In this dog eat dog world, you have to stand up and carve a living for yourself, lest you want to get trampled by others in the rat race. This is the dilemma Ditas has to face. Born in a destitute family, Ditas is left with no choice but to struggle with the daily demands of life. However, Ditas is also born with a strong resolve to make it in life, no matter what. As such, she didn't let her stature in life hamper her from building a promising future for herself. She makes a living by signing her own baby for money, a job that didn't bring her problems until a well-off couple approached her one day. This is when picture-perfect couple Rodel and Fina enter the picture. Young, good-looking, rich... these two couldn't wish for anything more, as it seems that they have everything anyone could want. Unfortunately, both Rodel and Fina are eager to start a family, something that's nearly impossible as medical complications prohibit Fina to bear children. Soon, they hear stories about a certain baby-maker, who in the end turns out to be Ditas.

Carissa (Heart Evangelista), a woman born with a face resembling a monkey. Carissa and her mother, who is also disfigured, experience abuse and mistreatment as a sideshow in a town perya, where they were abused and mistreated. Despite of this, she was defended by Mendez, a man who befriends the disfigured Carissa and who eventually falls in love with her despite her feature. Finally finding a chance to escape, Carissa is nearly run over by the car driven by Hugo (TJ Trinidad), a renowned plastic and cosmetic surgeon, who was also at the time working on a new, revolutionary surgical procedure. Also in the car is Alicia, Hugo's beloved wife, who shortly after the incident, dies in a tragic airplane accident. When fate decides to cross Hugo's and Carissa's paths anew, Carissa decides to take Hugo up on the offer to be a test patient for Hugo's new procedure. Hugo, meanwhile, decides to mold Carissa's new features on that of his dead wife. After having a charming new look, Carissa returns to her hometown to seek revenge over the locals who made her life a living hell. But can Carissa truly be happy with a face - and a life - borrowed from a woman long dead? How will Hugo live with a woman who shares his wife's face? And what will happen to her relationship with Mendez, a young man who was the only person to see the true beauty behind Carissa's "ugliness?"
