The most graphic sex scene occurs in the films closing minutes where we see a man aggressively thrusting into a woman (his bare buttocks are shown) on the floor. We see this on and off throughout the extended scene but some of the angles are quite explicit although anything other than buttocks is hard to catch.

A pregnant woman uses a knife to stab her belly with blood shown. She then slices herself repeatedly on her arm and then on her face while being told to by a group of people. Extremely graphic and disturbing.
A young pregnant woman is kicked in the stomach. Later on, she punches herself in the stomach to prove that she was pregnant (blood on her hands) and cuts herself (face and arms) as the other dancers encourage her to do so. The scene looks insanely realistic, as it's done in one take.

A woman's hair is set ablaze. We see/hear her howling in pain several times, and she is seen splashing water onto her burnt scalp. This is very graphic and detailed at one point.

Climax is an extremely dark film and not for the faint of heart. The depiction of an intensely bad LSD trip is insanely terrifying and heart-racing. As the dancers descend further into drug-induced insanity, the movie gets progressively more disturbing, graphic, and deranged.
Extreme and horrific panic inducing imagery from the 45 minute mark onward.
Definitely one of the most intense and terrifying movies ever.
