In the scene where the coffin is being screwed close the operator of the drill is cranking the drill in the wrong direction, to tighten a screw down would require a clockwise twist.
At exactly 16 minutes (in the Criterion DVD) as the camera pans right, there is a reflection in a glass window of the camera operator cranking the camera.
