The destruction of the ER that resulted in the grand opening scene here took place in I Am the Apocalypse (2015).

Goofs
A patient's heart rhythm was asystole, as heard by the steady alarm of the cardiac monitor. However, the doctor choose to defibrillate her. Defibrillation does not work on that heart rhythm. This is a common mistake made by many medical shows.

Train wreck

You'd think the writers would be smart enough not to start their series off with a literal train wreck when the entire show quickly flies off the rails and becomes a figurative one. I will give the actors some credit for keeping a straight face when having to deliver some of the worst lines ever written for television.
