Professor Cha is a brilliant thoracic surgeon with an arrogant, icy and clinical personality. Go Eun Tak is a genius intern with a warm, cheerful, and mischievous personality. He is also the son of the director of the hospital with no practical skills. On his first day, Go Eun Tak annoys Professor Cha by questioning his arrogance. Professor Cha retaliates by embarrassing his intern during surgery. After performing surgery on his ex-girlfriend's father, against his own advice, he rushes out for unknown reasons and gets in a car accident.

Professor Cha has trouble adjusting to being a "coma ghost". He meets the chairman, whom he operated on last (also as a "coma ghost). He begins to suspect that something went deliberately wrong after the surgery. He also meets another ghost that helps explain the rules of what he needs to do to stay alive.

The hospital holds a conference to determine what went wrong with the chairman's surgery. Professor Cha is ready to take over Go Eun Tak's body to make him confess. At the last moment, Go Eun Tak revealed that he never gave the chairman heparin, and the nurse confirms this. The Vice President of the hospital, Go Eun Tak's brother, attempts to transfer Professor Cha. Go Eun Tak stalls for time by moving his bed, until he is caught by his brother. However, Dr. Jang, the ex-girlfriend and chairman's daughter, claims Dr. Cha as her patient. It is also revealed that Dr. An is the one that administered the heparin and then hid from receiving calls from the emergency room.

The chairman reveals some secrets to Dr. Cha over a bottle of soju, including the fact that he is the one that sent Dr. Cha to Seattle. After it is revealed that the chairman has not woken up, the chairman's lawyer shows up to unveil a will (to be read later) and a dnr. Dr. Jang agrees to her fathers wishes and bids him farewell.

Chairman Go comes to the hospital. It is revealed that he has an old friendship with Tes. As we also learn, Tes is Dr. Oh's grandfather. We begin to learn more about the coma ghosts, including Bo-Mi, a phd candidate, that may have an infection, causing the coma. Seung-won begins to suspect Dr. Cha might really be possessing Seung Tak and sets up a surgical trap.
