bumper falls off the car that Nick Malloy is driving but in the next shot, the bumper is reattached to the vehicle.
When Heller first calls Nick in the ambulance, Nick is wearing gloves. When we see the closeup of the phone, the gloves are gone. When the camera goes back to showing Nick, the gloves are back.
When Nick is beating Tommy in the hotel room, the phone rings. In a close-up, the telephone is straight on the table but in the next shot it has moved and there is a wire over it which was not there before.
After Tommy's ankle bracelet comes back online and Nick is fighting with 2 cops, Tommy runs away and Nick drags him back to the police car. A long shot of the car shows the rear driver side door open. The shot tightens in and suddenly the door is closed.
When Nick and Tommy are driving out of the parking lot of the 'hotel', the Harbour Centre can be seen in the city background. The Harbour Centre is located in Vancouver, British Columbia, Canada. Although it is not made explicitly clear in the movie where it is set, the ambulances used the film do not have the British Columbia Ambulance Service color scheme; thus the setting isn't Vancouver or anywhere else in British Columbia.
In the beginning of the movie, when the cars flip over, Nick says the seat belt is stuck. He's carrying a medical professional's emergency kit. That kit would have had "gauze scissors" inside of it to cut the seat belt.
