(at around 7 mins) When Jill Valentine first enters the police station towards the beginning, after being called back on duty, and after she has finished firing her gun, she reloads it and you can see the magazine being slammed into the gun. Right after she says "I told you, shoot for the head." she slams the magazine into the same gun again. She does not use nor possess two guns in this scene.
(at around 1h 14 mins) When Cain asks why the helicopter hasn't taken off and LJ says it's because he usually drives a Cadillac, LJ starts to throw a punch with his left hand. Then the camera cuts to a shot of Cain's face, where we see him get hit by a right-handed punch.
(at around 21 mins) When Jill heads off to inspect the church in the initial scene, she pulls out a Beretta M-92 Handgun. When she is startled by the priest, she turns around and is now holding a Colt 45.
(at around 45 mins) When Alice falls down the chute to avoid Nemesis, she is shown breaking her ring finger on her left hand. When she is in the hopper and putting her hands to her head, you can clearly see that all 10 fingers are intact and unbroken. When she resets the finger, it is then the ring finger of her right hand.
(at around 1h 16 mins) In the helicopter scene near the end, the fight and the helicopter taking off are in pitch black/night time. We even see the helicopter in the air in the dark. Within 60 seconds however it is broad daylight.
Several errors were made regarding nuclear power. First, no nuclear weapon detonation could ever be mistaken for an accident at a power station - power reactors cannot create nuclear explosions like weapons. Second, "critical" describes a reactor at normal, steady state operation, it does not mean an accident is occurring or imminent. Finally, the "accident" is described in the movie as the worst accident since Chernobyl. According to the UN, Chernobyl has only caused 56 deaths to date and the final death toll for the accident will be around 4,000. The destruction of a city the size of Raccoon City and the death of all its inhabitants would be far worse than the Chernobyl incident.
(at around 27 mins) When they are in the church and Alice rides in on her motorbike, she jumps off it and when it hits the licker, she shoots the bike's fuel tank and it explodes. This is a common mistake in movies, bullets aren't hot enough to cause ignition and an explosion in a petrol tank.
Near the beginning of the film, when the 10 or so Suburbans speed past a few pedestrians in a quiet residential area, no one seems at all surprised by their presence and looks. The paperboy doesn't even react, although they drive past him just a few feet away and could easily have killed him.
At 18:53 (measurement made using the Hulu commercial-free stream), at nighttime (note the street light reflected in the storefront's windows), Alice pulls the door open and enters Fremlin's Surplus and More, which is deserted. There are no locks on the door, and no alarm sounds, even though the inventory consists of, among other things, over a dozen operational semi and fully automatic hand guns, along with the cartridges necessary for them (this is confirmed when she comes to the aid of Jill and company in the church and fires the handguns and Uzi-type weapons). So you have an unlocked entrance, no alarm, and an unlocked weapons case, all in a city that is basically under martial law. No owner of a store with such inventory would leave it in an unprotected condition like this, even if he.she were in a hurry to leave, since he/she would be legally liable for any crime committed by the deadly weapons.
Geny Walters is credited as "Mrs. Priest" for the film. She is the Priest's sister in the film, not his wife. Furthermore, Priest is a title, not a formal name.
The movie only implied that the spread of infection was due to the Hive being re-opened. A faux newspaper created by Screen Gems for Apocalypse, The Raccoon City Times, indicates hours after the initial outbreak, creatures began appearing in the Arklay Mountains feasting on victims.
(at around 44 mins) During the first fight between Alice and Nemesis, Alice is shown jumping over a fence. When she reaches the peak of her jump, you can see that the actress (Milla Jovovich) is desperately trying to untangle her arms, which have obviously gotten caught in the wires of the harness used to make her jump.
The slowing main rotor of the rooftop rescue helicopter is turning in the wrong direction.
(at around 1h 13 mins) Obvious stunt double when Alice is running around the round glass building with its windows being shot out.
In the scenes where Carlos and his team are fighting zombies in the city you can clearly tell that the scenes have been flipped because the Pizza Pizza signs have been reversed.
When Carlos is talking, just before getting into the semi, and driving into the horde to explode, traffic can be seen in the background.
When Jill walks into the police department she sees LJ handcuffed to a bench. She then shoots his handcuffs to release him. She's a cop in a police department so she could've just unlocked them with one of numerous keys in the place.
The woman on top of the building trying not to go over the edge as she's trying to fend off the zombies makes no sense. She clearly didn't want to fall off but waits for someone to kill all the zombies before turning around and jumping off anyway. If she was already bitten then she should've just jumped & not wait for someone to save her.
(at around 14 mins) When the man on the bridge announces to the crowd, "The use of ammunition has been authorized... in five," his mouth moves to "10" instead of "five". This countdown was originally going to be a 10 second countdown, but it was shortened to five.
(at around 36 mins) After the sniper on the roof shoots the zombie behind L.J., he makes the hand action to pull back the bolt of the rifle and the sound of it is heard but the bolt itself does not move.
(at around 46 mins) When Jill is trying to start the pickup truck (a Dodge), the engine turn-over sound is nothing like the real thing.
(at around 27 mins) After Alice has jumped of the bike in the church, you can hear that the bike accelerate by itself.
As the S.T.A.R.S officer sees the helicopter dropping supplies into the hospital, and says "it could be a radio" a person (crew) is very obviously seen walking in the background.
(at around 4 mins) When the first scientist is picked up by the agents you can see a boom operator wearing a white shirt reflected in the door just as she opens it.
(at around 5 mins) When Dr. Ashford's daughter is riding in the SUV, you can briefly see a tow truck pulling the vehicle as slides across the ground after being hit.
(at around 5 mins) When the car that Professor Ashford's daughter is in rolls, you can just briefly see the camera mounting equipment on the back of the car.
(at around 34 mins) When Carlos and Nicholai are signaling to the helicopter in the alley, you can see, on the ground in the background, a spotlight that is being used to light the scene.
(at around 5 mins) Although in most scenes the CN Tower has been edited out, near the beginning when the van with Angie in it crashes, you can see the CN Tower in the background.
Although Raccoon City is supposed to be in the U.S, in most of the city shots where it shows the larger buildings you can see the CIBC (Canadian Imperial Bank of Commerce) and Bank of Montreal logos.
Where the SUV flips, you can see a Scotiabank logo on a building sign. It's a Canadian company and the film is set in the US.
There is no possibility that the T-Virus can effect the bodies in the cemetery. As stated in the first movie, the T-Virus needs a body with residual neural activity, the bodies of the buried would have been dead for weeks. Furthermore, the T-virus would have to have been in direct interaction the a corpse, which are in sealed caskets inside a concrete chest.
The T virus is transmitted by biting the living and infecting them, yet the buried corpses in the cemetery reanimated. It couldn't have been that they died from bites and were buried, as many are in a state of advanced decomposition, and the movie is set 24 hours after the first movie.
There is no way that Terri's tape could have been written off as a hoax, even if Umbrella had nuked all evidence of the T-Virus with the missile destroying Raccoon City. As shown in Jill's apartment, there had already been news reports on television showing the initial outbreak and even video evidence of zombies attacking humans. The evidence was already there, even before her tape was recorded. Furthermore, there were several bits of footage collected on the tape to corroborate the news footage that could not possibly have been faked, such as the appearance of the lickers, the zombies at the graveyard, and even Terri's death where she's devoured by zombie children.
Unfortunately, this is a matter for debate, not an absolute fact. Despite an over-abundance of evidence and proof to the contrary (scientific studies, textbooks, newsreels. film, and so on), there are still plenty of people who retain minority viewpoints on a number of subjects. Therefore, the existence of news reports on TV could be denied, even with eyewitness testimony. All that Umbrella had to do was create enough doubt that they could have plausible deniability.

The classroom where Terri was devoured by zombie children was completely empty, save for the one zombie girl in the corner she mistook for Angela. When she backs up there's suddenly a horde of zombie children behind her. Where did they all come from? There was no way they could have all been hiding in a single spot in the relatively small classroom, unnoticed, together, without making any noise. They quite literally popped out of nowhere in the shot.
Where did Angela go after the Umbrella van crashed? She simply disappeared and never made any effort to contact her father or any Umbrella employees. The film seems to imply that she went directly back to the school after the crash, but if that's the case, why didn't she tell the school staff what happened? And if she didn't immediately go back to the school, then where was she until nightfall? She never explains her sudden disappearance to anyone.
Terri's death is entirely Jill's fault. Being a trained and seasoned police officer, she would never send a clearly unprepared and vulnerable civilian to go off alone armed with a handgun she doesn't even know how to use, in a building potentially filled with zombies no less, especially after she showed numerous times prior that she can't fight and easily panics. Furthermore, she would never suggest that the three of them (Jill, L.J. and Terri) split up to search the school.
Olivera shouts grenade immediately before lobbing a grenade at the approaching zombies. Any militarily trained soldier would say frag out or similar. Shouting grenade is a warning to fellow soldiers that a grenade has landed in their immediate vicinity and that they should take cover.
If Angela had been hiding in the school for a hot minute, then why didn't she warn any of her rescuers about the zombies or other creatures that are lurking around the building? She waits until the last minute when they're being attacked to give any kind of warning.
