When the thugs attack David Banner and his co-worker Roy at the lab right before the second Hulk out, Mike screams at Roy and tries to shoot him. Banner grabs Mike's arm and causes him to shoot one of his thugs in the chest instead. However, within a few seconds after David starts to turn into the Hulk, the thug who was shot is uninjured and is helping the thugs attack Roy.
When Donald Blake reveals himself to David Banner at the Joshua Lambert Institute, Blake explains that when he first met Banner, he had a beard when was a guest speaker at Harvard ten years ago. But 10 years prior to the events in this TV movie, David did not have a beard and he was already thought dead by the world while secretly looking for a cure of turning into the Hulk.
When Thor and the Hulk fight in the main laboratory of the Joshua Lambert Institute, the Hulk throws Thor out of a window at the right/northern side of the room where the Transponder controls and coffee counter are. However, the next day when David Banner and Donald Blake are making repairs in the lab, the broken window has been boarded up, but now it's at the left/southern side of the room where the damaged control systems are.
After Thor says "I think I made it angry", the Hulk throws him through a window of the main lab and Thor lands on the roof outside. However, when the Hulk is in the broken window looking out at Thor, the Hulk is in a new room and not in the main lab.
When Donald Blake is in the cave and first finds Thor's hammer inside his sarcophagus, he mentions the room was filled with thunder and the lid of the sarcophagus was blown off on its own. However, strings attached the lid are clearly visible when they pull it off of the sarcophagus.
The Hulk is wearing footwear a the foundry.
When David Banner confronts Josh Lambert and gets smacked in his face with a gun (final action scene at the factory) you can actually see the movie team being reflected in the car door when Lambert drives the car forward.
In the fight on the beach when Thor and Hulk fight the goons from the copter, the green make-up on the Hulk's face is noticeably lighter than that used on his body.
