Although the plot begins in 2017 and continues on to 2020s, it is set in an alternate future. A chain of events that start before, eventually unfolds unto the show we see starting in 2017.

One difference that has already been explained is the extreme racism of Gilead as described in the novel. Anyone other than white, non-Jewish people in the novel's Gilead are slaves or what one wit referred to as "disappeared".
